I was driving my Tesla in self drive mode, and ibecause it started raining. I reduce the speed to standard. I had a notification on the screen, an alert in red. That the tesla reached the maximum speed in that mode “standard”. I was surprised and decided ti canceled the FSD feature and took control of the vehicle. Then when I was driving in the expressway very close to my exit on the right lane, the car accelerated by itself and turn to the left unexpectedly. I tried to returned the car to my lane and I never had control again of the car. Not the steering wheel, not the breaks responded. I was spinning on the expressway until I hit the concrete barrier with the rear bumper. Hopefully I did not hit any car nor they didn’t hit me. The car shutdown after the impact. Tesla sent me the footage of the impact, ignoring the moment that caused that incident that was some seconds before. The brakes were automatically turned onz and the wheels did not move. I know something wrong happened. Specially because the incident some minutes before when I was driving in standard mode and accelerated with no reasons. I have safety concerns. Also I noticed that after the last upgrades the car was havung navigation issues that I reported immediately when the autopilot was removed and we have to select the options.

On Saturday 16th Oct 2026 around 10 AM EST, while driving down the ramp from Parking Floor 2 to Parking Floor 1 in the JFK Terminal 4 Short Term Parking garage (John F. Kennedy International Airport, New York), the Tesla vehicle suddenly accelerated forward very fast on its own without any input on the accelerator pedal. Either I was not pressing the gas pedal at the time, or I was slowing pressing as the car was going down on ramp. The car surged ahead with significant force and struck a fence at the bottom/end of the ramp area. There were two passengers in the vehicle. No other vehicles were involved. The impact caused: My Tesla Model Y to sustain severe front-end damage and appear to be a total loss. Injuries to both passengers. Minor scratches on my hands. Sequence of Events: I was driving the vehicle at low speed down the ramp from Parking Level 2 to Level 1 in JFK Terminal 4 Short Term Parking. Without pressing the accelerator, the car unexpectedly accelerated. Braking did not prevent the impact. The vehicle came to a stop only after hitting the fence. Today, I have reported this as a sudden/unintended acceleration event to Tesla and requested a full vehicle data log review. I would like Tesla to investigate following. 1. Why the car suddenly accelerated on very speed? 2. Why the steering got locked when it happened? 3. Since it is the closed parking lot, why Tesla did not stop automatically before hitting fence? I have been driving for more than 27 years, I never had any accident, and I never got any tickets in last 27 years.

Client was severely injured (traumatic brain injury, fractured skull bone and ankle with ongoing cognitve and physical symptoms, probably permanent) as a pedestrian when crossing Rt 28 in Chatham, MA. He was struck by a 2023 Tesla Model Y in reduced roadway visibility conditions due to sun/shade conditions. The auto braking system failed to activate in time to avoid the collision. This appears to be directly related to NHTSA investigation PE24031.

Pulled into hospital parking garage. Stopped to pull parking ticket. When the parking gate was raised, Tesla immediately took off at very high uncontrolled speed. Basically a runaway vehicle. There was no accerlation on my part. Brakes did not work. Hit parked car in the garage about 300 feet from the parking gate. I was taken to hospital with broken wrist. Car has been totaled.

It was raining on I-287 North bound in Bridgewater Township, NJ. I was driving my Tesla Model Y well below the posted speed as it was raining. Suddenly my car took a spin, i lost control of the steering. Car took a 360 degree turn from right most lane on 3-lane highway and ended up on the side of the left most lane, rear ended into the bushes, hit the trees and damaged the back of the car. My car is all wheel drive, still i lost the control. I was so lucky to be alive.

We were about the park in the marked parking lane and the vehicle unintedly accellerated, the brakes did not work and we saw message "Automatic emergency brake not available" and Air bags not available. We tried to control the vehicle to avaoid crashing into the porch of the infront apartment and steared the speeding vehicle and the vehicle hit the pillar of the aparement and finally came to halt few meters away in the parking lot as the front right wheel broke. 1. The ADAS Automatic emergency breaking seems failed 2. Forward collission warning seems not working 3. The safety of the driver and the passenger were at risk while the collission happened 4. I am not sure if the similar incidents happened or can be reproduced by the dealer/manufacturer. 5. Vehicle components inspected by the Tesla collission center but no software logs inspected and they said that they cannot get the device crash/event logs. 6. At the incident/while the incident was underway we saw few message that displayed "Automatic emergency brake is not avaiilable" and "Air bags are not available"

The contact owned a 2023 Tesla Model Y. The contact stated that while driving at 30-40 MPH, the vehicle unintendedly accelerated causing the contact to lose control of the vehicle and crash into the rear of a parked vehicle. The vehicle continued to accelerate and traveled across the street crashing into another parked vehicle. The vehicle continued to accelerate on its own forward and over a sidewalk and crashed into two street signs when it came to a stop. No warning lights were illuminated. No injuries were sustained. No air nags deployed. A police report was filed. The vehicle was towed to an autobody center. The insurance company deemed the vehicle totaled. The manufacturer was made aware of the failure, and a case was opened. The failure mileage was approximately 12,000.

WE WERE TRYING TO PARK THE CAR IN OUR GARAGE WHEN THE CAR SUDDENLY SPED UP. AND HIT OUR THE FENCE . i RELEASED THE PEDAL BUT IT KEPT DRIVING UNTIL I HIT THE BRAKE. AIR BAG DID NOT DEPLOY. THE CAR?S FRONT WAS SMASHED. OUR FENCE IS ALSO BADLY DAMAGED. I HAVE ASKED TESLA TO INVESTIGATE IF IT WAS BECAUSE OF THE AUTOPILOT TAKING OVER BUT HAVE NOT RECEIVED A RESPONSE FROM THEM YET AFTER MORE THAN A WEEK.

I am reaching out on behalf of my wife who met with accident on 3/24/2024.She was driving Tesla Y model and this accident occurred due to System failure of Vehicle , brakes and Steering wheel get locked and she was not able to do anything and car hit the couple of trees on other side of parking lot . She was not using Autopilot feature and it vehicle didn't stop after applying the brake instead of stopping , its get accelerated and steering was also locked.

Car was being driven into a car wash. Driver went to put car into neutral and car lurched forward and crashed into car in front of it without gas pedal being engaged. Vehicle has not been inspected by manufacturer or others. No warning lamps or other messages. May have been related to recently installed full self driving software that was automatically installed but was not requested by vehicle owner. Vehicle owner did not intend to engage any sort of self driving or automatic acceleration systems.

Vehicle: Tesla Model Y Blue Purchased in Oct’ 23 Tesla setting: Regenerative braking (single pedal driving) Issue: Brake Pedal/Acceleration Pedal Malfunction I was pulling my car out from the parking lot to pick up my mom, from the hospital entrance. I just turned right and had to take an immediate left to leave out of the parking lot. The Tesla was driving at approximately 5 miles an hour at that point. I lifted my foot from the gas pedal for the car to stop with its regenerative braking. However, instead of coming to a stop, the Tesla started rapidly accelerating, with my foot not even on the pedal. When I slammed the brakes, the car was unresponsive and continued accelerating. I started honking madly to warn people that I had no control over the car I was driving. The vehicle continued accelerating over 100-150 meters in 4-5 seconds and banged a parked car (Lexus SUV) at the end of the parking lot. It continued accelerating while pushing Lexus over the curb, destroying road sign poles and landscaping shrubs; crossing the street, and the middle curb. At the end of the street was a tree and a ditch. Lexus couldn’t move any further and Tesla stopped. All airbags deployed. One of the emergency response detection systems called 911 immediately. The first thing the police asked me was if I pressed the emergency brake. I said the car doesn’t have one. I can’t even turn off the vehicle while it is moving. The parking button takes care of it. I used to love my Tesla but I can’t trust it anymore. What I want from them at this time is: Investigate all the incidents thoroughly instead of blaming these as a user error. a) Think about adding the redundancy system. Something mechanical where the driver reacts within a couple of seconds. Software bugs are hard and sometimes it is impossible to find the root cause. Logs may not contain everything that you need to debug the problem. Consider it as a P0 issue. b) Compensate for the damage and property loss.

What component or system failed or malfunctioned, and is it available for inspection upon request? Slowed down entering a driveway when car sped up on its own , Forward Collision Warning/Emergency Automatic Breaking did not warn or do any breaking. NO SOUND or WARNING on the computer screen. I brake but the car still hit the post. I do not know what speed it accelerated to. I am not sure if Autopilot kicked in but the car accelerated when I have already released the accelerator. How was your safety or the safety of others put at risk? YES - injuries sustained. Has the problem been reproduced or confirmed by a dealer or independent service center? When calling Tesla on Dec 23, 2023 - was told by the rep that ticket has to be filed but I was disconnected and I was not able to get through the lines. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Yes - Car was totalled. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? I never saw any warning lamps come on as I always driven with distance. The camera was worked was worked on due to the wiper going on even when not raining on October 24 but the fix did not work. Once at the Emergency room, trying to call tesla or texting the no that responds when buying the car no longer accepting my text. The promise that when we get car accident, Tesla will call - no phone calls. All I received was text alert car suffered failure and was deactivated. I was in pain and not able to follow up and try to do report and look at pictures.. Today I just received a letter dated 2/8/2024 about Recall no 23V-838 in the mail. BUT the car is gone... The software version it had is 2023.27.7. The tech guy came here to the house on Oct 25, 2023 and checked everything.

After making a right turn, the car accelerated automatically and drove onto the sidewalk. The driver was hitting the brakes and attempting to redirect the vehicle with the steering wheel but both components of the vehicle were locked, causing the vehicle to lose control and crash into several things on the sidewalk (flower pot, parked cars, building pillars). The Tesla is completely dead so we are unable to retrieve any data within the vehicle. The safety of civilians on the street as well has the driver and passenger of the vehicle were at risk due to the lack of control of the vehicle. Two weeks after the accident, Tesla sent out a recall notice in regards to autosteer which can increase risk of collision, but prior to this no notice/warning was in place. The police were involved and at the scene at the time of the event and a police report has been made. No messages were seen on the screen or on the Tesla app in regards to any car mode (autopilot, cruise control, etc.) the vehicle could've been on, nor any warnings about risk of collision due to the recall.

At 9:37 a.m. on January 26, I was driving on Freeway 60. The three cars in front of me braked suddenly. I stepped on the brake quickly, but after I stepped on the brake for 3 seconds, the speed of the car still did not slow down, and it directly hit the rear of the car in front.Afterwards, on the Tesla app on, an alarm was displayed, saying that at 9:37, there was a problem with the steering wheel and brakes(unavailable) And also, there is a Manufacturer recall number:TBD NHTSA Recall Number 24V-035; TC2024-027 (the manufacturer has failed or is unable to remedy this safety recall for my vehicle in a timely manner, I want to file an online complaint with NHTSA)

Vehicle reversing malfunctioned , may be a software defect. Vehicle while reversing without pressing the accelerator went berserk and hit another vehicle in the driveway. Vehicle data was provided by Tesla but there is no evidence of collision nor any video recording of the incident. No issues were seen nor any warnings displayed prior to the vehicle reversing itself. However insurance deemed it as driver fault without any confirmation by the manufacturer (Tesla).

I experienced a sudden unintended acceleration while parking the car in my home garage, something I have done thousands of times with no difficulty. I had this brand new car for a 12 days before it smashed through my garage wall. My garage is a fairly tight fit, so as I always do, I partially entered the garage and stopped to make sure that the side mirrors were going to clear the garage entrance. Then, I lifted my foot off the brake in order to start moving forwards again, at which time the car suddenly accelerated like crazy through the remaining 12 ft of space, smashing the cabinetry through the rear wall of the garage. The car, my garage, and laundry room were severely damaged. I complained to Tesla about this vehicle problem, but they just say there are no problems with the car, and it must be the driver’s fault. They are implying that I floored the accelerator, which is nonsense. I have been driving for more than 40 years, and I do not floor the accelerator while parking in my home garage. This type of sudden unintended acceleration is a danger to public safety, and this matter has to be thoroughly investigated, and a solution has to be found to fix the problem.

Two times in one week with two different drivers driving the same vehicle the vehicle experienced sudden unintended acceleration while parking. In the first incident this resulted in damage to the bumper into a wired fence which the vehicle did not alert but rather accelerated instead of applied regenerative breaking. Four days later the same incident happened with sudden unintended acceleration occurring while parking the car. When sending the car to Tesla for service due to the safety concern and damage this resulted in they said this was a user error which can’t be the case as this incident happened with two different drivers driving the same vehicle. When reading about this there have been numerous complaints which have shown that there is an electrical “ They show that the sudden unintended acceleration of Tesla cars may have been caused by negative voltage spikes that are logged as a fully depressed pedal, fooling both Tesla and the NHTSA investigators that there has been no hardware or software defect.”

I was driving at a very slow speed of 1 mile per hr as I entered into the Kohls complex in Pleasanton ca. Then as I was pulling the car to a parking spot, all of a sudden I felt the car accelerated on me. My foot was always on the break and I pressed the break but the car still accelerated at a speed I had no control and it went over the parking bump and hit 2 cars that was parked opposite. I felt out of control and scary because the car was not doing as intended, to stop when pressed the break. I then pressed the park button. Then I also reversed because the car was not stopping. There was totally 4 cars damaged including my car. 2 opposite and 1 parked side of me. I could feel the sudden acceleration in the car and out of control feeling. Luckily there were no people in other car so no one was hurt. I was also not hurt. - Not sure what malfunction happened , but the break in that instant of time was not acting as it should for few seconds. Yes, the car is available for inspection - My safety was at risk as the car was no behaving the way it should and I got scared as to what would happened next. Since there were no people in the other car, there was no risk. but if there were they probably would have got hurt. - I reported the issue to Tesla center. They said they will look but never heard back anything from them . - None of them has inspected the vehicle yet. - No warning signs were there. after it hit i could hear the beep

The car was going at the speed limit or maybe 5-9 miles over speed limit when brakes were pumped to stop car as there was stop and go traffic on a bridge in bay area (CA). The car had quite a distance from the car in front of it. Despite brakes being applied, the car seemed to accelerate and brakes had to be applied hard. However, the car did not stop until it hit the honda in front of it. As the driver of the Telsa at the time of the accident, it seems as though car was accelerating despite brakes being applied. A huge sound was heard by me the driver and then the driver eat belt became malfunctioned and the car would not restart. After a few minutes, the car restarted but now the dash board states service needed and I could no longer fasten the seat belt. The front of the car appeared to have minimal bumper damage but the seat has not worked since the accident.

I bought Tesla in May 2023. My Tesla Model Y has suddenly accelerated in my garage on 08/29 @8.25pm EST and damaged my garage front door/drywall and broken my stair case in pieces. It was phantom acceleration. I have video from my ring security system camera video which shows clearly that I was parking in my garage extremely carefully, however the Tesla Model Y suddenly accelerated beyond my control (I was not accelerating) and put me under risk and damaged my property. I have auto insurance to cover my Tesla car damage. However, my biggest fear is how can I use this Tesla Model Y when we see this life threatening behavior. This unintended acceleration can put my life and others life into danger. I would like help from NHTSA to ensure my Tesla Model Y is defect free. I just bought couple of months ago. I would like to request NHTSA to help me get this product replaced from Tesla motors. I am afraid to drive this vehicle now. I am currently going through a big shock, trauma and fear that I may have severely injured people on the road or my family members in garage if this unintended sudden acceleration would have happened around them. The damaged vehicle is outside my garage. It is not drivable. Vehicle front bumper is completely damaged and fluid has been leaking. I have filed insurance claim. I don't know what should I do right now. I would like to seek advice from legal sources on my options. My car is covered under my insurance, however I don't know if my car is fixed - I can drive this vehicle with confidence due to this strange unintended acceleration issues. Please help me at the earliest.

I was pulling up to a ticket machine in a parking garage. I had my foot on the accelerator and was using regenerative braking and was essentially stopped. I was short a few inches from the button to get the ticket so applied almost no pressure to the accelerator – more like a tap so I could go forward a few inches. The car went into full acceleration mode, broke through the ticket machine barrier and kept accelerating. As soon as I sensed going forward too fast, I took my foot off the accelerator and put it onto the brake. I did not feel any response from the brake. With my foot. I managed to steer away from a wall in front of the car but hit a parked car. The impact pushed that car into a second car and then that car pushed into a third car and the third car pushed into a fourth car. My car was totaled as well as three others. Fortunately, the impacted vehicles were unoccupied and there were no pedestrians. No warning beeps or emergency braking occurred. One of my passengers had minor injuries (3 fractured ribs). A police report was filed. Previously (3 or 4 weeks prior to crash) I had an incident in an above ground parking lot. I had pulled into a parking space, backed up to straighten out then was pulling forward towards the curb. I took my foot off the accelerator, but the car kept going very slowly instead of regeneratively braking as it had done before. I put my foot on the brake. It was unresponsive. I kept pressing and finally it engaged, and I was able to stop the car about five feet over the curb and into the grass.
