I am reporting a possible safety defect in my 2024 Toyota Sienna Hybrid AWD, VIN [XXX] . The vehicle intermittently surges, lurches, or creeps forward while I am pressing the brake pedal and attempting to slow or stop. It feels as though the vehicle briefly adds power or braking force momentarily decreases even though my foot remains on the brake. The sensation is not normal creep alone; it feels like a brief unintended forward push while braking. The problem is intermittent, which makes it difficult to predict, but it creates a safety concern because it can happen when slowing, stopping, or maneuvering at low speed. I am concerned this could increase stopping distance or cause the vehicle to move forward unexpectedly. I am requesting investigation of possible defects involving the braking system, hybrid regenerative braking transition, brake actuator, pedal sensing, or related control software. It happens daily. While driving on highway, on 3/11/26, car powered down. I had to coast to side of highway. I was able to turn vehicle on after several minutes, but SEVERAL system malfunction error messages (hybrid, accelerator, pre-collision, tpms, airbag, engine…) appeared. The vehicle would not accelerate beyond 10mph. Once I moved it to a safe location, I had the vehicle towed to the dealership. The dealership technician was able to replicate the issue during test drive. The technician apparently had the vehicle towed back to the dealership last night. The car has had other related issues since June 2024 when I bought it. The TPMS system, the accelerator system, and the airbag system have all malfunctioned independently. The accelerator system malfunctioned in Jan. I had to pay for a replacement fuse despite the warranty. The vehicle is modified with Braunability equipment for wheelchair access. These modifications are approved by Toyota and were completed by MobilityWorks. Aside from the vehicle itself being unsafe to drive, the passenger sliding door has had numerous problems. On 3/07/26, the door mysteriously slammed shut after opening and would not open again. These are all dangerous situations that could have resulted in fatal accidents.

Looked online to see if other Sienna drivers had the same issue with the hybrid system. Brakes are accelerating a bit even when already pressing on the brakes. It has happened enough times to be concerning. We have not taken it to the dealearship as of yet but will be getting in touch soon , now that we know it's a known issue. No warnings from vehicle.

There are no messages/warnings. The vehicle will surge at very low speeds such as when I am coming to a stop, trying to avoid a person in a parking lot etc. It was SO scary this evening as I accelerated into a very very busy intersection...I pressed on my brakes harder and it was instant - but about 1 second (which is a LOT in this scenario) later it then slowed instead of accelerated. I have owned the vehicle almost a year. I will be bringing in for inspection this week and let the dealer know. I have put on about 11K in miles and would say this has happened a dozen times thus far with this evening being the scariest one yet. I figured it was just something normal for a hybrid but as I researched online - many people have this issue. TOO SCARY for me and dont want to hurt anyone or myself.

Van purchased new October 2024. On two separate occasions, one in late 2024 and next today, [XXX], when pulling into street from a stop, there is no throttle response. You push the accelerator at varying pressure and no response for 2-3 seconds. This is a severe safety issue as it could cause an accident with oncoming traffic. No error lights illuminated on dash. Have seen other complaints on NHTSA website and social media as well. Will take to dealer but reporting a major safety issue. When crossing a one-way road with oncoming traffic from the left, a vehicle that has recently passed might be triggering the forward collision avoidance system which cuts off throttle control to the Sienna, even when the passed vehicle is well out of the way. It is unknown if the collision avoidance feature is being triggered as there are no warning lights or messages in the display. This results in the Sienna having no accelerator response and sitting “dead in traffic” sideways in front of oncoming cars. This this is the second time this issue has occurred and today I was nearly t-boned by on-coming vehicles as there was no throttle response to move the car forward. This issue has been reported in 4th generation Sienna (‘21-‘24) forums on Facebook. This issue puts at risk vehicle occupants as well as other vehicles on the road.

Vehicle was driving on a slow traffic local street at roughly 10MPH. Spacing between car in front of me was around 30 ft. I was gliding towards front and ready to brake. Suddenly the vehicle start to accelerate hard by itself without driver’s operation. I tried to engage brake / gas pedal and was not helping. Front collision system did not activate like it suppose to. I ended up turn sharp right towards road curb and hit pole at average speed of 30mph or less. No air bag deployed. I then switch to Park. No warning lights pop up. All safety features on vehicles were turned on before the incident. Whats weird is the car is functioning normal now after incident. I contacted Toyota USA and waiting for feedback.
