The contact owns a 2024 Tesla Model Y. The contact stated that while driving 25 MPH and making a right turn into an alley with another vehicle approaching, the contact stopped to reverse; however, while using the tap feature, the vehicle unintendedly accelerated and crashed into a concrete barrier twice. The vehicle was uncontrollable. The contact stated that the air bags failed to deploy. The crash was not captured on the camera inside the vehicle. A Police report was filed. There was no injury reported. The vehicle was towed to a towing lot and was next towed to the local dealer but was not yet diagnosed or repaired. The manufacturer was not contacted. The approximate failure mileage was 14,000.

• FSD received a software update earlier this same morning. • While navigating a parking garage under FSD, the vehicle initiated an unprompted reverse maneuver. • The reversal happened with no warning and no meaningful time for driver intervention before impact. • The vehicle struck a structural beam, causing damage to the vehicle above the rear driver side door, as well as shattering and causing splintering of the roof glass.

VEHICLE INFORMATION Year: 2024 Make: Tesla Model: Model Y Mileage at time of incident:1,288 Date of incident: XXX Location of incident: [XXX] On XXX, I was operating my 2024 Tesla Model Y in [XXX] when the vehicle experienced a sudden and unexpected malfunction of its Autopilot/autonomous driving system. The vehicle activated in Autopilot mode without my intentional input, causing me to lose control and resulting in a serious crash. Prior to the collision, the vehicle's Autopilot system, engaged without my input and abruptly accelerated. I was seriously injured as a result. The vehicle, a brand-new 2024 Tesla Model Y, was in normal operating condition with no prior mechanical issues. I had not modified the vehicle in any way. Following the crash, Tesla provided data to attorneys regarding the incident which I believe to be inaccurate and inconsistent with my experience. I am requesting that NHTSA: 1. Open a formal investigation into this incident 2. Investigate the accuracy and completeness of data Tesla provides following Autopilot-related crashes 3. The Tesla Autosteer (equivalent of adaptive cruise control) failed to brake on seeing the car ahead braking. In the 5 seconds before impact, the car ahead had started braking. The autosteer did not brake for the first 3 seconds and then It asked me to take over 2 seconds before collision. It did not apply automatic emergency braking causing me to rear end the car ahead of me on the freeway at around 30mph This is supposed to be a safety driver assist feature which instead was the primary cause of the accident. The take control was too late to avoid collision. This is a serious safety issue.

The **TESLA GPS navigation destination was set to a point marked in the middle of the park** (not a parking spot or road). **Without any driver input** (no accelerator pedal pressed, no hands on wheel), the vehicle **suddenly shifted into Drive, jumped the curb, and accelerated approximately more than 30 feet] into the park grass**. This caused **damage to wheels, and all the passenger side of the vehicle to impact with a large rock placed in the park **Zero pedal input** — confirmed by driver and will be verified in Tesla server logs. No injuries. No other vehicles involved. **Root cause**: **Full Self-Driving (FSD) software / navigation system** incorrectly engaged **Lane Keeping Assistance** while parked, triggering **unintended acceleration** via **Speed Control** logic. The car was on software update and suddenly disappeared. The car **autonomously drove itself** toward the GPS point **from a complete stop**. Police report filed to LAPD (Case #[XXX]). I am writing to formally report a serious malfunction involving my Tesla vehicle. On my 2024 Tesla model y.I experienced an unexpected accelerator malfunction that caused the vehicle to surge forward uncontrollably. The incident occurred at approximately As I attempted to dive in slowly, the accelerator suddenly malfunctioned and my car creeped in slowly and rapidly increased power without my intention. Despite immediate attempts to brake and regain control, the vehicle continued to accelerate and subsequently collided with a gate, resulting in significant front-end damage. I firmly believe this was due to a mechanical or electronic failure in the vehicle’s accelerator or related control systems. The malfunction posed a serious safety hazard and could have resulted in injuries, and this has affected me mentally. I am requesting a full investigation into this malfunction, including an inspection of the vehicle’s electronic logs, sensors, and accelerator system to determine the cause of this unintended acceleration. The daughter only had pain in her neck and shoulders; there were no cuts or abrasions. The police arrived and issued an incident report. I was in a parking lot on [XXX] in Mamaroneck, New York waiting in line to get out at approximately [XXX]. Many cars were ahead of me due to the school drop-off rush, with parents and kids lined up in the parking lot waiting to leave. While I was waiting, the Tesla unexpectedly accelerated on its own, but I managed to hit the brakes just in time to avoid colliding with the car in front of me. A few seconds later, my car suddenly accelerated again without any input from me. I tried to steer away from the cars in front while repeatedly pressing the brakes, but they weren’t working. All I could think about was avoiding hitting another car or person, so I swerved the car toward the building in the parking lot. The impact was hard, and the car finally stopped, but the airbags didn’t deploy. I’m deeply shaken by the incident; I could have seriously injured someone or even worse. When I got home, I discovered that this issue is a known problem with Tesla, yet they’re placing the blame on drivers. Having never been in a car accident before, this experience has been overwhelming. As a mother and attorney, I believe it’s crucial to examine this situation more closely. Tesla controls the data, and it’s clear that responsible drivers wouldn’t suddenly veer into a building in a crowded parking lot without cause. I urge you to act on this life threatening defective car and investigate this problem before it is too late. The contact stated that while driving approximately 15 MPH the vehicle suddenly experienced unintended acceleration causing the driver to lose control of the steering and crash into a second vehicle. During the crash, the frontend of the vehicle was severely damaged but the air bag did not deploy. The contact sustained a right-hand injury that did not require medical treatment. Three occupants from the second vehicle sustained undisclosed injuries and were transported to the hospital. A police report was taken at the scene and the vehicle was towed away. The cause of the failure was not yet determined. The manufacturer and local dealer were notified of the failure. The failure mileage was 7,608.

The vehicle was under full self-driving mode and approaching a shopping center. There was a crossroad with signs on the corner. The vehicle was supposed to turn left but kept slowly approaching the signs on the corner. I was ready to step on the brake to stop the full self-driving mode while expecting the vehicle to turn left or step on the brake. Instead, the vehicle accelerated toward the signs, so I had to steer the wheel to the left and apply the brake immediately. The vehicle jumped a bit as it ran over the corner of the crossroad and stopped after about 10 feet. I was fortunate because I took action immediately, avoiding hitting other people or property. However, human reactions seem not to be quick enough to prevent some accidents as in this case. During this incident, the right corner of the front bumper of the vehicle hit the signs, and the top right corner of the vehicle (right above the front window) was also slightly damaged.
