The vehicle started to hydroplane in rain on the highway and the lane keeping assist aggressively tried to push me back, which ended up causing us to hydroplane more and causing the vehicle to flip around backwards and we ended up sliding completely off the highway into a ditch totaling the vehicle.

The steering wheel on our Mazda CX-90 has been getting stuck when turning the vehicle. This failure occurs when there is a slight turn in the wheel (approximately 25-45 degrees). The steering wheel gets stuck and the driver has to use force to "break" it loose. This action causes the vehicle to overcorrect into either the lane next to vehicle or into oncoming traffic. I have done research and Mazda has acknowledged that this is a known issue and even issued a recall for our 2024 Mazda CX-90. However, Mazda has also more recently come forward and said that their corrective action does not always correct the recalled issue. We recently had a near miss into oncoming traffic while trying to "unstick" our steering wheel. We have called and email Mazda and they have stated there is nothing new they can do to correct this issue.

On April 29, 2026, my 2024 Mazda CX-90 Turbo S Premium Plus experienced a steering malfunction while I was entering a right turn. The vehicle had Mazda steering recall 6524A / NHTSA Campaign 24V-022 marked closed/completed before this crash. I was traveling on a familiar road and attempted a normal right turn that I make regularly. As I attempted to turn, with no warning lamps, warning messages, or prior warning immediately before the incident, the steering wheel suddenly required abnormal effort, felt stuck, and would not turn normally. I was in complete shock from the sudden steering issue and abnormal resistance. The vehicle then struck the curb, left the roadway, and traveled down an embankment/grade of approximately 15 feet into a residential yard. The crash involved significant driver-side/front-left damage. The front driver-side wheel/tire separated and was found near the vehicle. The driver-side airbags deployed. Scene evidence included curb/edge impact damage, debris, and a deep ground-level rut/indentation below the roadway. This incident placed me and others at serious risk because the vehicle left the roadway and entered a residential property. The vehicle and failed components are available for inspection upon request. To my knowledge, the only prior inspection or repair related to the steering system was performed by the Mazda dealership in December 2025, when recall 6524A was marked closed/completed along with other recalls. The vehicle has not yet been independently inspected for this crash-related steering concern. Before the crash, the vehicle had multiple issues and spent significant time at the dealership for recall and non-recall repairs. One ongoing issue was a brief grinding/rubbing noise when accelerating from low speeds, which existed before the dealer work and continued afterward. Mazda/dealer indicated this low-speed grinding/rubbing noise was a known reported issue with no fix at that time.

I bought a fairly used 2024 Mazda on December 19, 2025 from a Mazda dealership and immediately noticed that there was some resistance with turning the steering wheel. I mentioned it to the sales person and was told that it was how the car operated. After purchasing the car, the problem kept persisting in many ways while on the freeway or when making turns. The steering will try to self correct with sudden jerks that manifest a violent pull against my efforts to keep the car on the road or the correct lane. I immediately took the car to a Mazda dealership by my house and they couldn't identify an issue with the lane changing sensor. They did however recommend I get new tires but I decided to get a second mechanical opinion by taking it to the Mazda dealership where I purchased the car. They also could not identify any issues or an ongoing steering recall, but recommended getting a new set of tires for a car that I had just purchased from them a couple of weeks earlier. Following my frustration, I decided to do some research on the Mazda CX-90 and that's when I came across the steering wheel recall 24V022. I told them about the recall and they could not find it under the VIN #. I called Mazda (The Company) and they could not also locate that recall using my VIN #. I pressed them to look closely and Mazda (The Company) finally established that my car was part of the recall under the batch number 6524A. I went back to the dealership where I had bought the car and provided them with the batch number and they were able to establish the recall for my car after haven refuted my claim. I mention this because it is VERY likely that other Mazda CX90s with similar issues may not have this recall show up under the car VIN #s, unless owners take the initiative to contact Mazda (The Company) directly. The dealership also told me that my recall was closed but they could not provide any proof of what remedial work that was done under the recall.

I have a 2024 MAZD CX-90 Preferred Plus AWD. I was driving my [XXX] grandson to preschool and it sounded as though the passenger's door opened while I was driving. In the split second I looked back to check, unfortunately, I hit a guard rail. My grandson was buckled in his car seat. his first words were "Meme, the door opened but I didn't do it". He was buckled in his car seat and I believe the safety door locks were on. There was no warning sound or light indicating the door was open while I drove. I pulled over, got out of the car and looked. the passenger's back door handle was positioned up but the door was closed. being so shook up, I didn't think to take any pictures. I then had messages on my car saying Dynamic Stability control malfunction - have the vehicle inspected and Engine Malfunction - have the vehicle inspected. I called my dealership to find out if there were any recalls on this type of problem (they said that there weren't any). I have come across a forum where a person states: "So can confirm 1st row passenger door does not provide any warning as well. Looking at the manual, I see that there is supposed to be a warning sound when any door/hood/liftgate is open while driving, as quoted below. If someone else confirms this is normal behaviour, then I think we should file a safety defect complaint with the NHTSA and Transport Canada. “Door-ajar Warning Light/Liftgate-open Warning Light/Hood-open Warning Light The light turns on if any door/liftgate/hood is not closed securely. In addition, the warning sound is activated if any door/trunk lid/liftgate is not closed and the vehicle is driven. Close the door/liftgate/hood securely.” This was a very scary situation. I absolutely will concede that something did happen with this rear passenger door. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

On [XXX], I was heading home from Brooklyn. It was a rainy day. The vehicle was functioning fine when all of a sudden the sign on the dashboard said "Iactive sense deactivated" which is something that always happens when it's raining. It had happened the Thursday prior to the accident when heading to the city and many other times. I had just crossed the George Washington Bridge, on [XXX] , going the speed limit or less, when the the iactive sense sign came up. About 1000 feet after I saw the sign and repeating it to my daughter the vehicle tried to get into the right lane, I was in the left lane of three lanes. I knew it was the iactive sense because i had felt it before many times. I braked and controlled the vehicle back into the left lane. said, "I can see the lane" The vehicle then sped on, moved right, then sped all the way right into the guardrail, all the while I tried to brake and turn left to avoid hitting the guardrail but the wheel was locked and the brake did not work. My right arm got hit by the air bag deploying. After hitting the guardrail we span all the way back into the left lane with the vehicle facing the opposite direction facing on coming traffic. I then hit the hazard lights a few times out of panic, then I had my daughter crawl to me (she was sitting behind me) in the front so we can get out because I thought there was smoke. Turns out it was the airbags' powder, according to the fire department. Another lady who was from the NYPD stopped to see what happened and I explained to her what happened and then the NJ police showed up and she left. When the police got there, I told him what happened. He repeated, "you lost control" at which I said "no the vehicle took control" From my observation the vehicle was like was angry and confused about where to stay and hitting something was the only way it would stop. I called Mazda, they said let the police and insurance take care of it. Geico said they don't read vehicle INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

I was parking the car at a low speed (2mph) when it suddenly accelerated with a great speed, jumped the curb in front and moved forward and up a small hill. The car collided with a tree before I was able to stop it. All ADAS features were activated in the car and no alarms sounded prior or during the event, which lasted between 1-2 seconds. The vehicle was inspected by Mazda and deemed safe to continue to operate. The dealer did not find a cause for the malfunction and suggested external influence. The inspection took about 3hrs, which included addressing multiple recalls on the vehicle. At time of incident I have owned the car for about two months and did not experience similar issues, however others did according to my web search. I found reports on popular forums describing sudden acceleration of Mazda CX-90 vehicles.

I was driving in EV mode and was behind another vehicle waiting to turn out of a plaza. The exit is on a slope which is important to mention. The car in front of me began to turn and as I was releasing the brake to move up the hill, the car engine kicked on, the RPM’s went up extremely high as if the accelerator was being pressed all the way to the floor, the car jolted forward on its own and hit the car in front of me. I kept trying to brake, but the car continued to accelerate on its own into the car in front of me a second time. I understand that when on a slope driving in EV mode, the cars engine will kick in to allow the car enough momentum to make it up the hill. However, in this case, the engine kicked in, but the momentum was extreme to the point that when I released the brake to roll forward the car shot forward so fast there was no avoiding a collision. This is extremely faulty and unsafe! My child and I suffered injuries as a result of striking the other car twice. We’re lucky we were not hit by multiple other cars as we were propelled into the intersection. I’m terrified to drive this car with my children. I contacted the dealership that instructed me to have the collision repairs completed first and then they would check out the car out.

Driving up snowy pass on I-90 approaching the top of Snoqualmie Pass. Vehicle stalled and warning lights flashed critical warning and loss of power. The vehicle stopped in the middle of the highway and would not start or go into drive. After a few minutes, because of speeding and adverse conditions, another driver collided with my vehicle from behind. Another following vehicle lost control and ran off the road. Nobody was injured. Mazda has not provided any information to what happened with the vehicle. We added about 12k miles in the 5 months of driving it, so we had lots of testing with it, and never ran into this serious of an issue with this or any other vehicle.

On 12/09/23, my 2024 Mazda CX-90 PHEV, purchased on 11/11/23, just got out of the driveway to make a left turn around 500 feet from the driveway when there was resistance in turning the steering wheel left, causing vehicle to swerve to the right and hitting the fire hydrant on the side of the road located just around the corner. Both airbags deployed causing extensive damage to the passenger front side of the car. The automatic emergency braking did not engage to avoid collision with the fire hydrant. One of the passengers suffered avascular necrosis of the hip as a result of the accident. The vehicle was towed to a collision center, then towed to the Mazda dealership, but according to the dealership, no inspection report was available in their system as of 03/19/24. The insurance company declared the vehicle a total loss. There were no warning lamps, messages, or other symptoms of the problem prior to the failure.

In October 2023 I began to notice that the steering was not working properly. When driving I would notice that the steering would stick and require more force to turn. It would click while turning. I brought it to the tully mazda (nashua NH) who told me that others were experiencing the same but that there was no fix for this yet. On 12/27/23 I again notified tully of this issue and there was no fix for it. On 2/9/2024 I discussed again with tully who advised me that there was a recall to be announced shortly. They did not tell me that there was a safety issue driving the vehicle. On [XXX] while making a left turn (there are two side-by-side turn lanes and I was in the inside/left) i completed my turn and while straightening out the wheel the vehicle overcorrected hitting the vehicle to my immediate right.
