What component or system failed or malfunctioned, and is it available for inspection upon request? -The engine malfunctioned. It is available for inspection. How was your safety or the safety of others put at risk? -The engine went out at 77k miles in the middle of driving with no warning. Could have caused a serious accident. We have always done proper oil changes. The rods separated from the pistons and punched a hole in the engine block. Has the problem been reproduced or confirmed by a dealer or independent service center? The dealer diagnosed it and our mechanic said this has happened with Toyotas too recently Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? -no Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? -no warnings or messages before the failure of the engine.

While driving on highway, on 3/11/26, car powered down. I had to coast to side of highway. I was able to turn vehicle on after several minutes, but SEVERAL system malfunction error messages (hybrid, accelerator, pre-collision, tpms, airbag, engine…) appeared. The vehicle would not accelerate beyond 10mph. Once I moved it to a safe location, I had the vehicle towed to the dealership. The dealership technician was able to replicate the issue during test drive. The technician apparently had the vehicle towed back to the dealership last night. The car has had other related issues since June 2024 when I bought it. The TPMS system, the accelerator system, and the airbag system have all malfunctioned independently. The accelerator system malfunctioned in Jan. I had to pay for a replacement fuse despite the warranty. The vehicle is modified with Braunability equipment for wheelchair access. These modifications are approved by Toyota and were completed by MobilityWorks. Aside from the vehicle itself being unsafe to drive, the passenger sliding door has had numerous problems. On 3/07/26, the door mysteriously slammed shut after opening and would not open again. These are all dangerous situations that could have resulted in fatal accidents.

My 2024 toyota sienna suddenly stopped for 2-3 seconds while driving in the city roads. I was caught off-guard with that behavior. I kept pushing on my gas pedal and after few hard pumps the power came back. This happened couple of times so far. Not sure what the main problem is.

The contact owns a 2024 Toyota Sienna. While the contact’s wife was driving 5 MPH out of a parking lot and attempting to make a left turn, the vehicle suddenly lost motive power and stalled. After approximately 3- 4 seconds while depressing the accelerator pedal several times, the vehicle eventually accelerated as needed. The vehicle was driven to the residence. There were no warning lights illuminated. The vehicle was not taken to the dealer to be diagnosed or repaired. The manufacturer was not informed of the failure. The failure mileage was 68,000.

Hello, leader. I bought my car (Sena Platinum Edition) for three months. Due to a mistake in the appearance, I need to replace the engine now. My new car will definitely depreciate if the engine is replaced, so I hope Toyota headquarters can refund me in full. I need to buy a new Sena! My house is full of Toyota cars, no matter what the result of Toyota Company's handling? I will still choose to buy a Toyota! Thank you!

I was at a complete stop. There were no obstacles around me or oncoming. When I pressed the accelerator nothing happened for a full 3 seconds, until my vehicle jolted forward at an excessive speed. Brakes were not working.
