After Tesla Berkeley performed an alignment on my 2024 Model Y on July 22, 2025, the car suffered a complete systems failure while driving on the freeway. Within minutes of leaving the service center, multiple safety systems shut down at once — steering assist, emergency braking, lane keeping, stability control, etc... all turned off, and the car went into manual mode. The vehicle became extremely difficult to control at highway speeds, forcing me to steer and brake manually without assist. Because the Service Center was closed that evening, I couldn’t contact anyone, so I drove back first thing the next morning (July 23) to report the issue. I was initially told the next available appointment was two weeks later, but I had to beg to be seen the same day because the car was unsafe to drive. Tesla redid the alignment and told me the car was fine. After reviewing the alignment reports, both the July 22 and July 23 data are mechanically impossible. The reports show rear camber changes from −1.8° / −2.3° to −1.5° / −1.6°, then to −2.0° / −1.8°, even though rear camber on a stock Model Y is not adjustable. This means the alignment equipment or calibration was inaccurate, and therefore the toe settings were also unreliable. The car was almost certainly released with incorrect rear toe, which explains the immediate handling instability and safety system shutdowns. A later independent alignment on October 13, 2025 (America’s Tire, Roseville) confirmed the correct geometry (−1.9° / −2.0°) and proved Tesla’s readings were wrong. Since the faulty alignments, my car developed severe inner-edge tire wear, and I had to replace all four tires prematurely. Traveling today using the cruise control function in Tesla Model Y, the car took over the control of the car and applied braking for no clear reason. Was going 70 miles an hour, had been using the cruise control for about 30 minutes when incident occurred. There were semi trucks in the slow lane, I was in the fast lane. There were no vehicles in front of my car. The car suddenly lunged forward as the brakes were automatically applied. Simultaneously, the steering wheel began to jerk back and forth causing the car to veer back and forth. Vehicles around me began braking as well to avoid rear ending my car and to avert hitting my car as it veered back and forth. I had to fight the steering wheel in an attempt to pull it off the active roadway and then the car suddenly gave all control back to me at which time I was able to disable the cruise control and take full control.

Three different occasions, while utilizing adaptive cruise the car applied all breaks, causing me to lunge forward with the car coming to a complete stop. I did not see anything that would had caused the car to apply the breaks and the camera's did not record any obstacle. This was brought to the Tesla dealerships attention and I was told there is nothing that can be done about it. If I had not been wearing my seat belt, the potential emergency braking could had resulted in me sustaining a blunt force impact to my chest with the steering wheel, not to mention a possible motor vehicle accident with the vehicle behind me. I did look through the manual and there is no way of me turning feature off or lowering to a warning versus a suddent stop.

On [XXX], at approximately [XXX], I was driving westbound on the [XXX], just before the [XXX] and [XXX] off-ramp, with my wife and our [XXX] baby in the car. While in traffic, I noticed the regenerative braking system was not slowing the vehicle down adequately, despite having a clear and sufficient distance from the big rig ahead of us. As the gap closed, I slammed down on the brake pedal, but it felt abnormally stiff and did not respond with the urgency or force expected. The vehicle failed to stop in time, resulting in a rear-end collision with the big rig. Thankfully, we were not injured due to the speed of the accident. The breaks have not been inspected due to it being in the collision center There was an alarm but it sounded off too late and the car ended up colliding. shortly after i purchased this car it began to put itself into park while the car was moving slowly and even when it was in car wash mode going through a brush carwash that requires cars to be in neutral. i reached out to tesla support to investigate and they explained this is a safety feature that engages if the driver leaves the seat while the car is not in park. i explained to them that in both cases i had been sitting firmly in the seat and there should have been no reason for the car to put itself in park and asked them how i turn this auto feature off. they said there was no way to turn it off. i explained to them that ALL automation features should be optional, they argued with me. came to find out some sensor in my driver's seat was malfunctioning. they took the car to replace the seat/sensor, but i reiterated that all these auto features should be optional and a consumer should be able to turn them off. my car was rendered unsafe to drive due to the malfunctioning sensor, i should have had a way to turn off the feature, i asked them to open a ticket with their engineers to add settings to turn off this automation feature. fast forward to 6/25/25, as i am pulling my car into a parking space at a store the car SLAMS itself in park, so hard that i thought maybe i had hit something hard with one of my wheels. i followed back up with Tesla support and am being told there is still no way for me to turn off this automation feature. no automation feature should EVER be hard coded into a motor vehicle. the risk presented by malfunctioning automation features is too great, and if i want to completely control every aspect of my car i should be able to, regardless of how much automation it is capable of. i am filing a complaint now because Tesla is clearly not taking these concerns seriously

I was driving at a speed of 60 miles per hour in rainy weather when the car’s emergency braking system suddenly activated. At the same time, the steering wheel locked due to the braking, and I completely lost control of the vehicle. The car spun multiple times before crashing into the center divider of the highway and finally coming to a stop. This was an extremely dangerous incident from a safety perspective. This was the first time such a braking malfunction occurred. The incident has not yet been investigated by the insurance company, and no warning or notification was displayed by the vehicle at any point.

Upon approaching a railway crossing, the signal was green, indicating it was safe to proceed. However, instead of maintaining a controlled speed, the car suddenly accelerated while attempting to cross. Before my spouse could take control, the vehicle struck the boom barrier, which was either lowering or had already lowered at that moment. The incident happened so suddenly that she did not have enough time to react and override the system. Fortunately, no injuries occurred, but the vehicle sustained damage to the front bumper, windshield, and frame. The unexpected behavior of FSD at a railway crossing raises serious safety concerns regarding its decision-making. Safety Concerns: I have been a long-time appreciative user of FSD, but this incident has shaken my confidence in the system. I now fear using FSD. Given the serious nature of this incident, I request an urgent review and response regarding my concerns, as well as coverage for the necessary repairs. Request for Investigation & Cost Coverage: Access to Vehicle Logs: I request access to the vehicle's telemetry and camera recordings to determine why this happened. The contact owns a 2024 Tesla Model Y. The contact stated that as he was driving forward at approximately 5 MPH after having stopped for a traffic signal his vehicle was crashed into the driver's right front fender by another vehicle. The contact stated the vehicle's front collision sensor did not detect the other vehicle. The contact stated that the driver's side front fender and the hood on his vehicle had to be replaced. The contact was not sure about the damage to the other vehicle, but there were no injuries. The police arrived on scene and wrote a report but he was not given the report number. The vehicle was towed to a local dealer and the body damage was repaired, however, the failure of the front collision avoidance sensor was not diagnosed. The contact stated that the failure reoccurred. The contact was driving at approximately 35 MPH at night when he crashed into the rear of a vehicle that was double parked on the street. The contact stated that the parked vehicle was in a poorly illuminated section of the street. The contact stated that his forward collision avoidance sensor had failed to engage and had not stopped his vehicle. The contact stated that the police were on the scene and had not issued a report. The contact had the vehicle towed to a local body shop, where the damage to the front bumper, grill, and headlights was repaired. The manufacturer was not informed of the failure. The failure mileage was approximately 45,000.

I was driving on a three-lane highway (I-495) using the adaptive cruise control (autosteer) when I suddenly noticed the car to my right dangerously veering toward my vehicle. In an attempt to change lanes to the left, my car wildly overcorrected to the left, nearly sending me into the highway median, followed by another overcorrection to the right. The steering wheel felt almost uncontrollable. Luckily no collision.

When using FSD, I've noticed that on occasion at a red light, the car will start moving, albeit very slowly, a fraction of a second before the light turns green. I've assumed the car was watching the traffic lights from the other direction using its cameras. So on the [XXX] when this happened, and the car started moving I let it go thinking the light was about to turn green. But I was shocked that the car drove through the red light. Admittedly there were red lights in all directions. I know I'm supposed to be keeping control but it was so shocking that by the time I reacted the car was already in the intersection so I let it go. I continue to get completely FALSE forward collision warnings. Part of this is because my front camera housing keeps fogging up and over the past 529 weeks I’ve gotten a half dozen false FCWs. When these faulty alerts occur, a loud audible beeping is heard and red alerts are displayed on the screen. These faulty alerts are dangerous distractions, causing panic, fear, and uncertainty in the driver, causing them to take their attention off of the road while they assess what just occurred. This is a critical flaw in the functionality of Tesla vehicles that the company has refused to address. This also makes me insurance rate spike way higher and I’m getting charged extra for this.

Tesla has told me the AP4 computer has failed in my car which has resulted in them having to replace it. I had this issue start on 1/12, and I scheduled the earliest appointment available for 1/30. They did not offer me a rental or any assistance during this time, and I need my vehicle to commute to work and conduct other essential activities. It has just over 1,000 miles on it and is ~3 months old now. This issue impacts: backup camera, emergency braking, blind spot detection, lane keep/centering, assisting with parking, automatic high beams and wipers, cruise control/ adaptive cruise control, Vehicles built in GPS to be stuck at one location every time, has made my touchscreen not automatically brighten or dim, my auto dimming mirrors be too dim in most driving conditions, constantly fail to install system improvements and safety updates, and because my vehicle keeps trying to do this, it has caused increased battery drain as well. This has increased the safety risk of my vehicle tremendously, and i have received no reparations for it.

Back in late November 2024, Tesla pushed an Over-The-Air (OTA) update to my 2024 Tesla Model Y. Immediately after pushing this OTA, all of my car's cameras stopped working (INCLUDING my rear or "backup" camera). This is in violation of federal law as all new vehicles are required to have a backup camera. Additional safety-critical items were also impacted: the rear-view and both of the side mirrors remain tinted to the darkest level, reducing my visibility. The side mirrors are also tilted in the "downward" position, making lane changes incredibly difficult. Since the front-facing and side-facing cameras do not work the vehicle's forward and side collision detection systems are inoperable. The vehicle suffers from additional failures (GPS navigation, Maps, Autopilot, among many others). I have presented these issues to Tesla and they have continously pushed out my repair date. I have also asked for a loaner vehicle due to the failure of the car's safety systems (some of which are federally mandated!!). Tesla has ignored my pleas for a loaner vehicle and has also remained silent when I have repeatedly asked to return the vehicle under Washington State lemon laws. My safety and the safety of others have been put at grave risk due to the backup camera and collision detection cameras not working. Tesla has NOT inspected the vehicle as they continuously push my service date further out. No obvious warning lamps, messages, or other symptoms appear. The user doesn't know these federally-mandated safety features are inoperable until they put the car in "reverse" or attempt a lane change.

Around the end of November, my 2024 Tesla Model Y's cameras stopped functioning. The sensors for blind spots and possible collision prevention stopped working. The GPS was also not working, the car believes it's been in the same spot for over a month. The lights of the car also do not work as promised, as now I have to manually turn them on on my dashboard and it goes on and off. I have been trying to get into Tesla to have this fixed but the appointment keeps getting pushed out due to the part that needs to be fixed not being in stock. I brought my car into Tesla for them to confirm that it is the part that is missing and that is the case but they couldn't fix it because they don't have the part. It's been over a month and my date was pushed out again, probably the sixth or seventh time. Tesla claims that there will be a software update that will fix this issue, however, my car is not updating and won't even accept a software update.

I initially visited the Milford, CT Tesla Service Department and communicated through the Tesla Ap on 12/14/2024 regarding my 2024 Tesla Model Y, which has only 2,357 miles on it. The car's cameras, sensors, & navigation stopped working, making driving unsafe. Now, the bright lights do not go on. The vehicle's camera/sensors are critical for safe operation. Tesla said the car may need a new computer, but they have none in stock. They texted me that they have many people facing the same issues.

Tesla Full Self Driving v13.2.2 attempted to make a left turn while having a red light. The vehicle successfully came to a complete stop due the red light but once traffic cleared the intersection it attempted to proceed with the left turn while still having a red light. According to Tesla the AI4 computer experienced an internal short circuit. This has rendered all cameras including backup camera, GPS, parking sensors, and automatic wipers inoperable. The visibility out of the vehicle is very poor to the rear without the backup camera. Parking without the rear camera and parking sensors is difficult. Not having physical controls for the wipers makes turning them on/off and changing speeds difficult, causing a lot of drivers distraction.

Around Dec 16 all cameras in my 2024 Model Y Tesla stopped working and it has not worked since that date. This affects many systems, the most important of which is that I now have no rear backup camera. I believe vehicle manufacturers are required to have functioning back up cameras. I notified Tesla service on 12/16 and was told there was a known computer hardware problem which would cause my incident. Tesla service told me they were unable to obtain the needed computer replacement for a month at least. They informed me there was no safety risk, but I disagree since I don’t have a functioning back up camera, nor lane change camera assistance. Also the display panel is very dark and impossible to read most of the time, posing another safety concern. Why has Tesla not issued a public statement about this problem, and why has NHTSA not mandated a formal recall?

Vehicle Information: • Make: Tesla • Model: Model Y • Year: 2024 Description of the Problem: After a software update pushed by Tesla, the AP4 computer in my 2024 Tesla Model Y shorted out, causing a complete failure of all systems controlled by this computer. The following safety-critical and driver-assist features have become non-functional: Cruise Control (adaptive or otherwise) Back up Camera Forward Collision Warning Lane Departure Warning Lane Keeping Assistance Blind Spot Warning Parking Collision Warning Full Self-Driving and Auto Park features Cameras required for Autopilot functionality Additionally, because the GPS system has failed, the vehicle’s Navigation system is inoperable. This impacts Tesla’s unique route planning feature that sets up Supercharger stops during long journeys. Without a functioning navigation system, I cannot properly plan my travel or locate charging stations, which severely affects the usability of the vehicle. Safety Concerns: The failure of these systems, especially safety features like forward collision warning, lane keeping assistance, and blind spot monitoring, puts both the driver and others on the road at risk. Losing access to safety-critical functions after an update is unacceptable and poses a serious danger to vehicle occupants and surrounding traffic. Incident Details: • The issue occurred immediately following a software update. • Tesla pushed the update remotely without providing any warning or indication of potential risks. • The failure appears to be related to a hardware short in the AP4 computer, directly caused or exacerbated by the update. Desired Outcome: • A thorough investigation into Tesla’s software update process and its effect on vehicle hardware and safety systems. • A resolution that prevents similar failures in other vehicles and ensures system updates do not compromise vehicle safety or critical functionality.
