The contact owns a 2015 Lexus NX 200t. The contact stated that while driving at various speeds, an abnormal, ticking sound was emitted from the fuel tank. As the failure persisted, the vehicle entered "Reduced Engine Power Mode" and the check engine warning light illuminated. The vehicle was taken to an independent mechanic shop where a diagnostic check was performed, and it was discovered that the fuel pump was defective and needed to be replaced. A local dealer was contacted and an appointment was scheduled to have the vehicle serviced. The contact arrived for the appointment but was informed that the fuel pump was on backorder and the appointment needed to be rescheduled. The contact informed the dealer that driving the vehicle felt unsafe because the commute exceeded one hour. The dealer assured the contact that the vehicle was safe; however, while driving the vehicle back to the residence, the same failure returned. After restarting the vehicle, the contact drove the vehicle to the residence. A second appointment was scheduled with the dealer, and a service advisor arrived at the residence of the contact in a rental vehicle and drove the vehicle back to the dealer for service. The vehicle was returned to the contact; however, the failure persisted. The dealer was notified and informed the contact that the failure resulted from a loose panel that technicians had difficulty bolting down. The vehicle was towed back to the dealer for diagnostic testing and repair. The contact test drove the vehicle with a mechanic inside the vehicle and informed the mechanic that the same failure remained noticeable. The failure returned a few days after the repair. The contact stated that both the dealer and the manufacturer avoided phone calls to service the vehicle. The vehicle was not yet repaired. The failure mileage was approximately 130,000.

NHTSA has issued service bulletins regarding Lexus servo motor issues, primarily affecting the HVAC system, because these motors can have internal electrical malfunctions. This malfunction can cause symptoms like uneven or inconsistent air temperature from the vents, lack of airflow from selected vents, improper mode control, and the storage of Diagnostic Trouble Codes (DTCs) related to the damper control servos. This is an issue because it affects Safety and Convenience due to the following: While these issues primarily affect comfort and convenience by disrupting the climate control system, a malfunction of other servo motors (which control things like air flow direction or even steering in other situations) could create a safety risk. Experiencing this in my 2015 NX 200t today made me feel unsafe, as the vents were blowing out extremely hot air. And during hot Texas weather that is unsafe, and I have children that I have in the car with me at times. The Lexus dealership in Grapevine Texas is asking for $2,600+ to fix servo motor 1 and 2 after they ran diagnostics. It seems as if a vehicle that was in the same year and is experiencing the same exact issue as other vehicles in that year should also be covered as those that got it fixed under Lexus’ warranty. What makes the NX model different from the 6+ other models that Lexus a covered under the NHTSA safety bulletin/ recall?

Hello, I own a 2015 Lexus NX200T. On 9/13/2025 I was pulling into a spot to park, going slow at maybe 4-5 mph, my vehicle suddenly violently accelerates on its own. I had to slam multiple times on the brakes to avoid killing two pedestrians right in front of my vehicle. I was in seconds of killing them and pinning them between the vehicle that was in front of me. I did everything in my power to keep hitting the breaks as hard as I could until it stopped within inches of the two pedestrians. This was extremely dangerous and could have killed a mother and daughter in that split second. This appears to be a case of unintended acceleration, which I understand is a serious safety issue. I've been told by both the dealership and Lexus that because no damage occurred, it's not their problem. But this is clearly a safety defect, not just a maintenance issue. I would like Lexus to take this very seriously. I was shaken and very scared. I have NEVER had this happen before. Lexus suggested that the floor mats may have got caught - NO absolutely not. I have the mats locked in place with clips. I did not hit the accelerator instead of the breaks as that is what is assumed. No external damage occurred, but the unintended acceleration was violent. This incident took control away from me. Even though no accident occurred this time, the potential for injury or death is very high. I contacted Lexus and the dealership, but both declined to inspect the vehicle, stating it was not their issue because no damage occurred. I believe this a very serious safety defect that should be investigated. I am requesting NHTSA to investigate this potential defect in the Lexus NX200T model. If other owners experience the same, this could indicate a widespread safety problem. I was very lucky I did not kill anyone; it would have changed the day's events forever for the lives of two innocent people and myself. I don't feel safe to drive the vehicle after this incident in fear that it could happen again.

On June 3, 2024, a CARFAX Vehicle History Report for the 2015 Lexus NX 200T (VIN: [XXX] ) reflected an odometer reading of 37,783 miles as of August 14, 2021, and 26,148 miles as of May 9, 2023, when the vehicle was offered for sale . A subsequent CARFAX report dated August 19, 2025, altered the August 14, 2021 entry to show only 21,864 miles, while still listing 26,148 miles on May 9, 2023 . This change in recorded mileage demonstrates that the odometer history was manipulated to conceal a rollback and to make the May 2023 mileage appear consistent at the time of sale by Countyline Auto Center, Inc. The discrepancy of more than 15,000 miles in the same vehicle history constitutes odometer tampering and misrepresentation, which induced [XXX] to purchase the vehicle at an inflated value. The contact owns a 2015 Lexus NX. The contact stated that while driving approximately 65 MPH the vehicle suddenly experienced a loss of automotive power and stalled. The contact indicated that the failure had occurred while driving on a very busy freeway. During the failure, the warning messages "engine power lost" and "traction control off" were displayed along with the check engine light illuminating. The contact was able to restart the vehicle and drive to the local dealer who diagnosed that the low-pressure fuel pump was faulty and needed to be replaced. The vehicle was not yet repaired. The contact was informed that the low-pressure fuel pump was previously replaced under the NHTSA Campaign Number: 20V012000(FUEL SYSTEM, GASOLINE). The contact indicated that the vehicle had experienced that same failure listed in the recall. The failure mileage was 208,000.

The vehicle has 119285 miles on it. I have started to see oil leak from the transfer-case/transmission. I called lexus back in August 2023, they were aware of certain vehicle developing oil leak problems. They issued recall on certain vehicles and issued a TSB for oil leak repair. I called lexus today and took the vehicle to the dealership. There is an understanding that my vehicle won't be covered under recall even though the problem is known to lexus. This issue may lead to unexpected breakdown of the vehicle and lead to loose of control of the vehicle.

Left work for work in the morning and as I turned off my street to get onto a main street, I drive a block and the Check Engine Light appears and message appears on my console State "AWD System Malfuction." See Dealership Service. Drove to the dealership to drop off my care, and after 3 days of inspecting my car, the service specialist messaged me with a service summary of a list of items that need to be repaired to have my car run safely again. The total service totaling over 10k, which included repair head cylinder, valve guides, gasket, valve exhaust, cylinder head gasket cover. After speaking to the Service Specialist, he told me the issue is what they refer to "engine wrap" where the cover to the engine distorts (due to heat and time) and messes with the engine itself. He informed me this is a known issue that can occur with the 2015 NX200t.

Since I am just finding out how to file a complaint I figured I would. I still have this car with me. The incident happened about 2 years ago. I was driving down the highway going to Castle Rock Colorado. When all of a sudden my car is thrown into neutral and my rpms go to 6,000. I had no way to accelerate and the car wasn't in gear to help decelerate . Since I was going down the highway at a high rate of speed trucks and cars were all around me.. I immediately turned on my four ways and pulled the car over to the side of the highway. As there was nothing wrong with the motor and it seemed like it was just with the transmission I. Decided to continue but as I continued when I got to 4 gear it would once again throw it out of gear and I couldn't accelerate and I had to pull over. At this point I knew something was wrong. I immediately got the car to a safe area and got it home and the next day got the car to the mechanic. The mechanics said that I needed a new transmission 4 gear is just missing. When the mechanic was putting back together. The very expensive $7,345.00 transmission. He got the parts directly from Lexus and when he went to test, drive the car with the same but new components the transmission immediately failed again, not allowing him to shift in the 4 gear and stalling the car putting him in danger on a busy road. When he got back to his shop he called back to Lexus to verify that the parts that he was given were correct. They verified that the new parts he was given are the old design and that he needed upgraded newer parts that lexus has designed and should have never gotten the other ones . At this point he got the proper parts again and for a second time put the upgrade parts in and it worked . By Lexus not giving the mechanic, the proper upgraded parts. Not only did they put me in danger with the part breaking the first time on the highway they put the mechanic in danger by not giving him the upgraded parts the first time ... thank you

I purchased this vehicle in 2015 - it has been dealer serviced and has never been in an accident. At just under 90,000 miles on the odometer, a major fluid leak started. It turns out that the rear main (crankshaft) seal had failed, causing a leak of transmission oil. No warning lamps or messages appeared. This failure resulted in an extraordinarily expensive repair, over $2,500.00 at the dealer, Lexus of Greenwood Village (Colorado). It was not covered on warranty. This is a major safety issue - had I not noticed the leak, which occurred without warning, the engine could have seized on a highway, leading to a crash. This failed seal should be designed to last far longer than 90,000 miles. Premature failure of this kind is highly likely secondary to a defect in design or materials. I have complained to Lexus, which is unwilling to take any responsibility for this. Please investigate as to whether a recall is warranted, and whether the manufacturer should be responsible for the repair of this obvious defect. Thanks.

12-VOLT BATTERY IN TRUNK AREA SHORTED/FAILED. THIS RESULTED IN THE FOLLOWING DANGEROUS CONDITIONS: 1. INABILITY TO OPEN THE TRUNK FROM INSIDE OR OUTSIDE THE VEHICLE (THERE IS NO EMERGENCY TRUNK RELEASE IN THIS VEHICLE, POWER ASSISTED TRUNK RELEASE DISABLES MECHANICAL DOOR OPENING AFTER BATTERY FAILURE). 2. ENGAGED CHILD LOCKS DID NOT RELEASE WITH BATTERY FAILURE, SO PASSENGER DOORS WERE ALSO NONFUNCTIONAL FOR EXITING THE VEHICLE. 3. HORN WAS DISABLED, WITH NO WAY TO SIGNAL FOR HELP. THESE SAFETY DEFECTS WERE FORTUNATELY DISCOVERED IN MY GARAGE AS I WAS ATTEMPTING TO CONNECT A CHARGER TO THE 12-VOLT BATTERY LOCATED IN THE TRUNK. WHILE CLIMBING INTO THE BACK SEAT, I INADVERTENTLY SLID THE CHILD LOCK LEVER ON THE SIDE OF THE DOOR PANEL INTO THE LOCKED POSITION. I REMOVED THE HEADRESTS TO GAIN ACCESS TO THE TRUNK BECAUSE THE TRUNK DOOR COULD NOT BE OPENED FROM THE EXTERIOR OF THE VEHICLE. I THEN NOTED THAT THERE IS NOT EMERGENCY TRUNK RELEASE LEVER IN THE TRUNK AREA. ONCE THE PASSENGER DOOR CLOSED, THE ONLY EXIT FROM CAR WAS THE FRONT DOORS. WORSE STILL, THE CAR HORN IS DISABLED WITH BATTERY FAILURE, ELIMINATING THE POSSIBILITY OF SIGNALING FOR HELP. SHORTING OF THE 12-VOLT BATTERY IN THE LEXUS NX 300H, TURNS THIS VEHICLE INTO A SAFETY HAZARD.

THE UV PROTECTION ON THE NAVIGATION SCREEN IS COMING OFF LOOKS LIKE IT HAS BEEN SCRATCHED WHEN I ASKED LEXUS ABOUT THIS I WAS TOLD MY WARRANTY WAS UP AND THAT IT WOULD COST ME ABOUT 950.00 TO REPLACE IT. I SEARCHED THE PROBLEM ON LINE AND MANY PEOPLE HAVE COMPLAINED ABOUT THIS LEXUS IS A WHERE OF THE PROBLEM AND WON'T DO ANYTHING ABOUT IT WHEN IT HAPPENS. I WAS TOLD IF IT WOULD HAVE HAPPENED BEFORE MY WARRANTY RAN OUT IT WOULD BE COVERED. IT IS CLEARLY A DEFECT OF THE UV PROTECTION ON THE SCREEN .

WAS ENTERING HIGHWAY AND REACHED SPEEDS OF ABOUT 70MPH WHEN ALL OF A SUDDEN THE SUNROOF EXPLODED AND SHATTERED. I IMMEDIATELY CONTACTED LEXUS AS I KNEW NOTHING COULD POSSIBLY HIT MY CAR AT THOSE SPEEDS AND NOT COME FLYING THROUGH THE ROOF AND DAMAGE THE FRAME AND OTHER MOVING PARTS. THE ONLY THING THAT SAVED ME WAS THAT I HAD THE INTERIOR SLIDE HATCH HALFWAY CLOSED SO GLASS DIDN'T ENTER THE VEHICLE. I IMMEDIATELY SHUT IT AND DROVE TO THE DEALERSHIP. THE AGENT WHO LOOKED AT MY CAR REPORTED THAT THIS TYPE OF INCIDENT HAS HAPPENED IN THE PAST AND IT LOOKED REMOTELY SIMILAR. WE CHECKED FOR A ROCK LIKE STRUCTURE BY SHINING A LIGHT IN THE ROOF AREA AND NOTHING WAS FOUND. I CONTACTED LEXUS EXECUTIVE OFFICE AND WHILE THEY WERE EXTREMELY HELPFUL, THE THEORY THAT SOMETHING HIT MY CAR WAS NOT SUPPORTED BY THE LAW OF PHYSICS OR MOTION. IF SOMETHING HIT THE TOP OF THE VEHICLE WHILE I WAS DRIVING DOWN THE HIGHWAY AT 70MPH THE SPEED OF IMPACT FROM THE OBJECT WOULD BE DOUBLED EVEN TRIPLED AND DEFINITELY INSIDE MY CAR OR ON TOP OF MY HEAD. THE TRAJECTORY PATH FROM THIS OBJECT HAD TO BE SPOT ON AND COME FROM EXTREME HEIGHTS AND DISTANCE TO LAND ON TOP OF THE CAR. THERE WOULD HAVE BEEN MUCH MORE VEHICULAR DAMAGE AND I COULD HAVE BEEN SERIOUSLY HURT IF THAT WERE THE CASE AS THE HOLE IS MORE ON THE PASSENGER SIDE. ALL IN ALL NO ONE WAS HURT AND THANKFULLY MY SUNROOF WILL BE REPLACED. I FEEL AS THIS HAS HAPPENED ON MORE THAN SEVERAL OCCASIONS AS TO WHY I WAS TAKEN CARE OF SO QUICKLY. I JUST LEARNED THAT THERE IS A CLASS ACTION LAWSUIT IN ANOTHER STATE ABOUT THE RX SUNROOF EXPLODING.

ON 7/29/2018 - WHILE DRIVING THE VEHICLE IN ROUTINE STREET (NON-HIGHWAY) TRAFFIC I FELT A SLIGHT DRAG AND THEN THE YELLOW WARNING LIGHT APPEARED INDICATING 'AWD SYSTEM MALFUNCTION 2WD ENGAGED.' LESS THAN A WEEK EARLIER I HAD THIS PROBLEM ADDRESSED AT A LOCAL AUTO SHOP AND THEIR DIAGNOSTIC INDICATED THIS WAS A TURBO GATE PROBLEM AND THE GATE WAS NOT CLOSING ELECTRONICALLY. I WAS ADVISED TO TAKE THE VEHICLE TO THE DEALER IF THE PROBLEM RETURNS. I HAVE CHECKED ONLINE AND SEE THIS APPEARS TO BE A PROBLEM FOR OTHER NX300T OWNERS AND CONSIDER THIS MALFUNCTION CAN BE CATASTROPHIC SHOULD THIS MALFUNCTION OCCUR WHILE DRIVING DURING BAD WEATHER CONDITIONS. IN ADDITION TO THIS PROBLEM I WAS HIGHWAY DRIVING APPROXIMATELY DECEMBER 2018 WHEN I EXPERIENCED THE VEHICLE SURGINDAS THOUGH HYDROPLANING BY THERE WAS NO BAD WEATHER AT THE TIME. I HAVE NOT HAD A REPEAT OF THIS OCCURRENCE.
