The vehicle repeatedly displays a "Stop Vehicle and Check Brake System" warning together with an ABS warning, accompanied by a loss of normal braking response. On XXX, while driving at approximately 60 mph on I-85 northbound near Exit XXX in [XXX], the warnings appeared and the brakes did not respond normally. I was maintaining a substantial following distance, and the braking system restored itself before I reached the point of needing to brake hard, so no crash or injury occurred. Had hard braking been required in that window, I do not believe the vehicle would have stopped as commanded. This is a recurring fault. The vehicle, purchased certified pre-owned and still under the Hyundai factory warranty, has been taken to the dealer (Ed Voyles Hyundai, [XXX]) three times for the same condition. On the first visit the Integrated Electronic Brake (IEB) unit was replaced; on the second, the Smart Junction Box was replaced; on the third, the dealer removed an aftermarket tracking device wired to the data link connector and referenced Hyundai TSB 24-GI-011H, representing the problem as corrected. The braking-loss event on XXX occurred after that repair. The vehicle has been out of service approximately 30 days total. Because the failure is an intermittent loss of braking response at highway speed, I consider it a serious safety hazard and no longer drive the vehicle. OnXXX, as we approached what I call the 6 o’clock position of a round-about in [XXX], we had braked almost to a full stop (0-5 mph) to wait on the car ahead of us to enter the round-about when we felt increasing movement. Despite braking, to wait on that car, our car with increasing speed started shoving that car into the circle. Fortunately, he was able to drive away from us, but that car had been damaged on the back and right side. With that obstacle out of the way, our car rapidly and exponentially increased speed - all while we were fully braking. Two witnesses said they saw our brake lights as we sped along. We hit the curb as we approached the 12 o’clock exit, and that turned us so that we crossed both lanes of the road and headed towards the far sidewalk and storefronts. When we hit that curb it pushed us diagonally to face the street again. Hard to estimate but we must have been doing 50 mph when we hit a very large brick planter and we finally were wedged to a stop when we slammed in between two vehicles parked parallel to the sidewalk. The motor ran for a few seconds after we stopped, and finally quit on its own. We KNOW the brake was applied the whole time; there were no visual or audible alarms from the car as we entered “UNINTENDED ACCELERATION”. Despite the incredibly strong impact (our car has been deemed totaled) the front air bags did not deploy. Only the driver’s side curtain deployed. The bumper of the parked transport van on our front left was damaged, and the small SUV on our front right was deemed totaled. No people were in those two parked cars; the first car was hit at the initial slower speed so those people were not injured. By some grace, no pedestrians were in our path or they would surely have been killed. The driver of our car (my husband) was miraculously not injured, but CT scans and x-rays showed I had several contusions and a fractured tibia that will require surgery due to brake, propulsion, no air bags. On [XXX], I received a warning on my 2023 IONIQ 5 that said “stop vehicle and check braking system” and the car went into “turtle mode”. I took it into the dealer for repairs and they said the issue was related to the 12V battery and returned the car to me as repaired. Then, on [XXX], I was driving the car and I got the same error message but this time the car’s brakes didn’t work. Thankfully, I was able to pull off the surface road I was on and pulled into a residential area where I used the parking brake to stop the car. I then had it towed to the dealer and they are currently in the process of repairing it. My son and I were in the car at the time of the [XXX] incident and were very fortunate that we did not hurt anyone or ourselves when the brakes didn’t work. As of today, the Hyundai dealer (Roseville, CA) says they are repairing a brake component that was defective but they missed this on the 2/28/26 inspection. I am reporting a safety-related defect involving unintended vehicle movement on my Hyundai Ioniq 5 (Model Year 2023, SEL AWD). Observed behavior: With the vehicle in Drive and the Auto Hold function active (indicated by the white “AUTO HOLD” indicator in the instrument cluster), I brought the vehicle to a complete stop by pressing the brake pedal. Auto Hold engaged (text now green), the vehicle remained stationary, and I removed my foot from the brake. After approximately three minutes of remaining stopped, a driver-assistance system error message appeared on the instrument cluster. At that point, Auto Hold disengaged (the “AUTO HOLD” indicator disappeared), and the vehicle began to creep forward on its own. I did not command acceleration, did not press the accelerator pedal, and did not intentionally release the brake. The movement was unexpected and created a safety risk. The first occurrence happened in a school drop-off line with the vehicle stopped near other vehicles and pedestrians. I have since reproduced the behavior and captured it on video. Related observations: Similar driver-assistance system errors have occurred while the vehicle was in Park and in Drive, with or without Auto Hold active. In those cases, the vehicle did not move because it was either mechanically locked or I had my foot on the brake. In all cases, Auto Hold deactivated. Safety concern: This behavior raises concerns about the reliability of Auto Hold and its interaction with braking and driver-assistance systems, particularly after the vehicle has been stationary for an extended period. Unintended forward movement presents a significant safety hazard. I am reporting this issue for investigation of potential defects in software, sensors, or control logic. When parking in a supermarket parking lot I slowly approached the space while braking. As I turned left into the space and released the brake pedal to allow the car to slowly roll into the space as I always do, the car surged as if someone floored the accelerator. I veered left to avoid a head on collision with the parked car facing me and went diagonally between the right front fender of that car and the left front fender of the car parked in the space to my left. I sideswiped both sides of my car and damaged the front fenders of both cars before I was able to apply the brake and bring the car to a stop. There was a mother with her two young children loading groceries into the car in front of me. Fortunately I avoided hitting anyone, but the witnessed the accident. I was in shock. My car suffered $25 in damages. There is absolutely no way I accidentally applied the accelerator, and if I had I would never have floored it! The car malfunctioned. I reported this incident to Hyundai also. They supposedly were sending someone to inspect the car at the auto body repair center.

I bought my Ioniq 5 in April 2023, starting in June 2023 I started having issues with the internal navigation system locating my vehicle on a different route than I was actually on. When I connect to Apple CarPlay to be able to use Google Maps or Waze, it also throws my cell phone system off and will be taken into wrong streets. Vehicle navigation also detected me driving in the wrong direction alerting me not to enter the street. I took video and photos to the dealer starting in June 2023 for a few months. They first updated the software which took over 6 hours, and my car spent the night at the dealer. Unfortunately it did not resolve the problem, they then replaced the antennae also not resolved issue. They replaced the camera in front of the rear view mirror, also no resolution. Lastly they replaced the head unit under warranty. My car continues to have the same issues with navigation and recently while on the freeway and the navigation and Apple CarPlay were acting up, when i switched lanes it alerted me of a collision and car began to break while driving 65 miles per hour. This could have caused me to be rear ended if there would have been a vehicle behind me. I do not feel safe driving this vehicle which i depend on the navigation maps for my daily work. Currently i am avoiding driving on freeways to ensure the car does not suddenly brake and cause me to be rear ended. I contacted the dealer and they stated they have not detected the same issue on other Ioniq 5's sold at Puente Hills Hyundai in California. In researching through the internet, i found there are others experiencing the same issue. Dealer asked me to bring my car in once again, but i am not sure what they can do. Hyundai needs to figure out the issue on the Ioniq 5 navigation system and issue a recall to fix the safety glitch with the system.

12/30/2023 at approximately 5:30pm I drove home with the iPedal engaged. The iPedal was installed as a download to the car in August 2023. My daughter was in the passenger seat. I began to back into my driveway but shifted to Drive to straighten out the car. The car began to roll forward I took my foot off the accelerator but the car did not stop in spite of the iPedal being engaged. I put my foot on the BRAKE PEDAL which went all the way to the floor to no avail. The car rolled slowly into an electrical pole which brought the car to a stop. I previously filed a complaint here but I have since learned that iPedal may disengage if the car is put into reverse. There is not warning light to alert the driver that iPedal is disengaged so the driver is not aware that the brakes may not work as expected. I believe this may have been the case with my accident. However, this does not address why using the foot pedal for the brakes did not stop the car. Have found many complaints about this on various forums and believe this aspect of iPedal needs to be addressed. It should not disengage unless the driver instructs the car to do so. I have not been able to find documentation of this functionality in the manual, but I have searched and will continue to do so.

Neither iPedal nor the foot brake pedal had any effect on the car. I then put the car in reverse and backed the car into my driveway and the brakes worked as expected. When I then attempted to charge the vehicle, the charging port would not open. I could hear the motor working to open but it would not open. On 1/11/2024, I again attempted to open the charging port and this time it did open. So, TWO different electrical (including the brakes and the charging port) were not working properly. Inasmuch as the charging port again worked and the brakes again worked, there clearly was a malfunction that was random and unpredictable and also nearly impossible to reproduce but if this happened once, it could happen again. The brakes failed. Had we been traveling on a busy road, or at the top of a hill, we would have faced significant bodily harm, death to ourselves or others. This car had been driven locally, predominantly on local roads. The car is available for inspection and has only been driven on the dealer lot since the the incident. There were no warning lights or messages to indicate that any issues existed. The manufacturer has executed an inspected but has not reproduced the problem. The manufacturer and dealership claims the car is safe to drive without addressing the brake failure. I do not believe the car is safe to drive without a determination of how the bake failure happened or without a repair to that issue.

The car proceeds by itself and does not stop even when brakes are engaged. This happened 2 times. Once in reverse and once forward, both caused severe damages to the car. In the forward incident, the driver was wearing seat belt but air bags deployed with impact.

Intermittent Electrical / Brake failure . Put vehicle in reverse . Brakes failed to stop vehicle . Vehicle has had electrical issues before . Waring lights came on for steering and tire pressure in the past . Recall then came from Hyundai for the ICCU .45 minutes before the collision, the front collision warning came on when I was stopped at an intersection with no one in front of me . I stopped the vehicle and turned the vehicle off . Turned on the vehicle and it reset the warning lights and sounds , and I continued to drive . Later while pulling out of a parking space in reverse ,I stepped on the brakes but the vehicle did not stop . I pumped the brakes to no avail . After the collision , the brakes worked again when the tow truck driver placed the vehicle on the flatbed .The Hyundai dealer , after the vehicle had been in their possession for several weeks with the battery disconnected, said there was no problem They also had a factory rep look at the vehicle and he made a report . We have not been able to get a copy of the report . Vehicle is still at the dealership . When notified by the factory rep of their claim that the vehicle had no problems , the vehicle had been out of service more then 30 days .

The following is an incident that happened yesterday to my wife, who is an experienced driver: Whilst driving south on I5 from Seatac toward Tacoma in heavy rain with surface spray the screen above the steering wheel mentioned collision avoidance was not working and radar was not working. When I applied the brakes to slow down no response from the brakes. I then let off the brake and tried several times to apply the brakes each time with no success. I still had full control of the steering and could still steer the car. This lasted for about 30s then the brakes started working again. The error messages regarding collision avoidance and radar not working would continue to pop up even after the brakes were working. After stopping the car and turning it off and restarting several hours later the car no longer displayed any error messages. No vehicle or passenger were hurt in this incident, however if the traffic had slowed or stopped this outcome could have been very different.

When driving in I Pedal mode (which is the one-pedal mode), the brake lights do not light up when decelerating, and it almost caused us to be rear-ended yesterday. Drivers pull aside me and tell me my brake lights don't properly come on when I slow down. Apparently this defect was on the national news several months ago and Hyundai promised a software fix by mid-July. I was just at the Carlsbad, CA Hyundai Service Center this morning and was told they do not have a software fix for this issue. This car should officially be recalled by Hyundai and the NHTSA and owners should be warned officially they are driving a dangerous, defective car.

When using the one pedal driving or "iPedal" mode, the brake lights do not illuminate when braking unless your foot is completely off of the accelerator pedal. This is very dangerous because the vehicle can be stopped very hard without completely removing your foot from the gas pedal. I have stopped using this mode after almost being rear-ended three times within a week due to the car behind me not realizing that I am braking. This issue is gaining media attention. After doing research online, I have learned that there are lots of other people that have noticed the same behavior and have stopped using this mode as a result.

Drivers following the car are not aware the car is actually braking not coasting. This is causing a rearending hazard. The Ioniq 5 when using level 2 regen, level 3 regen, and i-pedal (one pedal driving), does not turn on the break light when decelerating, even if coming to a full stop unless the driver removes their foot completely from the pedal or hits the break pedal. These driving modes allow the driver to perform regenerative breaking almost to a complete stop without turning on the break lights and warning other drivers behind you. With i-pedal specifically, the driver can 100% perform all stop and go traffic maneuvers without using the break and never have the break lights activate. This appears to be the default behavior of all Ioniq 5 vehicles and is controlled via software. The vehicle does not appear to measure rate of deceleration so even if the driver were to decelerate rapidly, the brakes would not activate. When using the most aggressive braking energy recovery mode in the Hyundai Ioniq 5 (i-Pedal), the brake lights do not illuminate despite the aggressive nature of the braking provided by this mode. This mode brings the car to a full and complete stop and is very similar to commanded braking via the brake pedal. When using the L1-L3 braking energy recovery modes, the deceleration experienced by the car is more or less mild enough that it would be similar to a downshift in a manual transmission car. But the brake lights only illuminate if your foot is fully off the accelerator pedal. I am not aware of the regulations or SAE guidance to know if this is consistent with best practice, but it could be better with the brake light coming on once a certain deceleration (x m/s2) is reached. It is my opinion that a software change in the car's ECU could effect a fix for this. A mandatory recall would force the issue with Hyundai to actually do something about it. Note that Kia and Genesis share the same basic EV platform and may share the same logic fault in brake light activation. The regenerative braking will not turn on brake lights unless the accelerator is fully unpressed. In the more aggressive modes (L3 and I-Pedal), the stopping power is very strong. Without brake lights, it catches other motorists off guard and they nearly hit me. For reference, I-Pedal mode is able to bring the car to a full stop on its own. This is dangerous. The brake lights need to come on depending on the rate of deceleration, not the accelerator pedal state. This can happen at any speed, but becomes more dangerous at highway speeds.

When using most of the regenerative braking levels, including the one-pedal driving mode named “i-Pedal,” the brake lights will not illuminate until the drivers foot is fully off the gas pedal, or manually applying the brakes. This can create a dangerous, life threatening scenario leaving surrounding drivers unaware the vehicle is slowing. This has not been reproduced by the automaker not inspected by them, police, insurance, etc but has been verified on video. No warning lamps appeared.

When using i-Pedal (one-pedal driving) mode in this vehicle, it can decelerate at a very fast rate but does not illuminate the brake lights unless the driver’s foot is completely off the pedal. For example, I can be driving 60mph and decelerate to ~2mph as quickly as other drivers coming to a complete stop, but my brake lights will not illuminate until my foot is **completely** removed from the pedal. This is contradictory to the way one-pedal driving works, since coming to a smooth and slow stop requires that you slowly lift off the accelerator pedal, during which the brake lights do not illuminate because your foot is still pressing the pedal (albeit less and less as you come closer to the stop).
