Component: Honda Collision Mitigation Braking System (CMBS/AEB). Vehicle is in GEICO insurance claim process, not yet released. EDR data has not been extracted and is available for inspection if preserved before the vehicle changes hands. Safety risk: Two failure modes. (1) False AEB activations occurred multiple times — while driving at ~20 mph and while reversing at low speed, with no obstacle present. Hard sudden braking with no warning created rear-collision risk. (2) On 6/23/2026 at ~9:50 AM, AEB failed to activate when I struck a parked vehicle. Airbags deployed on impact, briefly incapacitating me. My foot remained on the accelerator. Standard post-collision safety design calls for automatic power cutoff and brake application after airbag deployment, specifically to prevent continued acceleration when a driver may be incapacitated. This vehicle did neither — it continued accelerating, significantly worsening the collision. I sustained soreness and bruising. Dealer confirmation: I did not take the vehicle to a dealer for the phantom braking. Per other NHTSA complaints for the Prologue and Honda's broader CMBS issues (Engineering Analysis EA25002), dealers have reportedly been unable to replicate the issue and told owners the behavior is normal. Inspections: GEICO assessed the vehicle for the claim. No inspection by Honda or independent mechanic for AEB specifically. EDR data not yet extracted. Warning indicators: During false AEB activations, the collision warning appeared on the dashboard at the moment of braking, not before. No advance warning. No persistent warning lamps or diagnostic messages indicating CMBS malfunction at any point during ownership. Phantom braking occurred intermittently throughout ownership.

I have had two safety issues with this vehicle where there were system failures that did not record DTC codes. And an additional failure that does not record codes and cannot be duplicated. After leasing the vehicle for seventeen months, and during repair for an unrelated non-safety issue, it was discovered that the steering wheel airbag had a loose connection. No warning appeared at any time on the instrument panels. As well the one pedal function would switch into regular drive mode, as in a gas powered vehicle, randomly. After multiple tries and incidents the dealer service department has been unable to duplicate. Then on March 6 of this year I bought in the vehicle yet again for this one pedal failure, and was notified there was an existing recall. This software update was performed. On March 17, just 11 days after the update, while backing into my garage with the garage door going up the windshield wiper was clipped by the door, destroying both the wiper and shattering the back window. I have backed into my garage thousands of times and never had an issue. Why this happened on this date I do not know. What I do know is that the vehicle gave no warnings and did not break for the object. At the time my dogs were clipped into their seat belt harnesses in the center well. By a miracle no glass shattered beyond the back storage area and I was able to safely remove them. I checked after and all of the collision warning systems were listed as active, including automatic braking for rear collisions. I had the dealership run scans twice and all scans showed no DTC codes and all systems active. I can only assume that all prior updates have failed, and there is a substantial defect to the vehicles ability to communicate. I have a copy of the scan performed. The second scan was run on April 9 because the dealership failed to provide a printed copy of the first. I have that second scan. My vehicle fails to communicate or activate essential information, a safety hazard.

Shortly after purchase, I observed damaged/torn underhood insulation and reported the condition to the selling dealership, with concern that the vehicle should be inspected for related issues. Prior to a crash incident, I experienced concerns involving braking-related behavior and advanced driver assistance features, including lane assist and object/collision warning functions. During the crash, while reacting to a roadway hazard, the vehicle swerved and struck a pole, and I did not experience warning or intervention from driver assistance features as expected. Following the incident, a diagnostic pre-scan identified multiple faults involving brake control, radar sensors, image processing, and communication systems. Since the incident, similar concerns have continued, including recurring braking/driver assistance concerns, and the issues remain unresolved. I am submitting this complaint due to concern that braking and advanced driver assistance systems may not have functioned as intended and may present an ongoing safety risk.

Direct cause - Unknown. What I do know however was that the automatic emergency break system on my vehicle was activated, causing the vehicle to stop in the middle of driving. This resulted in the vehicle behind me hitting my car. Prior to the incident occurring, no warning indicators (lamps, messages, etc.) came from the dashboard. The vehicle just stopped. Immediately after this accident occurred. I turned off the forward collision safety feature, as I was scared of the issue (vehicle stopping) repeating itself. To date, my vehicle has not been repaired or my insurance agency contacted, as I do not believe I should be held responsible for the cost of this repair.

On 2/19/2026 I was taking an freeway exit ramp from I 15 south to Carrol Carrol canyon and I stepped on the brake but the car did not slow (my perception was it was speeding up), the vehicle collision warning did not sound and the vehicle did not emergency brake, therefore I hit a stopped vehicle at a stoplight. Automatic Emergency Braking often triggers for no reason when backing up this vehicle. This is a safety issue 1) Random stops in a parking lot are disorienting to both the driver of the car and the drivers around them 2) The AEB system creates a very hard stop. During the last incident I was with my father who had recently had neck surgery and had additional pain for about 2 days after the false AEB trigger. Thankfully, it did not trigger any long term complications.

I have had multiple issues with this vehicle and it has been in the shop more than 80 days between April and Oct 2025. First, the entire battery system went out in the all electric vehicle. Honda said that since it was a collaboration car with GM, that it would take a long time to fix because their technicians weren’t very familiar. It took almost 8 weeks in the shop before we got it back. Not long after, the entire dash (electronics) went out and the shop had it again for over a week. they never found or could replicate the issue. The vehicle went back in after having multiple incidents of emergency braking when there was no emergency, or even vehicles in my vicinity. The final time this happened, I was traveling nearly 25 miles per hour and the car decided to slam on the brakes, making me feel like I hit a brick wall. My head slammed the steering wheel and my daughter was terrified. I drove it straight to the dealership and they said “what’s wrong with the car this time”…they had it for weeks, couldn’t replicate the issue and tell me it’s fine and to take it home. They suggest TURNING OFF THE SAFETY STSTEM AS A FIX, which seems criminal. The safety features should work properly. I shouldn’t be instructed to turn them off. I am very frustrated and have been working with Honda on a buy back out of good faith, but it’s been weeks and we have no answer. I am forced to drive a gas vehicle, costing me gas money now, when I bought an electric vehicle to avoid this cost. All around, it has cost me more money than it’s been worth and I’ve made multiple car payments as it’s sat at a dealership unusable. Again, it has spent more than 80 days in 6 months at the dealer with no resolution. My sound system blew out last time they had it checking out the braking issue so I waited another week to have that replaced, meanwhile the fix to the braking system was that I was told to turn the safety feature off. While driving my 2024 Honda Prologue, the automatic emergency braking system (part of Honda's Collision Mitigation Braking System) has activated unexpectedly and forcefully multiple times — with no obstacle or vehicle ahead. The most serious incident occurred on [insert date], while traveling at approximately [insert speed] mph. The vehicle suddenly slammed on the brakes without warning or any visible threat in the road. The sudden stop caused whiplash injuries to my wife and two children, who were passengers in the vehicle. We did not crash, but the vehicle behind us narrowly avoided rear-ending us. I have attempted to get the vehicle serviced, but the nearest dealership told me there is a wait time of over a month, so no diagnostic has yet been performed. Given the potential severity of this defect — especially involving passenger injuries and potential crashes — I am deeply concerned. Many other Honda owners have reported similar phantom braking behavior across multiple models, and I believe this defect needs to be investigated urgently by NHTSA. Honda has not issued a recall or provided public guidance on how to disable or address the system, leaving drivers at risk.

I turned my car on and it made an extremely loud, dangerously loud, loudest noise you have ever heard loud, static noise. I turned it off and waited and turned it back on and same thing. I tried shifting it to drive, and the noise stopped. However, every time I did anything where the vehicle would make a noise, it would make a full-volume static blasting noise instead. It was not safe to drive and I am afraid it has permanently damaged my eardrums. This is a known issue (see links below) and needs to be fixed immediately. Where to begin? not even 2 weeks after having this vehicle it nearly caused a major accident and gave me whiplash & neck problems (have doctors documentation as proof) when the automatic breaking system hard breaked my vehicle twice in the same trip (20 minutes from work to home) with the second time resulting in my car skidding quite a few feet on a highway in the middle of rush hour traffic (want to note the distance in front of the car was quite significant both times) also both CV joint axles need replacing on top of rear passenger door is not the right size & often pops open. Honda dealership has my vehicle for 31 days back in December January stating parts for door would be in within the month and the cv axle would be beginning of march. here it is March 26th and I've heard nothing from the dealership after multiple attempts of trying to get a hold of them. also worked with Honda and opened a claim who also is ignoring my calls. I hardly want to touch the vehicle as I'm afraid of it causing a major accident or the door popping open as I'm driving with my children in the car.

Break failure on highway causing MVA- multiple attempts were made to slow down and break vehicle to no response what so ever. Pressed breaks all the way down, nothing, no air, no resistance, no sounds, vehicle kept on going. In an attempt to save myself from an accident that could have been much worse, I jumped out of the moving vehicle, landing the back of my head on the cement highway floor toward left end of bumper. This vehicle is not safe and should be taken off the roads. I have 2 toddlers and am traumatized. The vehicle stopped after some impact in the middle of the highway. From the moment of receiving this car there has been some sort of issue with it's breaks. Initially short stopping on it's own when gas peddle not pressed- had the setting fixed for that to then experience multiple whiplashes for no reason while driving. Attempted to connect the starlink on multiple occasions as well to no success and just a bunch of wasted time trying. In addition to all the alarming safety concerns, this car would not heat up well- 40 minutes into my ride I'd start to feel some warmth. I would love to have this case investigated further and sue honda for allowing such a dangerous vehicle on the roads. My accident was bad but the honest truth is that it could have been worse and no one should have be subject to this danger.

Vehicle abbrupted stops when driving with the front collision on. I drive up the same hill everyday and when slightly turning left along a curve going up hill, and if there is a parked car present on the right side of the road, the car will abruptedly stop. If I slow down to less than 20 mph, the car won't stop, but doesn't seem like it should happen at normal driving speeds.

The fire department and the police arrived on the scene. A police report was filed. There were no injuries reported at the scene of the crash, however, the driver of the other vehicle was later treated for unknown injuries. The vehicle was towed to a local tow yard. The contact stated that both vehicles were condemned totaled lost. The manufacturer was made aware of the failure and a case was filed. The failure mileage was 1,000. The VIN was unavailable.

The contact stated that he was depressing the brake pedal but the vehicle was not stopping. The contact stated that he crashed into a vehicle in front of his vehicle on the other vehicle's passenger side front door. The air bag deployed. The contact stated he was not aware of any warning lights. The contact stated that the police and emergency service vehicles arrived. The contact was not injured and did not receive any medical attention. The police filed an incident report. The vehicle was towed from the scene to an impound lot and later was declared a total loss by the contact's insurance provider. The manufacturer was informed of the failure. The failure mileage was approximately 2,000.
