Internal computer failed on two occasions in first 3 months of ownership. The internal computer controls nearly all safety features related to visibility and navigation. This includes failure of the backup cameras, lane change cameras, adaptive cruise, blind spot assist, and other driver assist safety features. Without these functioning systems the vehicle immediately becomes unsafe to drive. This failure occurred at 56 miles on the odometer, and the again +/- 2 months later at about 3000 miles.

Tesla shared I have an AP-4 computer short that disables all cameras (rear back up camera, blind spot camera, lane deviation detection, emergency assisted forward braking), navigation, and auto lighting features are disabled. Other non-essential features do not work as well.

The following safety-critical and driver-assist features have become non-functional: Cruise Control (adaptive or otherwise) Forward Collision Warning Lane Departure Warning Lane Keeping Assistance Blind Spot Warning Parking Collision Warning Full Self-Driving and Auto Park features Cameras required for Autopilot functionality Additionally, because the GPS system has failed, the vehicle’s Navigation system is inoperable. This impacts Tesla’s unique route planning feature that sets up Supercharger stops during long journeys. Without a functioning navigation system, I cannot properly plan my travel or locate charging stations, which severely affects the usability of the vehicle. Safety Concerns: The failure of these systems, especially safety features like forward collision warning, lane keeping assistance, and blind spot monitoring, puts both the driver and others on the road at risk. On 12/13/24 after a software update my rear , front and side cameras stopped working. This includes blind spot cameras, and side turn signals. In addition the side mirrors are tiny because the car was designed to use cameras. The back window also has very low visibility. Tesla has confirmed that the car needs a new computer for all cameras and software navigation. The replacement computers are on back order and have scheduled a service replacement in six weeks time. In the meantime, they refuse to provide a loaner car. This car is brand new with only two months worth of usage. They say the car is drive able, but without the main safety cameras I don’t feel safe, the aesthetic of the car was designed to have working cameras. It is very hard to use the tiny side view mirrors and look out the tilted rear window. This is a commuter car, driving over 90 miles on highway per day. I am concerned for safety issues.

Tesla has told me the AP4 computer has failed in my car which has resulted in them having to replace it. I had this issue start right around Thanksgiving, specifically [XXX]. I scheduled the soonest appointment available for 12/13 however Tesla contacted me and moved my appointment because they stated that the part was unavailable at this time and should be delivered by 01/20/25. Not offering me a rental or anything on my brand new '24 Tesla. It has just over 2500 miles on it and is ~2 months old now. This issue impacts: emergency braking, blind spot detection, lane keep/centering, assisting with parking, automatic high beams and wipers, cruise control/ adaptive cruise control, Vehicles built in GPS to be stuck at one location every time, has made my touchscreen not automatically brighten or dim, my auto dimming mirrors be too dim in most driving conditions, constantly fail to install system improvements and safety updates, and because my vehicle keeps trying to do this, it has caused increased battery drain as well. Additionally, the vehicle came to a stop on the highway when my wife and I were in the car on the night of [XXX], forcing us to perform an emergency pullover and abandon the car 22 miles north of Rochester, MN overnight. This has increased the safety risk of my vehicle tremendously... Tesla has not seemed to publicly acknowledge the issue yet either. All cameras stopped working. With this, all safety features are not working. I can’t see to back up at night. No collision avoidance, no lane departure warning, high beams don’t stay on, turn signal doesn’t stay on. It has been two weeks and Tesla has no fix. There was no warning, just noticed it one day as I was driving it and was very dangerous in dark snowy conditions. I called Tesla service immediately and they gave me an appointment for following week. Then next day I got message from them that there are a large number of vehicles having the issue and they said they don’t have a fix for it and moved my appt out 7 weeks. I have gotten no help from them and I told them I don’t feel safe driving the car. They have not provided another car.

Tesla service advisor states - car is totally safe to drive with just mirrors till appointment in few weeks. No backup camera, navigation, battery life dying faster. It seems like a liability to me to drive this way and very negligent to me.

At night, on paved and well-painted rural roads, where there is no other sources of light, the pillar cameras incorrectly warn they are blocked or blinded. At the same time the Lane Departure Warning and Lane Keeping Assistance stop functioning without warning. On the image below I let the car drift over the line. As I crossed the line, the display should have highlighted the line in blue and there should have been a flashing warning at the top of the screen and an audible warning. None of those occurred. Since lane keeping assistance was enabled. It should have also steered the car back into the lane. I can regularly reproduce the issue. We don't understand why they are using the pillar cameras for this and not the forward cameras that can take advantage of the headlights. It is unsafe, especially since I am not warned of the system is not available.

In a 90 mile drive I experience phantom breaking 4 different times. No car was in front of me, in fact nothing at all was in front of me when my car slammed on the brakes 4 separate times. One time there was a car somewhat close behind me. I also twice experienced audible warnings and flashing alarms to take control immediately of the car when my hands were on the wheel and I was already in control of the car, it was alarming and shocking to get these alarms while already in control and driving. I believe this car, as currently configured, can pose a danger to other drivers on the road.

This is a complaint regarding a malfunction of the Tesla Full Self-Driving (FSD) feature, which resulted in damage to my vehicle and may potentially pose a significant safety risk to public. On 07/02/2024, my newly purchased Tesla [Model-Y 2024] was parked next to a pillar in a parking garage. After ensuring that the Full Self-Driving sign was on, I activated the FSD feature. However, as soon as the function was engaged, the vehicle instantly took off and made a right turn. This sudden and unexpected action caused the right side of the back passenger door to scrape against the iron guardrail on the pillar and caused damages to the right back passenger door and a portion of right back fender. This action happened so sudden that did not have the chance to react and take manual control. I contacted Tesla to report the issue and seek assistance, but after +4 weeks of contacting and restless following up with Tesla service teams, Tesla stated that the software was functioning properly and denied any responsibility for the malfunction, and they closed my case without offering any resolution or further investigation. Luckily, this incident happened in a confined parking garage. It could have resulted in more severe damage or injury had the circumstances been different. Additionally, Tesla's dismissal of the issue and refusal to accept responsibility is deeply concerning, especially about the reliability and safety of their Full Self-Driving technology. I sincerely request your office to investigate into this incident, and I hope you could advise any appropriate action to examine and address this issue. I attached a few photographs of where my vehicle was parked before the FSD was turned on, and the damages on both my vehicle and the guardrail at the time of the incident. I am more than happy to provide any further information or clarification. Thank you for your attention to this matter. I look forward to your guidance and assistance.

The contact owns a 2024 Tesla Model Y. The contact stated that while driving at an undisclosed speed with auto-polite activated, the vehicle suddenly pulled in the left direction across three lanes of traffic. As a result, the vehicle collided with the passenger-side front headlight and came to a complete stop in the roadway. The air bags did not deploy. No warning lights were illuminated. The vehicle was towed to an independent mechanic, who was unable to diagnose the failure. The vehicle was not repaired. A police report was filed, and injuries were reported. The contact transported himself to his primary care doctor, who treated him for nondescript injuries to his neck, shoulders, right hand, and hips. The manufacturer was notified of the failure, and a case was opened. The failure mileage was 9,000.

I was on [XXX] heading towards Chicago and following the traffic. I started changing the line to pass the vehicle in front of me and suddenly my tesla completely lost control and started drifting to the sides. I released the accelerator right away but it made it even worse. It seems that acceleration continues to happen that’s what made car to drift. Eventually car slowed down and I was able to make it to my line. I don’t understand why line assistance didn’t work to keep the vehicle stable, why after releasing accelerator the vehicle continued to gain the speed. It was sunny day and dry road.
