Vehicle Information: • Make: Tesla • Model: Model Y • Year: 2024 Description of the Problem: After a software update pushed by Tesla, the AP4 computer in my 2024 Tesla Model Y shorted out, causing a complete failure of all systems controlled by this computer. The following safety-critical and driver-assist features have become non-functional: Cruise Control (adaptive or otherwise) Forward Collision Warning Lane Departure Warning Lane Keeping Assistance Blind Spot Warning Parking Collision Warning Full Self-Driving and Auto Park features Cameras required for Autopilot functionality Additionally, because the GPS system has failed, the vehicle’s Navigation system is inoperable. This impacts Tesla’s unique route planning feature that sets up Supercharger stops during long journeys. Without a functioning navigation system, I cannot properly plan my travel or locate charging stations, which severely affects the usability of the vehicle. Safety Concerns: The failure of these systems, especially safety features like forward collision warning, lane keeping assistance, and blind spot monitoring, puts both the driver and others on the road at risk. Losing access to safety-critical functions after an update is unacceptable and poses a serious danger to vehicle occupants and surrounding traffic. Incident Details: • The issue occurred immediately following a software update. • Tesla pushed the update remotely without providing any warning or indication of potential risks. • The failure appears to be related to a hardware short in the AP4 computer, directly caused or exacerbated by the update. Desired Outcome: • A thorough investigation into Tesla’s software update process and its effect on vehicle hardware and safety systems. • A resolution that prevents similar failures in other vehicles and ensures system updates do not compromise vehicle safety or critical functionality.

I had my computer replaced (AP4) on the Model Y. After a software update around Nov 13, my computer glitched. The car was drivable, but no functionality (cameras, navigation. Etc). After the computer was replaced it seems fine. Just wanted to report after the fact. Thanks

Cameras not working with latest Software update on November 26th. without even inspecting the car in-person, Tesla service agent told need new AP4 computer on 2024 model Y which is 1 month old . I think Tesla knows about this issue and there is lot of wait time for getting the AP4 computer. Tesla service advisor states - car is totally safe to drive with just mirrors till appointment in few weeks. As per NHTSA, if a car is not having a backup camera, that is a safety violation and there are lots of Tesla vehicles driving without backup cameras. No backup camera, navigation, battery life dying faster. It seems like a liability to me to drive this way and very negligent to me.

Bought my Tesla Model Y on [XXX]. Exactly one month later on [XXX] I start my car to find that none of the cameras are working. This means Autopilot, cruise control, rear camera, blind spot monitoring and many more safety features are completely down. In addition the GPS does not function meaning my car is not trackable and Maps don't work, another safety concern and main reason we bought the car. This is a widespread issue that Tesla Service Centers have been informed about from Tesla that it is a bad computer that needs to be replaced - in a 1 MONTH old car. My only option is to wait for a computer to be assigned to me which is at earliest on January 21st of 2025, almost 6 weeks without a fully functioning brand new $46,000 Car with only 1400 miles on it. Tesla are also unwilling to make any kind of restitution such as a loaner car, or a refund for the Full Self Drive and Connectivity packages I just purchased but cannot use as my car is broken. Tesla has told me the AP4 computer has failed in my car which has resulted in them having to replace it. I had this issue start right around Thanksgiving. I scheduled the soonest appointment available for 12/16 however Tesla contacted me and moved my appointment because they stated that the part was unavailable at this time and should be delivered by 01/20/25. Not offering me a rental or anything on my brand new '24 Tesla. It has just over 1,000 miles on it and is ~3 months old now. This issue impacts: emergency braking, blind spot detection, lane keep/centering, assisting with parking, automatic high beams and wipers, cruise control/ adaptive cruise control, Vehicles built in GPS to be stuck at one location every time, has made my touchscreen not automatically brighten or dim, my auto dimming mirrors be to dim in most driving conditions, constantly fail to install system improvements and safety updates, and because my vehicle keeps trying to do this, it has caused increased battery drain as well. This has increased the safety risk of my vehicle tremendously... Tesla has not seemed to publicly acknowledge the issue yet either.

Vehicle cameras stopped working. Navigation stopped working. Screen also stopped working for a time. Was able to get it back on but it went off multiple times since. If I can't see the screen, I can't tell how fast I'm going to do anything else. Cameras control many of not all of the safety systems. So those are all inoperable. Issue is being reported across multiple forums all over the internet as "short in ap4 module."

Since I purchased the car 4 months ago my car has been going into the service center non stop for every issue you can imagine. Tesla spins and blames all issues and ignores me. Corporate says they can’t do anything and then ignore. Service center providing inaccurate or not providing service invoices.Deny my buyback requests. Repairs are failed and new damage occurs at every service appointment. New issues arise with the car every time I drive it. I rarely drive it as it’s not safe. Tesla continually cancels service appointments or does not have parts in stock. issues: -Safety restaurant system fault -electric motor issues: loud high pitched whistle/whine when driving. -car loosing power while driving -seat moving while driving -window clicking -water dripping out of dash vents. -Misalignments: frunk, trunk, trim -paint drips, bubbles, cracks, peeling -battery range issues -battery coolant fan issues -defects interior plastic surfaces -seat defects -hole left in front of car between dash and windshield with damages windshield. New dash uneven over airbag. -plastic trim falling off

With beta self driving engaged, I turned south on [XXX] intending to turn west on [XXX]. At [XXX], there are 2 right turn lanes. I approached the intersection turning from the left lane. Rather than turn right, the car continued straight. I was able to manually override the system and luckily brought the car under control; otherwise, I am sure I would have ended up in a very bad spot. Later the same day beta did not recognize a turn lane into my community but rather proceeded straight. I have since disabled the beta FSD. I am responding to today's CNN article of your pending investigation. I cannot respond to your other questions above. Thank you! Component Failure: Coolant line and connector to battery pack under Tesla Model Y failed, causing vehicle shutdown. Part available for inspection. Safety Risk: Driving on mild dirt road, vehicle experienced progressive issues leading to sudden shutdown. Potential for serious accident, especially at higher speeds or in traffic. Problem Confirmation: Tesla service center confirmed coolant leak from damaged connector caused shutdown. Reported as known issue that requires complete battery replacement, despite it just being a connector. Inspection: Examined by Tesla technicians. Further inspection possible if required. Warning Signs: Screen displayed progressive warnings: 1. "Coolant low, schedule service, but okay to drive" 2. "Power reduced, okay to drive" 3. "Rear motor disabled be careful going down hills" 4. "Pull over immediately" - followed by shutdown Additional Details: - Small rock likely hit connector on dirt road - Coolant connector not threaded, not easily replaceable - Common issue in Tesla models, especially Model Y - Warnings progression didn't adequately convey severity - Battery replacement cost exceeds $15,000+, unreasonably high given Tesla's practice of refurbishing and reusing batteries Recommended Action: 1. Redesign coolant connector for impact resilience 2. Implement threaded design for easier replacement of connector (not entire battery) 3. Replace plastic shielding under vehicle with 1/8" aluminum shielding 4. Significantly reduce battery replacement costs, considering Tesla's ability to refurbish and reuse batteries This single point of failure poses significant risk and potential for extreme repair costs. Coolant connector vulnerability to minor impacts suggests design flaw affecting Tesla owners driving on unpaved surfaces. NHTSA investigation needed to prevent future incidents, ensure safety, and address unreasonable repair costs for Tesla Model Y owners and other road users..

I was on [XXX] heading towards Chicago and following the traffic. I started changing the line to pass the vehicle in front of me and suddenly my tesla completely lost control and started drifting to the sides. I released the accelerator right away but it made it even worse. It seems that acceleration continues to happen that’s what made car to drift. Eventually car slowed down and I was able to make it to my line. I don’t understand why line assistance didn’t work to keep the vehicle stable, why after releasing accelerator the vehicle continued to gain the speed. It was sunny day and dry road. The Tesla accelerated by itself while I am driving in a heavy traffic. To avoid hitting the front car, I braked several times, which didn’t work at all. It still accelerated, instinctively I steered wheels to the right front and hit a small hill. Airbags are out. I purchased my Model Y with the Tow Package. We used the trailer for the first time. There was no 12v auxiliary power to the trailer. The 7 pin trailer connection is incomplete. Post purchase, we found Tesla purposely disconnected the 12v accessory power pin when they switched low voltage systems in the 2022 model year rather than add the necessary power reduction part to send 12v power to the trailer. This is a serious safety issue because the 12v accessory pin on the trailer connection is the used to maintain charge on emergency trailer surge braking systems (engages trailer breaks if the trailer disconnects from vehicle). This known safety hazard was not conspicuously communicated before purchase. A notice does not appear on the Model Y Tow package page (attached). This is very uncommon and unexpected for manufacturer installed tow packages with 7 pin connectors to leave the 12v auxiliary power disconnected.
