So we call Lexus make appointment see what’s going on they told me fuel pump problem we need change. We google saw Lexus a lot car recall beacuse this issue. So we so mad . we bought a lot car they never have this problem. It was so danger . If you car not any light on it show you carnot any problem you can driver but it was so danger. I know Lexus recall a lot this problem car we got similar problem they don’t know fix my car .they don’t care about us safety. They just care about money sell to you car that’s over .

While driving, my vehicle's check engine light came on and the vehicle entered limp mode without warning. It would not exceed 25 MPH and was stuck in 2WD, displaying the message "reduced engine power – take vehicle to nearest dealer." This created a serious safety hazard, as the vehicle could not maintain safe speed in traffic. The vehicle had to be towed to the dealership. The Lexus dealer diagnosed trouble codes P219C00, P160500, P030027, and P0301300 — cylinder #1 air/fuel ratio imbalance, rough idling, random misfire, and cylinder #1 misfire. Through active injector testing, the technician confirmed the cylinder #1 direct (high-pressure) fuel injector has failed, and recommended replacing all four direct injectors at a quoted cost of approximately $6,600. The dealer's inspection report also cited Lexus service bulletin L-SB-0007-21 for possible worn exhaust valve guides, a separate documented defect in this engine that produces these same misfire codes and limp mode, with a repair (cylinder head and valve replacement) that can exceed $10,000. Lexus has known about these failures in the 8AR-FTS engine since at least March 2021, when it issued L-SB-0007-21 covering 2018–2021 NX 300 vehicles. The bulletin acknowledges affected vehicles enter limp mode. Despite this, Lexus has not issued a recall, and owners whose vehicles fail shortly after the warranty period expires are left paying thousands of dollars for a known defect. My warranty coverage lapsed shortly before this failure occurred, and Lexus corporate declined meaningful assistance. A vehicle that suddenly cannot exceed 25 MPH on public roads is a crash risk to its occupants and surrounding traffic. I am requesting NHTSA investigate this defect for a potential recall.

The message “Electronic Fuel System Failure” was displayed, and the vehicle stalled on several occasions. The vehicle was taken to the local Auto Zone, and the contact was informed about the fuel system failure. The vehicle was later taken to the local dealer, who diagnosed that the fuel pump needed to be replaced. The vehicle was not yet repaired. The manufacturer was notified of the failure and informed the contact that the vehicle was not included in an extended warranty coverage associated with the failure. The failure mileage was 88,000.

No warning. I had four high school wrestlers in my car and we had just gotten off the highway after driving for 7 hours from Panama City to Port St. Lucie when it happened. My car went into limp mode making it dangerous to continue to drive. My car is currently at the service center at Lexus of Treasure Coast off US1. The technicians have verified that l have a faulty fuel pump and mentioned the recall but stated my year was not included. With my vehicle being a 2020 I feel as though there is a high likelihood that my fuel pump and others are the same as what has been used on previous years that is not being recalled. I would appreciate you reviewing this matter as it seems enough of a coincidence that I could be right.

While driving at interstate highway speed at approximately 70 mph in heavy rush hour traffic, the vehicle suddenly made a popping sound and immediately lost all engine power. The accelerator became unresponsive, and the vehicle rapidly decelerated without warning. I was forced to coast across lanes to the shoulder to avoid a collision. The engine shut down completely and the vehicle would not restart. This created a serious risk of injury or death due to loss of power in high-speed traffic. The vehicle had to be towed and is currently undrivable. Prior to this incident, the vehicle had a long history of repeated engine misfires. Spark plugs were replaced twice, and a fuel line repair was performed. Despite these repairs, the misfires continued. The sudden engine shutdown occurred without warning.

I am submitting this complaint to report a significant safety defect in my Lexus vehicle. Without any prior warning or history of mechanical problems, the vehicle experienced a sudden and severe loss of engine power while I was merging onto a busy highway. The vehicle rapidly decelerated despite continued acceleration input, forcing me to navigate to the shoulder while surrounding traffic was traveling at high speed. This unexpected loss of power created an immediate and substantial risk of a collision and posed a serious danger to myself and other motorists. Following this incident, I took the vehicle to a local auto shop, which identified diagnostic code P030002, indicating a misfire in Cylinder 2. I then promptly brought the vehicle to an authorized Lexus dealership. Their diagnostic confirmed the same code and attributed the failure to a “Cylinder 2 direct fuel injector leaking.” Lexus has already acknowledged this defect in TSB L-SB-0007-21, demonstrating that the manufacturer is aware of this recurring issue. My vehicle is five years old with fewer than 100,000 miles, yet this defect resulted in a dangerous loss of engine power under highway conditions. The dealership quoted over $3,000 to replace a single injector, and the remaining injectors may fail at any time, further compounding the safety risk. Despite its knowledge of this defect, Lexus has not issued a recall, implemented preventive measures, or offered assistance, citing that the vehicle is slightly outside the 70,000-mile warranty. I have therefore been left with a vehicle that is unsafe for continued highway operation. Given the severity of the incident, the known recurrence of this defect, and the manufacturer’s lack of corrective action, I respectfully request that NHTSA investigate this matter, as it presents a substantial and ongoing hazard to consumers and the driving public.

I am writing to formally express my concern regarding ongoing issues with my vehicle following repairs performed at Crown Lexus in Ontario. In December 2024, I experienced rough idling, slow acceleration with vibration, fluctuating RPMs, and stalling. The check engine light turned on, and an error message instructed me to pull over and contact a local dealer due to engine/transmission trouble. I promptly brought the vehicle to Crown Lexus, where it was diagnosed with a faulty cylinder head requiring full replacement. The repairs were completed at that time. However, over the past four months, the same issues have reappeared. I returned the vehicle to Crown Lexus, where I was told no problems were found and the car was safe to drive. Unfortunately, just two days later, the issues persisted, and I am now bringing the vehicle back in for a third time with the same engine/transmission symptoms. Given the recurrence of these serious issues and the prior repairs conducted, I kindly request a thorough diagnostic review and a resolution to ensure the vehicle is safe and functioning properly. Two (2) times (6-weeks apart) while on Freeway, loss of power and some shaking occurred, making it difficult to bring vehicle to a safe stop. Vehicle has only 80k miles, low for a Lexus brand. Other cars moving at freeway speeds made it more dangerous to stop vehicle safely without getting hit. Failure in both circumstances was related to fuel injectors, cylinder no. 4 at first then second failure six weeks later was on cylinder no. 2, the fuel pump appears to be a known problem on these cars and many Toyota/Lexus models. Low fuel pressure is stressing the injectors to the point of ultimate failure. My wife and kids nearly crashed on the freeway on the first incident, the vehicle with reduced power makes it difficult to control at freeway speeds. Vehicle is well maintained, no warnings were evident, immediate failure occurred. Dealer where vehicle was purchased indicated no alerts/bulletins were evident, repair shop (not dealer) also indicated no bulletins were evident on the first incident. Second failure, another local Lexus dealer indicated no alerts/bulletins where evident of any problems associated with the failure. When escalating issue to OEM/LEXUS Brand Engagement Center, they were able to confirm (they were able to read the failure code when the repair shop connected to the vehicle) the failure mode symptoms was associated with a fuel pump alert. Why both Lexus dealers did not know this is odd. My vehicle has been at the dealer now for over 30 days with no feedback from Lexus on the repair. This failure condition at freeway speeds is a "Safety" risks, Lexus should take this seriously, the fuel pump and weakened injectors should be recalled before someone gets hurt. Please help NHTSA!

It sometimes takes two to three tries for someone to successfully put on the seat belt. other times the first time works. Bridgestone 235/55 R18. I was driving in the far left Lane on the interstate going 65mph. The tire pressure warning came on. The vehicle seemed too shaky to continue on. I attempted to merge right to get off on the next exit and the vehicle began to shake violently. Before I could exit safely the tire shredded with several loud pops in the 3rd lane of traffic before I was able to merge to the exit. I had to stop on the side of the interstate. I was strong enough to keep control of the vehicle by gripping the steering wheel. The safety of myself and my children were in grave danger traveling on a busy interstate. The contact owns a 2020 Lexus NX. The contact stated while driving approximately 35 MPH and attempting to make a turn, the steering wheel was difficult to operate. There was no warning light illuminated. On another occasion, the steering wheel veered to the left and right. In addition, there was a sudden unintended acceleration. The brake pedal was depressed but the vehicle did not reduce in speed. The contact crashed into a side tree, columns, and a divider in order to stop the vehicle. All of the air bags did deploy as intended. There was smoke that caused the contact and his wife to have difficulty breathing. The vehicle was towed to the dealer; however, was not diagnosed or repaired. The vehicle was then towed to the contact's residence. The vehicle system automatically contacted emergency. The police and fire and paramedics arrived on the scene. There was a police report filed. The contact and his wife were unconscious. The police cut the seat belt to pull the contact out safely; however, the wife released herself. The wife injured her left knee; while the contact was injured with bruises and smoke inhalation. The paramedics transported the contact and wife to the hospital. They both received medical treatments and x-ray, then were admitted to the hospital for an overnight stay. The manufacturer was notified of the failure. The approximate failure mileage was 65,00.

I was on a major interstate (I-495 in Va), driving at a speed of 60 to 65 miles per hour, when my driver side door suddenly opened. The dashboard alert came on that the door was open and the interior light came on. With my left hand, I held on to the door for my dear life, as I tried to pull over in a safe area to shut the door. Lexus has tested the car and could not replicate the issue of the door opening while driving at a high speed. I'm very concerned about my safety with this car. I do not feel safe at all. ON 11-05-2020 I WAS DRIVING MY NEW LEXUS (3200 MILES ON IT) ABOUT 55 MPH ON A 4 LANE HIGHWAY IN LIGHT TRAFFIC. IT WAS A SUNNY AFTERNOON AND 84 DEGREES. ALL OF A SUDDEN THERE WAS A VERY LOUD EXPLOSION SOUND FROM ABOVE OUR HEADS AND THEN THE SOUND OF GLASS FLYING OFF FROM THE VEHICLE ONTO THE ROADWAY BEHIND US. THERE WAS NO ROAD DEBRIS, FLYING ROCKS OR CEMENT TRUCKS NEARBY THAT MIGHT HAVE CAUSED A ROCK TO FLY UP AND BREAK THE GLASS ON THE SUNROOF. I THOUGHT WE HAD BEEN SHOT AT. I WAS ABLE TO PULL OVER OFF THE HIGHWAY AND GOT OUT TO INSPECT THE DAMAGE. THAT'S WHEN I REALIZED THE SUN ROOF HAD EXPLODED. IT EXPLODED UPWARD AS THE SHARDS OF GLASS INDICATED. THE INTERIOR SUN VISOR PORTION HAD BEEN CLOSED WHEN THIS EXPLOSION TOOK PLACE OTHERWISE MY SELF AND MY PASSENGER WOULD HAVE BEEN SERIOUSLY CUT BY FLYING SHARDS OF GLASS. IT WAS AT THIS POINT I TOOK THE PICTURES THAT I AM ATTACHING TO THIS REPORT. WE IMMEDIATELY CALLED THE LEXUS DEALER AND BROUGHT THE CAR TO THEM ON THE NEXT DAY. AFTER 1 WEEK THEY SAID THE DAMAGE WAS CAUSED BY FLYING DEBRIS BUT DID NOT EXPLAIN HOW THEY CAME TO THIS CONCLUSION. THEY ALSO SAID THIS WASN'T UNDER WARRANTY AND WE HAVE TO PAY $1700 TO HAVE IT REPAIRED. TO DATE, 11/24/2020 WE STILL DO NOT HAVE OUR VEHICLE BACK FROM THE LEXUS DEALER. WE HAVE FILED A CLAIM WITH OUR INSURANCE COMPANY, STATE FARM. I DISAGREE WITH THIS EXPLANATION AND HAVE FOUND HUNDREDS OF DOCUMENTED CASES SUCH AS MINE THAT HAVE BEEN REPORTED TO NHTSA WHERE THE SUN ROOF EXPLODES OUTWARD FOR NO REASON. I HOPE THE NHTSA WILL INVESTIGATE THESE ACCIDENTS AND REQUIRE MANUFACTURERS TO USE BETTER, SAFER GLASS IN THE SUNROOFS. THESE SUNROOFS ARE BECOMING VERY COMMON. THE CONTACT MENTIONED THAT THE VEHICLE WAS NOT DRIVEN SINCE WEDNESDAY FROM THE INCIDENT DATE ON SATURDAY. THE CONTACT STATED THE FIRE WAS EXTINGUISH BY THE FIRE DEPARTMENT, AND A FIRE DEPARTMENT REPORT WAS FILED. THE INSURANCE COMPANY INSPECTED THE VEHICLE AND STATED THE CAUSE OF THE FIRE WAS UNDETERMINED. THE CONTACT MENTIONED THAT ONLY THE FRAME OF THE VEHICLE REMAINED AFTER THE FIRE WAS EXTINGUISHED.
