Incident Date: February 12, 2024, at 1:00 PM Location: [XXX] Vehicle Model: Tesla Model 3 VIN: [Insert VIN of the Vehicle] License Plate: [Insert License Plate Number] Description of the Incident: On [XXX], while driving westbound on [XXX] at a speed of 35 MPH, [XXX] experienced a sudden and dangerous malfunction of the Tesla Model 3. Despite the driver's best efforts to maintain control, the vehicle autonomously shifted from the innermost lane to the outermost lane, jumped over the concrete sidewalk, and fell into a trench. Fortunately, no other vehicles were involved, preventing what could have been a fatal accident. The police report filed with the Suwanee, GA police department documents the incident, and the car was subsequently towed to a body shop, where it was declared totaled. It is essential to note that the auto-drive feature was disabled, and no subscription to the self-driving feature had been purchased or renewed. This raises serious concerns about a potential malfunction in Tesla’s system, and it is imperative that this issue is thoroughly investigated. Attempts to Resolve: Since the incident, numerous attempts have been made to contact Tesla for a root cause investigation and resolution, but Tesla has failed to provide any substantive response or investigation. Below is a history of our efforts: 1. March 3, 2024: Incident claim submitted during a call with Raquel. No claim number received. 2. March 14, 2024: Called Tesla Customer Service and spoke with Anthony. Case was assigned to Duluth, GA service center, but no case number was provided. 3. After Tesla Berkeley performed an alignment on my 2024 Model Y on July 22, 2025, the car suffered a complete systems failure while driving on the freeway. Within minutes of leaving the service center, multiple safety systems shut down at once — steering assist, emergency braking, lane keeping, stability control, etc... all turned off, and the car went into manual mode. The vehicle became extremely difficult to control at highway speeds, forcing me to steer and brake manually without assist. Because the Service Center was closed that evening, I couldn’t contact anyone, so I drove back first thing the next morning (July 23) to report the issue. I was initially told the next available appointment was two weeks later, but I had to beg to be seen the same day because the car was unsafe to drive. Tesla redid the alignment and told me the car was fine. After reviewing the alignment reports, both the July 22 and July 23 data are mechanically impossible. The reports show rear camber changes from −1.8° / −2.3° to −1.5° / −1.6°, then to −2.0° / −1.8°, even though rear camber on a stock Model Y is not adjustable. This means the alignment equipment or calibration was inaccurate, and therefore the toe settings were also unreliable. The car was almost certainly released with incorrect rear toe, which explains the immediate handling instability and safety system shutdowns. A later independent alignment on October 13, 2025 (America’s Tire, Roseville) confirmed the correct geometry (−1.9° / −2.0°) and proved Tesla’s readings were wrong. Since the faulty alignments, my car developed severe inner-edge tire wear, and I had to replace all four tires prematurely. I was driving in McKinney, TX on [XXX] taking a friend to his destination. FSD (full self driving) was turned on and enabled for this drive. Normally the car is extremely good at noticing its surroundings, including school zones amidst other things. It is getting better all the time with software updates. However, I turned right into a school zone where the car did not notice that there were other cars going to pick up children. The school zone sign looked like it was abnormally above the ground and so there is a possibility FSD didn’t read it. Unfortunately a police officer noticed me and by the time I did the car was going 16 miles over in a 20 mph school zone. No warnings or anything was shown on the screen to indicate that I had come into a school zone, the speed limit was showing at 40 mph and not 20. Car did not indicate to me that I was not paying attention to the road or doing anything dangerous; it was just going with what it perceived to be flow of traffic. The contact owns a 2024 Tesla Model Y. The contact stated that while driving at an undisclosed speed, the front driver’s side seat failed to adjust to the intended position while the digital screen was in use. No warning light was illuminated. In addition, the contact stated that when the manual front driver’s side seat control pad was in use, the manual front seat control pad felt fragile and moved the front driver’s side seat forward into the steering wheel. The vehicle was taken to a local dealer, and the front driver’s side seat control pad was replaced. The manufacturer was contacted; however, no assistance was provided. The failure mileage was approximately 8,790.

This vehicle is experiencing vibration at highway speeds. it was reported several times to the dealer without resolve. The upon the 3rd trip to the dealer, they are now saying that somehow all 4 rims are bent. None of the wheels have any visible indication of an impact, scratch or damage to the tires. As the only driver to this vehicle, i know firsthand it has not experienced any impact in way of significant bumps, pothole or otherwise. This is diagnosis is of serious concerns if the vehicles wheels will hold up under normal driving and if it were to experience a bump such as one found on a highway paving ridge during construction would it break or become uncontrollable.

When in reverse, the reverse camera disappears if the driver turns the air conditioner on. I was in reverse, pushed the air conditioner button, the camera disappeared and I hit a car behind me. There were also no warning sensors when I got close to the car. It seems to be a safety issue for the reverse camera to ever disappear while in reverse. I’ve had many other cars and while in reverse, you can still turn on the radio or the air conditioner without losing the reverse camera visibility. a child could easily be run over because of this

I am filing this complaint to report a serious safety defect in the Tesla Model Y. On [date], my wife suffered a rear tire blowout at freeway speeds in our Tesla Model Y. Despite regularly checking tire tread depth and relying on Tesla’s built-in tire monitoring systems, the blowout occurred because the inside edges of the rear tires had worn down to exposed cords, while the outside tread still appeared to have ample depth. Tesla Service attempted to explain this away as “normal wear and tear” and a need for alignment. However, this is misleading. The Model Y’s rear suspension has built-in negative camber that cannot be adjusted from the factory. Alignments can only adjust toe, not camber, so this design characteristic inevitably causes accelerated inside-edge wear regardless of alignment. More critically, this risk is not disclosed to consumers: It is not explained during the sales process. It is not documented in the Owner’s Manual. Tesla’s sensors only track pressure, not tread depth, meaning there is no warning system to alert the driver. The average consumer has no way of knowing they must crawl under the car to inspect the inside edges of the tires. To an owner, the tires look safe until catastrophic failure occurs. In addition to the safety hazard, this hidden defect creates a financial burden. Because of the blowout, I was forced to spend $1,200 on new tires unexpectedly. This premature and unforeseen cost compounds the danger by placing families under financial stress along with physical risk. This represents a foreseeable safety hazard that endangers Tesla drivers and passengers. I believe this condition warrants a formal NHTSA safety investigation and potential corrective action (such as a recall, suspension redesign, or mandatory disclosure). Sun glare is a well-documented driving hazard, yet Tesla’s Y model’s visors are too short to offer sufficient coverage. This compromises driver safety and forces customers to seek aftermarket solutions for a problem rooted in design. A responsible manufacturer should address this defect directly. Driving home as sun was setting, I was completely BLINDED by the sun. It was terrifying as the visor was too short to stop the sun glare. I was lucky I didn’t hit something or somebody plus my own safety.

Phantom braking happened. This is the 3rd time to my car. This time, my car stopped automatically when I was already decelerating close to 2-5 mph and the car in front was very far away. I was not using FSD nor autopilot. The emergency auto stop sign came up out of nowhere and my car jerked really hard and my child in the back experienced the jerk and this injured his shoulder.

Traveling today using the cruise control function in Tesla Model Y, the car took over the control of the car and applied braking for no clear reason. Was going 70 miles an hour, had been using the cruise control for about 30 minutes when incident occurred. There were semi trucks in the slow lane, I was in the fast lane. There were no vehicles in front of my car. The car suddenly lunged forward as the brakes were automatically applied. Simultaneously, the steering wheel began to jerk back and forth causing the car to veer back and forth. Vehicles around me began braking as well to avoid rear ending my car and to avert hitting my car as it veered back and forth. I had to fight the steering wheel in an attempt to pull it off the active roadway and then the car suddenly gave all control back to me at which time I was able to disable the cruise control and take full control.

the steering wheel used to be centered in the middle. when you turn right, its resistance would turn it left back to the center point. or when you turn left, its resistance would turn it back right towards the center point. so after each turn such as u-turn, one would grab the steering wheel lightly so it can slide back to the center position. i will call it the "no resistance" point for that centered steering wheel position. now the problem is, that no resistance point has moved a few degrees right and it brings the wheel with it. i now have to turn the steering wheel left (with the resistance that it wants to go back to right) to make the steering wheel centered and go forward straight. car goes forward straight when the steering wheel is centered so it's not an alignment problem. but in order for the steering wheel to be centered, i have to turn it slightly left because the no resistance point has shifted a few degrees right. now this is dangerous because the car can now take you off the lane easily. car is equipped with lane keeping assistance and i find the system helpful but the system is not activated some times under some circumstances. I visited a dealer and they claimed the wheel is centered that there is no need for any repairs.

Three different occasions, while utilizing adaptive cruise the car applied all breaks, causing me to lunge forward with the car coming to a complete stop. I did not see anything that would had caused the car to apply the breaks and the camera's did not record any obstacle. This was brought to the Tesla dealerships attention and I was told there is nothing that can be done about it. If I had not been wearing my seat belt, the potential emergency braking could had resulted in me sustaining a blunt force impact to my chest with the steering wheel, not to mention a possible motor vehicle accident with the vehicle behind me. I did look through the manual and there is no way of me turning feature off or lowering to a warning versus a suddent stop.

There is a Tesla software defect. 1. I stepped into my Tesla Model Y. There was no vehicle in front of my driveway. A delivery van came to park in front of my driveway temporarily. It is a traffic violation according to the California Vehicle Code Section 22500(e). 2. I put my foot on the brake and put my car in the reverse gear. 3. Then I made an adjustment on the CONTROL screen. 4. I put my foot on the accelerator to continue to back up my car. The screen doesn't show the rear view anymore. This is a Tesla software defect that the screen should display the rear view as soon as I push the accelerator in this case.

The contact owned a 2024 Tesla Model Y. The contact stated that while shifting the vehicle into drive (D), the vehicle inadvertently accelerated. The contact stated that he depressed the brake pedal, but the vehicle was gaining forward momentum. Additionally, the steering wheel would not turn right or left. No warning light was illuminated. The contact stated that the vehicle gained speed and crashed through a chain link fence and drove into an alley. The contact stated that the air bags deployed. The contact stated that the vehicle then crashed into a building wall and finally stopped. The contact stated he had abrasions on both hands from the air bag. The contact stated he was treated by EMT on scene. The fire department and police were also at the scene. The vehicle was towed to an impound lot and declared a total loss by the contact's insurance provider. The manufacturer was not informed of the failure. The failure mileage was approximately 25.

While in traffic, I noticed the regenerative braking system was not slowing the vehicle down adequately, despite having a clear and sufficient distance from the big rig ahead of us. As the gap closed, I slammed down on the brake pedal, but it felt abnormally stiff and did not respond with the urgency or force expected. The vehicle failed to stop in time, resulting in a rear-end collision with the big rig. Thankfully, we were not injured due to the speed of the accident. The breaks have not been inspected due to it being in the collision center There was an alarm but it sounded off too late and the car ended up colliding. While driving with Full Self-Driving (FSD) engaged, my vehicle proceeded through a red traffic light without any driver input on two separate occasions within the past few weeks. On both occasions: - The traffic light was clearly red. - The vehicle accelerated forward without prompt while first briefly coming to a full stop it then proceeded forward while traffic light remained on red. - There was no visible obstruction or confusion. These are serious failures in the FSD system’s signal recognition and pose an extreme safety hazard. I request a formal investigation and safety recall if warranted.

shortly after i purchased this car it began to put itself into park while the car was moving slowly and even when it was in car wash mode going through a brush carwash that requires cars to be in neutral. i reached out to tesla support to investigate and they explained this is a safety feature that engages if the driver leaves the seat while the car is not in park. i explained to them that in both cases i had been sitting firmly in the seat and there should have been no reason for the car to put itself in park and asked them how i turn this auto feature off. they said there was no way to turn it off. i explained to them that ALL automation features should be optional, they argued with me. came to find out some sensor in my driver's seat was malfunctioning. they took the car to replace the seat/sensor, but i reiterated that all these auto features should be optional and a consumer should be able to turn them off. my car was rendered unsafe to drive due to the malfunctioning sensor, i should have had a way to turn off the feature, i asked them to open a ticket with their engineers to add settings to turn off this automation feature. fast forward to 6/25/25, as i am pulling my car into a parking space at a store the car SLAMS itself in park, so hard that i thought maybe i had hit something hard with one of my wheels. i followed back up with Tesla support and am being told there is still no way for me to turn off this automation feature. no automation feature should EVER be hard coded into a motor vehicle. the risk presented by malfunctioning automation features is too great, and if i want to completely control every aspect of my car i should be able to, regardless of how much automation it is capable of. i am filing a complaint now because Tesla is clearly not taking these concerns seriously

The contact owned a 2024 Tesla Model Y. The contact stated that while his son was driving at approximately 38 MPH with his sister inside the vehicle, the brake pedal was depressed; however, the vehicle lost steering and braking functionality and failed to stop as intended, causing the vehicle to drive through a crosswalk. The contact stated that the steering wheel failed to respond while being turned, causing the vehicle to crash into a curb and then into a parked vehicle, which was able to stop the vehicle. The contact stated that the vehicle was towed to a Tesla Collision Center, and between Tesla and the Insurance provider, the vehicle was deemed a total loss. The Insurance Company reviewed the event data recorder (EDR) reports and confirmed the failure which contributed to the crash. The contact stated that the Police was not contacted because it was a single vehicle crash. The other vehicle owner was notified of the crash and was provided the needed information to file a claim. A Police report was not filed. The contact's son was taken to the local medical center in a family vehicle. The contact's son and daughter sustained headaches and concussions, and both received medical attention. The contact drove his son to the local medical center, where he was admitted and remained overnight. Additionally, the contact's son was continuing outpatient treatment for the injuries sustained. The manufacturer was made aware of the failure but provided no assistance. The failure mileage was approximately 3,500.
