While driving approximately 45mph on a smooth dry road, the drivers side front lower ball joint broke in half. This caused the wheel to separate from the lower control arm and get lodged between the fender well and the front subframe. This caused the inner tie rod to bend and the cv joint to separate from the inner connection. The rubber brake line was torn in half and undercarriage damage due to it slamming to the pavement. I notified Tesla and scheduled an appointment. This happened on 02/26/2025 at approximately 7am. The vehicle did not drive abnormal or give any warning before the incident. All maintenance and services were up to date.

The car veered suddenly to the left (out of the left lane of a highway) while supervised in auto steer. The driver could not move the steering wheel to avoid a collision with the guardrail as the steering wheel had locked up. The collision warning didn’t sound and emergency brakes did not activate upon impact. The airbags were deployed and the car was totaled.

Car isn't cooling while supercharging. Problem originated while at Tesla Collision in Fort Lauderdale. Mike DiPietro, the general mgr stated they don't have a supercharger at Tesla Collision Fort Lauderdale to test it and they're not willing to drive it to the supercharger to test it because it's a service issue. When I dropped the car off it cooled while supercharging and now that it's not cooling while supercharging it presents a fire and safety hazard because it will only cool the moment the car is unplugged from the supercharger and then cools full blast for 45 minutes after the car is unplugged. I am well aware of the proper functioning of the cooling of 2015 Tesla Model S P90DL's because I have two of them and have since 2017. The fact that the cooling system kicks ONLY AFTER I UNPLUG from the supercharger clearly indicates the car sees a need to cool but because of a software issue. MIKE DIPIETRO, THE GENERAL MGR OF TESLA COLLISION FORT LAUDERDALE IS NOW THREATENING TO SUE ME BECAUSE I'M REPORTING A FIRE SAFETY/ACCELERATED BATTERY DEGRADATION ISSUE AND I SIMPLY WANT TO PICK THE CAR UP IN THE CONDITION WHICH i DELIVERED IT WHERE THE SUPERCHARGING WAS WORKING PROPERLY COOLING WHILE SUPERCHARGING. THREATENING LEGAL ACTION BECAUSE I'M REPORTING AN ISSUE THEY MAY NEED TO COORDINATE WITH TESLA SERVICE TO FIX AMOUNTS TO BULLYING. If I pick the car up it implies my consent the car is fixed and is NOT a fire safety hazard. They are unwilling to fix the supercharging fire the car is presenting because Tesla Service is a separate entity than Telsa Collision. This is NOT a valid excuse and reason not to fix this fire and safety issue. I was involved in a significant vehicle crash with my Tesla Model S, the vehicle wa a complete write-off and was towed away. I was taken via ambulance unconscious from my injuries. The vehicle was struck by a perpendicular vehicle who ran a stop sign and hit the front driver's side firewall/door area. The impact was hard enough to drive my car sideways against all wheels into the curb, over the curb, onto the sidewalk, and into a pole. My head hit the drivers window hard enough to break the glass and render me unconscious. No airbags deployed, curtain/seat/otherwise - no side impact airbags even. Seatbelt was worn, I was extracted by first responders - I later spoke with Tesla regarding my complaint of no air bag protection, and was told they would take this seriously and investigate. I later found out the car was auctioned off (presumably for scrap) with no evidence of Tesla even attempting an investigation and zero response indicating otherwise. After reaching out again to Tesla seeking a response, they assured me that they have GPS coordinates, Police Reports, asked for pictures (which I did send), and they promised to respond - that was more than 5 months ago and still no answers.

The Model S, which was bought in 2015, had a sudden acceleration while I was trying to park near the entrance gate of the parking lot. The accident happened on May 25, 2023. Fortunately, nobody got hurt and the car was totaled. I feel strongly that this should be investigated, as this can be a very serious public safety issue. It was inspected by the insurance company. There were no warning messages prior to the failure.

The contact owns a 2015 Tesla Model S. The contact stated that while he, his wife, and his son were in the vehicle he had begun veering right pulling into the driveway, at approximately 5 MPH, he had begun slightly releasing the brakes when the vehicle suddenly accelerated at accelerated high speeds. The contact immediately depressed the brake pedal however the vehicle speed continued to accelerate. As a response, the contact attempted to maneuver the vehicle to avoid crashing into the garage. The contact crashed into a nearby tree, jumped the curb, and became airborne crashing into the side of the home. The driver's side of the vehicle crashed into the side of the home where the laundry room was located ricocheting hitting the kitchen and bedroom windows and eventually coming to a stop. Due to the home being on a slope, the vehicle ended up falling into a nearby embankment after coming to a stop. All air bags deployed. The vehicle was towed. The contact was informed by insurance the vehicle would more than likely be totaled however this had not been confirmed. A police report was filed. The contact received emotional distress, cuts, and bruising to their hands. arms and forehead. The contact also suffered from back pain. The contact's wife sat in the rear on the driver's side and received emotional distress, trouble breathing, bruising on their face, and bleeding in the mouth. Their wife also received neck, back, and head pain. The contact stated that their wife ended up falling to the floor of the vehicle as a result of the crash. The contact's son sat in the rear on the passenger's side and received emotional distress, and back and neck pain. All persons received medical attention on-site, such as cleaning the wounds and icing any bruising. They were advised to seek additional care, however, have not been able to do so as of yet. The crash caused damage to the wall and flooring structures, electricity, and water systems in the home. The vehicle had not been taken to the dealer. The cause of the failure had not been determined. The manufacturer was not made aware of the failure. The approximate failure mileage was 60,000.

April 21, 2023 me and my daughter was driving from Jacksonville Beach to Fernandina Beach around 6pm. Exiting from 295 to i95, I removed my autopilot to reduce speed, I was very focus driving, not on my cellphone and even sleepy. Then I saw the car in front of me suddenly stopped, i know i have enough distance and confidently press the break hard enough to make it stop. But the tesla, while started beeping loud as it sensored the sudden stop of the car , seems to just slowed down and not stopping even with the full hard press of the break, the Tesla just keep on moving slow until it just hit the car and we just heard a loud bang from the airbags that blew up, gas fumes all over inside the car, shocked with what happened, I immediately opened the car door due to the gas fumes. It was traumatizing. I have been driving for almost 20 years, a lot of experience from high way to city driving, years of experience with crazy Manila traffic and highways, using different kind of cars, SUV, family van, even U-Haul, I am very familiar with how breaks work and how breaks could save life and save from accidents even with a split seconds, but the experience with Tesla’s break system is different. With this crash with Tesla, I remember this was not the first time the break didn’t work as expected. Had the same instance happened before with Tesla, though we didn’t hit any car but almost – me and my daughter together with her classmate was driving to school. Suddenly the car in front stopped due to the traffic light turned red. Again the car started beeping loud, I know we have enough distance and confidently press the break, but to my surprise, and the kids started getting scared when the Tesla seemingly not stopping and seemingly gonna hit the car as it continuous to drive and the break not helping to stop quickly even when the break was fully pressed.I believe this is a serious issue.My car now assessed Total Loss and just paid for market value, lost $20k in a year span

System Failure: Adaptive speed control Safety Risk: My 2015 Tesla Model S, at highway speeds, smashed into the rear of a jacked up Ford F250 Pickup. A Head on collision at highway speeds put my wife (and dog) and I at great risk. Has the problem been reproduced or confirmed by a dealer or independent service center? I am unsure if or how Tesla is dealing with the problem of adaptable speed control failure. Has the vehicle been inspected? Yes. Dan's Paint & Autobody and USAA Insurance Company adjuster My accident happened in the southbound passing lane at mile marker 178 on I-10, south of chandler on the Gila River Community in Maricopa/Pinal County It was dark outside with heavy traffic in both southbound lanes. My software version was 2021.40.6 I had the distance between cars at the closest setting , and had the adaptive speed control set at ~75. I was attentive with both hands on the wheel, eyes forward watching traffic with my wife whom was also watching the traffic. Without warning a Ford F250 stopped hard in front of me and my adaptive speed control did not react. The few beats I gave the adaptive speed control to respond first cost me precious braking time. I did hit my abs brakes and could feel the vibration as they attempted to slow down, but it was too late. I submarined under the bumper of the truck at a very high speed. There was no warning lamps, messages at the time of the accident. There was one early symptom that I recall. There was a Tesla software update a few days before the collision. On 11/27/22 before leaving for my trip, the dash screen to the far left did not display the map which it usually does when you set the GPS to map the path. I rebooted with the steering wheel toggles, and the display returned to normal. There were no other software malfunctions that I recall prior to the adaptive speed control failure.

No early alert before slamming on the brakes, only tapping a rear bumper on a 1999 Toyota Tundra. No damage to the Toyota but to my surprise, the insurance company said the Tesla's totaled. The airbags for the driver were deployed only, with no other passengers in the car. Quite a loud noise. I have an EDR report, not sure how to interpret it. Did not hear any warning lamps. Fortunately, both the other driver and I suffered no injuries. I would like to find out exactly what happened.

THE CENTER CONSOLE DISPLAY OF MY TESLA MODEL S HAS ABRUPTLY STOPPED WORKING. I CAN NO LONGER ACCESS THE MAJORITY OF THE FEATURES OF MY VEHICLE, SUCH AS TURNING ON A/C, ACCESSING GPS, CONTROLLING THE SUNROOF, ADJUSTING VEHICLE SETTINGS, ADJUSTING MY SIDE MIRRORS, DEFOGGING THE WINDSHIELD, ACCESSING THE REARVIEW CAMERA, AND ANY OTHER SETTING THAT IS CONTROLLED BY THE CENTER DISPLAY. THIS HAS MADE DRIVING THE CAR NEARLY IMPOSSIBLE. BECAUSE OF THE LOW VISIBILITY OF THE REARVIEW WINDOW, I HEAVILY RELY UPON THE REARVIEW CAMERA. SINCE CENTER CONSOLE DISPLAY WAS OFF AND HENCE NO REARVIEW CAMERA, I ENDED UP DAMAGING MY BUMPER INTO A LOW-PROFILE OBJECT THAT OTHERWISE WOULD HAVE BEEN VISIBLE HAD THE CAMERA BEEN ENABLED. I ONLY DROVE MY VEHICLE ONCE AND DAMAGED IT ON THE FIRST DRIVE WITH THE CENTER CONSOLE OFF. I CONTACTED TESLA TO HAVE THIS PROBLEM FIXED, BUT WAS TOLD I WOULD NEED TO PAY APPROXIMATELY $1,500.00 TO REPLACE THE MCU WITH THE EXACT SAME VERSION. GLUE COMES OUT EVERY MINUTE CREATING A DANGEROUS SITUATION BECAUSE IT GETS STICKY ON YOUR HANDS AND WHILE YOU TRY TO REMOVE IT THE SCREEN GOES BLACK OR FREEZES. THE MOST COMMON ONE IS THAT THE SCREEN OR MCU FREEZES UP CONSTANTLY AND THEN THE CAR CAN'T BE RUN. HARD RESET WORK SOMETIMES AND SOMETIMES WON'T . I CONTACT THEM AND THEY WANTED ME TO PAY $2600 FOR SOMETHING IT IS THEIR DESIGN FLAW . THIS IS A DANGER FOR THE PUBLIC SAFETY AND NEEDS TO BE ADDRESS BY TESLA REPLACING THE MCU AT NOT COST SINCE IT IS A DESIGN PROBLEM . SUDDEN UNINTENDED ACCELERATION. THE VEHICLE WAS STATIONARY AT A STOP LIGHT. AS THE LIGHT CHANGED, I BELIEVE THAT I TOOK MY FOOT OFF OF THE BRAKE AND THE CAR STARTED ACCELERATING. I TRIED TO PUT THE FOOT BACK ON THE BRAKE BUT I HIT THE CAR IN FRONT OF ME BEFORE THE CAR STOPPED. MY TERSLA EXPERIENCED A SUDDEN ACCELERATION EVENT ON JULY 31, 2019. IT WAS BEING PARKED IN A TARGET PARKING LOT ABOUT TO BE PUT IN PARK WHEN IT SUDDENLY EXPERIENCED A FULL ACCELERATION. IT JUMPED OVER A 6 TO 8 FOOT WIDE CONCRETE MERIDIAN, TOOK OUT A BUSH AND HIT A PARKED PICK-UP, MOVING IT ABOUT 15 FEET AND TOTALING THE TRUCK. THE CAR HAD ALMOST $40,000 IN DAMAGE AND THE DRIVER WAS INJURED. TELSA FIRST SAID THEY WOULD HAVE THEIR READ OUT IN ONE WEEK THEN 3, THEN 4 - FINALLY AFTER ABOUT 4 MONTHS THEY SAID IT WAS DRIVER ERROR BUT THE DRIVER DENIES TOUCHING THE GAS OR BRAKE. SHE WAS ALREADY STOPPED AND IT HAPPENED SO FAST SHE DIDN'T HAVE TIME TO REACT.

ON JANUARY 2, 2019 BETWEEN 1:45 AND 2 PM I DROVE FROM MY HOUSE INTO A CLINIC PARKING LOT. AS I DROVE MY 2015 TESLA MODEL S FORWARD AND STEERED TO THE RIGHT INTO A PARKING SPOT, I CONSCIOUSLY EASED MY RIGHT FOOT OFF FROM THE ACCELERATOR PEDAL, THUS LOWERING MY SPEED FROM 10-9 MPH TO ABOUT 5-6 MPH. THE BRAKE PEDAL WAS NOT PRESSED, ALLOWING REGENERATIVE BREAKING TO SLOW DOWN THE VEHICLE. ABOUT A SECOND INTO THE PARKING TURN, THE VEHICLE SUDDENLY JOLTED FORWARD, ACCELERATING UNEXPECTEDLY. I FELT AS IF THE BRAKES FAILED TO WORK WHEN I REFLEXIVELY REMOVED MY RIGHT FOOT FROM THE ACCELERATOR, AND STOMPED THE BRAKE PEDAL AS HARD AS I COULD. MY VEHICLE CAME TO A STOP ONLY AFTER COLLIDING WITH A CHEVROLET SPORTS UTILITY VEHICLE PARKED IN THE SPOT ACROSS. AS I REGAINED MY BEARINGS, I CAUGHT MYSELF STILL FLOORING THE BRAKE PEDAL WITH MY RIGHT FOOT, AND CONTINUED TO FLOOR IT UNTIL I AFTER I HAD SHIFTED IT INTO PARKING GEAR, AND MADE SURE THERE WAS NOBODY WALKING IN THE VICINITY. THE IMPACT FROM THE TESLA MODEL S BUMPED THE CHEVROLET SUV PARKED IN FRONT ABOUT 5 FEET BACK. THE MODEL S ITSELF STOPPED BETWEEN 2-3 FEET INTO THE NEXT PARKING SPACE, SLIGHTLY INTO THE OPPOSING PARKING SPACE ON THE RIGHT SIDE, AS SEEN IN THE INCLUDED PHOTOS. THIS WOULD INDICATE THE BRAKES **EVENTUALLY** WORKED. HAD THEY NOT, THE TESLA WOULD HAVE BEEN EXPECTED TO ROLL FORWARD AND INTO THE SUV'S BUMPER AFTER THE COLLISION. THIS IS CALLED TESLA CREEP MODE, WHICH I HAVE ENABLED (SLOWLY ROLL FORWARD WHILE NO PEDAL IS PRESSED). I ASKED THE POLICE OFFICER TO POSITION HIS PATROL CAR BEHIND MY VEHICLE BEFORE I MOVED IT AND ADVISED THE POLICE OFFICER I DID NOT TRUST THE CAR. I USED AN APP TO CALL FOR A TOW THROUGH MY INSURANCE, BUT THE TOW'S NUMBER WAS DISCONNECTED. I WAS ABLE TO DRIVE OFF WITH THE PATROL CAR IN ESCORT.

2015 MODEL S 85D WAS REVERSED ONTO DRIVEWAY THEN PLACED IN PARK AND DOORS WERE CLOSED AND LOCKED. A FEW MOMENTS LATER THE VEHICLE STARTED ACCELERATING FORWARD TOWARDS THE STREET AND CRASHED INTO A PARKED CAR. FRONT WHEELS WERE RECEIVING POWER WHILE REAR WHEELS WHERE LOCKED AND DRAGGING RATHER THAN WHEELS SPINNING. I REVERSED VEHICLE BACK ONTO DRIVEWAY AND IT HAPPENED ANOTHER 2 TIMES AFTER FIRST INCIDENT WITHIN A 30 MINTUE TIME SPAN.

HERE IS A MODEL X IN THE JUNKYARD. OCCUPANTS HAVE CERTAINLY BEEN INJURED. THE LEFT HAND SIDE OF THE CAR IS ALMOST COMPLETELY UNDAMAGED BUT THE LEFT HAND REAR SUSPENSION HAS SNAPPED OFF. THAT WHEEL IS LYING ON THE GROUND NEXT TO THE WRECK. IT DOES NOT HAVE SO MUCH AS A BROKEN SPOKE OR DAMAGED TIRE. IT IS HIGHLY LIKELY THAT THE CAUSE OF THE ROLLOVER WAS THE LEFT HAND SUSPENSION FAILURE. IN ANY OTHER MAKE OF CAR EVEN IF A WHEEL RECEIVED A HARD IMPACT IT WOULD BE THE WHEEL ITSELF THAT WOULD BREAK AND NOT THE SUSPENSION ITSELF. THIS IS NOT A ONE OFF FREAK ACCIDENT. THERE HAVE BEEN HUNDREDS OF SIMILAR CASES OF SUSPENSION BREAKAGE ON TESLAS. IF TESLA NEEDS TO USE ALLOY SUSPENSION PARTS FOR WEIGHT REDUCTION IN ORDER TO ACHIEVE LUDICROUS ACCELERATION THEN AT LEAST IT SHOULD BE USING BETTER QUALITY CASTINGS AND REGULAR INSPECTION AND CRACK TESTING. SAME THING APPLIES TO TESLA EMPLOYEES AND OFFICERS. THERE IS A CHILD SEAT IN THE BACK OF THIS TESLA. HOW MANY DEAD CHILDREN DOES IT TAKE FOR YOU TO LAUNCH AN INVESTIGATION? PLEASE DO YOUR JOB AND DEMAND A RECALL OF THESE CARS WHICH ARE SIMPLY UNSAFE AT ANY SPEED. THANK YOU. OUTRAGED CITIZEN.
