What component or system failed or malfunctioned, and is it available for inspection upon request? I purchased four tires from Belle Tires on April 1, 2026, 14 days ago (note: mileage at accident 10,776 vs mileage at tire purchase 10,360 or 416 miles of wear). On Wednesday, April 15th, I was about to change lanes after entering the Tri-State Expressway (route 294) going southbound from Half-Day Rd. (route 22), I had crossed the first two lanes without incident, and moving into the third lane, when I experienced a blowout of my driver’s-side rear tire. The car started a fishtail counter-clockwise spin and crossed the third and the fourth lanes (i.e., the far left lanes) before smashing into the concrete median, separating my Southbound lanes from the opposing traffic lanes. I have kept both the tire and rim in my garage. How was your safety or the safety of others put at risk? As I started sliding or spinning, I cannot honestly say, whether or not I hit the brakes or can really describe the spinning that occurred. As the rim of blowout tire was evenly torn up on its outer edge, I know that the tire was turning. All I can recall is hearing the car hit the concrete median twice. I know that it had to be counterclockwise since I started going southbound, and after the car came to a rest next to the median, I was facing southbound again. Has the problem been reproduced or confirmed by a dealer or independent service center? I took the tire on it’s rim to the Belle dealer who would not acknowledge a blowout, but he kept insisting that he is not an expert on crash interpretations. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Allstate Insurance, Toyota used a Bosch representative to retrieve the Electronic Data Recording (EDR) information. I have copy of this report available for expert interpretation. No warnings were given prior to this blowout.

Last year my vehicle rear camera and sensor failed to which caused me to backup into a mailbox. I had brought it up after a car wash at the dealership and they said they didn't know about any recalls at the time. I recently saw the recall for this year and make and would like this to be investigated further. I also have an estimate for the damages that I can provide.

After applying the brakes in a sudden situation, the automatic emergency braking came on but the car didn't stop. The brake pedal was pushed all the way to the floor board but the car didn't stop and crashed into the vehicle in front. This happened once before but didn't end in a crash. This was a total brake failure.
