This incident is reported by a law office. The car being driven by our client was a TESLA Model 3 which made a SUDDEN UNEXPECTED ACCELERATION when our client was slowly looking for a parking space at a parking lot. The TESLA car suddenly accelerated at a high rate of speed without client pressing on the accelerator. On the contrary, our client was stepping on the brakes in an attempt to stop it but the TESLA did not stop but still continued to accelerate. This accident caused property damages and injuries to our client, his passenger, and others.

As I was approaching my off ramp (which has a very tight turn), my automatic brakes failed which caused me to slam onto the brakes, and skid into one of the off ramp signs. All 3 of my children were in the back, the back driver side window shattered, I lost my driver side mirror, and there was damage done to the back door and wheel well (all on the driver's side). Although the damage looked minimal, no one was willing to work on the vehicle and I was strongly encouraged by my insurance company to claim it as a total loss. Granted, I had this vehicle for approximately 2 months, and had put at least $4,000 into it.

On [XXX], around 1:00, I was driving normally on the highway. I remember using autopilot, and the speed limit was 55 mph, but I'm not very sure. At that time, it was raining heavily, and I was following the car in front, not doing anything else. Suddenly, I noticed that the Tesla car turned sharply to the left by itself! I couldn't control it. Let me repeat: the Tesla car suddenly turned left by itself. I instinctively grabbed the steering wheel, turned in the opposite direction to correct it, and pressed the brakes, but it was too late. I collided with a car in the adjacent lane, and the car later turned about 180 degrees and stopped! The left side airbags in the front and back all deployed! However, I were not injured. I got out of the car, immediately called the police, and contacted the owner of the collided vehicle. Later, the police arrived to document the incident. I believe it was a malfunction of the Tesla that caused the accident! I did not make a left turn manually! It was in normal driving mode. What component or system failed or malfunctioned, and is it available for inspection upon request? I have no idea, should check Tesla's driving records and the recorded video from the incident. Has the problem been reproduced or confirmed by a dealer or independent service center? No. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? No. Were there any warning lamps, messages, or other symptoms of the problem prior to the failure, and when did they first appear? On the day of the accident, in the morning, I noticed that the car exhibited a slight phenomenon of accelerating and then stopping on its own while I was driving, It was first appear but it wasn't severe. On Jan. 1, 2024 at around 2:19-2:20PM, the vehicle in front of me made a sudden brake causing my vehicle to trigger the Automatic Emergency Brake. My foot was on the brake pedal as well, I even floored the pedal to make my car stop right away but it continued anyway, the pedal felt so loose/soft. My car almost made a full stop but when the vehicle in front of me started moving, the car started to somehow accelerate, I was confused as to why the car was moving when my foot was firmly pushing the brake. This unexpected move caused me to rear-end the vehicle in front when they abruptly stopped for the second time. My thought is since the car in front moved, the Tesla thought the vehicle in front was no longer a threat and stopped applying the brake to make a full stop. I watched a lot of AEB videos online. 1. Automatic Emergency Brake - malfunctioned, maybe, the car log has it but I took a photo that AEB was in progress during the incident 2. My experience is similar to a car without a brake - no matter how hard I pushed the brake pedal, the car didn't stop. It's frightening because I'm out of control over the situation. 3/4. My car was already repaired but I haven't taken it to a Tesla Service Center to inspect the system - the manual brake is working though 5. There were chimes and screen notifications that an Automatic Emergency Brake was in progress. I can upload the dashcam recording if you need it.

The contact stated after leaving the dealer for a previous issue with a locked steering wheel, the contact was driving 50 mph on an entrance ramp as he accelerated to merge onto the highway, the steering wheel seized to the right in non-autopilot mode causing the contact to crash into the guardrail on the right side. The contact injured his neck and received medical attention. The emergency respondent was not called, but the contact obtained a police report. The vehicle was towed to the dealer but remains unrepaired, and needed engineers' diagnosis and legal investigation by Tesla. The contact received notification of NHTSA Campaign Number: 23V838000 (Electrical System). The manufacturer was made aware of the failure. The failure mileage was approximately 1,798.

I was pulling the car in to a parking lot in a mall. I was driving very slow to bring the car to a halt. All of a sudden the car just accelerated and went and hit a pillar right in front of the parking lot. I don't know what caused the vehicle to suddenly accelerate. I am pretty sure I did not step of the gas pedal. I checked the dashcam for recordings but couldn't find any. We have experienced this sudden unintended acceleration before on the road as well. I think I must have engaged auto-pilot or auto-steer by mistake which caused this.

Summary: FSD malfunctioned, failed to disengage, causing loss of steering control, thus causing car to steer into wrong lane and resulting in an accident. My car has been driving in FSD on Lawrence Expressway and it was doing everything correctly. Approaching that upcoming tight bend, the FSD slowed down the car, and I took over by steering the steering wheel to the right to deactivate FSD and taking the tight turn, right before the white rows of triangles on the street. At this moment, the steering wheel forcibly turned to the opposite direction (i.e. left). I tried to correct the steering wheel by turning right even more, but the steering wheel wasn’t responding to my input for a split second. At this moment, my car is now on the wrong lane of Wildwood Ave and my car finally responded to my steering input. I had to swerve in the opposite direction to avoid the incoming traffic and ended up brushing against an SUV, followed by an impact on the curb, before finally slowing down. Seeing that there was incoming traffic from the back, I drove down to a residential street to park my car for safety reason. The SUV driver called the police to handle the situation, we had our information and testimonies recorded down. I have already followed the safe practices of driving with FSD according to the manual, but in this situation, I am not certain which technology is responsible for causing my car to override my manual steering input. When I was trying to park my Tesla Model 3 car it suddenly accelerated and hit a parked car in front of me. I took the car to the repair shop and told them the situation. My reason for the accident didn’t make sense to them so I stated that I may have pressed accelerator instead of brakes but I know that I didn’t press the accelerator. My car has been at the repair shop since 10/14/2023. So, far no one from the Tesla Motors, Inc. came to investigate the claim. Even if they repair the car, I don’t feel comfortable driving the car due to the fact that unintended acceleration may repeat itself.

I was reducing my speed as I approached a stoplight, and my vehicle rapidly and aggressively increased in speed. It made a revving sound and lurched forward, causing me to rear-end a truck. There is no possible way that this was human error and it was terrifying. The person who I rear ended mentioned that the impact felt like a double impact, as if I was continuing to accelerate after he was hit. I have requested the data from the accident time period from Tesla. An insurance representative is organizing an inspection. This was the second incident where the vehicle lurched forward on it's own. The first one happened about six months prior when I was at a complete stop. It did not cause an accident but it did frighten me into an audible scream. I did not report it because after some basic information gathering, I decided that it somehow must have been human error. Every claim I've seen has had the outcome of user issue. During both lurching incidents, a revving sound was made and it felt like and electrical surge of some sort. I am extremely clear on the fact that I did not cause the vehicle to dramatically crash and I feel helpless to know that it will likely be ignored by Tesla. What if I had been in front of a cross walk when the vehicle surged and lurched forward and a life had been seriously injured or even taken? Something must be done to expose this issue and get to the bottom of it. I am happy to provide any additional information in order to prevent this from creating future disasters. It just happened four days ago so the information I have is limited.

I am writing to report an incident involving my Tesla Model 3 (Blue) and to initiate the process for an insurance claim. The incident occurred on [XXX], at approximately 2:15 PM. My fiancée was the driver of the vehicle at the time of the incident. Incident Details: - Date: [XXX] - Time: Approximately [XXX] - Vehicle: Tesla Model 3 (Blue) Location: The incident took place on the freeway merge from [XXX], where it merges onto [XXX]. Heading home. Circumstances: My fiancée was operating the vehicle in Autopilot mode while maintaining her hands securely on the steering wheel. As she approached the freeway merge from ramp [XXX], heading onto [XXX], she experienced an unexpected jolt. Despite her continuous control of the steering wheel, the Autopilot system seemed to be accelerating without her input. Simultaneously, my fiancée felt a sudden tire malfunction, which resulted in a significant loss of control over the vehicle. Despite her best efforts to maintain the car's position within the lane, the Tesla Model 3 veered off the freeway and entered the adjacent Ravine area. Impact and Airbags: Upon entering the Ravine area, the vehicle experienced a collision. multiple plants/highbrush/uneven terrain as well as having to maneuver around 2 large palm trees which resulted damage to side mirror. Notably, none of the vehicle's airbags were deployed during the incident, and there were no sensor alerts, except for the tire pressure warning light. Also there wasn't any record of dashcam recording when we checked the car. We are seriously concerned that the airbags did not deploy , attached are photos concerning the damage. I would like to emphasize that my fiancée was diligently following safety guidelines and had her hands on the steering wheel while using Autopilot. This incident occurred suddenly and unexpectedly. On 09/11/2023 at approx. 6AM CST, my wife went to a nearby Walmart store for groceries. As she was parking the car in the parking space, the car started to accelerate by itself and went around the parking lot in random directions. She couldn't control the steering and brake despite the continuous tries. It was such a traumatic experience, and the car went on to hit couple of other parked cars and the cart return stations in the parking lot. After few seconds of random self-drive in multiple directions of parking lot, it finally came to a halt as it got on a curb that flattened the tires. By God's grace, the car stopped right before entering the main busy road due to flat tires and also no pedestrians at that time. Otherwise, the situation could have been worse as the car was going in an uncontrollable manner. The incident was reported to 911 and they got the car towed to a towing garage. We got the incident report from the police department. They also have the CCTV footage of this incident. We also have the dashcam videos recorded by TESLA and pictures taken after the incident. I will provide any supporting documentation you might need to understand this incident and address the issue. We are trying to come out of the trauma from this incident and would like to bring this issue to the attention of all concerned parties for next steps.

It happened in a parking lot around 8:20 pm on 09.13.23. When I slowly moved to parking space car suddenly accelerated, jumped over raised pavement for pedestrian walking, hit parked car on the other side of walking path, continue going with uncontrollable high speed across the parking, then car slowed down and I finally parked. Woman who witnessed accident and called the police told the police officer that she saw brake lights of my car were on. Two women from the car I hit showed me the markings from my tires on pavement. My husband who arrived to the place of accident by this time took pictures of these marks and next morning we return back to take another pictures and video. On May 22, 2023 I had an annual physical exam at the doctor's office at 9:00 am. Upon completion of the visit, I drove my model 3 from the parking spot (3rd level) towards the ground floor. At the final turn, at the ground floor, I felt that the car had an unusual resistance when I tried to navigate the turn. The steering wheel felt as if it were locked. As a result of not being able to turn the steering wheel, I hit seven cars that were parked at the ground floor and incurred substantial damage. The car is totaled unfortunately. I am hoping that the NHTSA may have a way to analyze the true cause of this accident before they send the car to the salvage market.

I was stopped before turning left into a parking space. I was driving slowly. The car stopped but I did not engage the brake as a Tesla will stop once the foot is removed from the accelerator. At that point the car leaped forward at full speed hitting parts of a building and a fence. I did NOT step on the accelerator. No one was injured.
