The passenger side active head restraint (AHR) burst open injuring the owner who was sitting in the passenger seat while the vehicle was moving. There was no rear end impact and there was no reason for the AHR to malfunction.

AFTER BEING REAR ENDED AT A STOPPED POSITION AND PUSHED INTO ANOTHER STOPPED VEHICLE IN FRONT, THUS CAUSING A DOUBLE COLLISION, THE DRIVER SEAT HEADREST COMPLETELY EJECTED FROM DRIVER SEAT. DRIVER SEAT ADJUSTMENT BAR CAUSED DEEP TISSUE DAMAGE TO LEGS. THIRD ROW SEAT SPLIT AND SEPARATED SEAT SURFACE FROM BACK SUPPORT. INCIDENT TOOK PLACE AT A REDLIGHT FOR A LEFT-TURN LANE FROM THE HIGHWAY TO AN INTERSECTING ROAD. VEHICLE WAS STRUCK BY A MINIVAN STILL MAINTAINING FULL HIGHWAY SPEED OF AT LEAST 55 MPH.
