What 208 owners told NHTSA about the 2022 Mitsubishi Outlander Mhev

2 issues contributed to accident: 1st - Vehicle did not go into Park when button on gear shift was pushed. Driver did not notice that vehicle remained in Drive and exited the vehicle. 2nd - Vehicle did not automatically engage the emergency brake/park mode it is designed to do when Vehicle is in drive and driver door opens. As a result, Vehicle began to roll forward. Driver was knocked down and fell to ground and Vehicle ran over her legs, coming to a stop with the rear Driver side tire on her leg.

On January 12 at 9:54am I was driving less than a 1/4 of a mile. I stopped at a stop sign and my cabin was filled with smoke. I thought it was coming from the engine so i turned the heat off for the car. I proceeded to the next stop sign and I then realized that my driver side seat was on fire and the smoke was coming out from between my legs for the seat was burning from the inside. I have taken the car to the dealership and it is reported that there is a faulty heating element in the seat. The car can not be started for each time the car is started the fire reignites. Apparently the fuel system (fuel canister, fuel dispender, fuel vapor valves). It caused fuel, gasoline, odor to enter inside the vehicle cabin, causing dizziness and nausea to me, my wife and two kids. The problem was confirmed by the dealer. They try to fix it on 3 different occasions without sucess. As of today they requested the vehicle as they recognized is a safety issue and handed my a temporary vehicle. They told me that the repair date is indefinite as the parts doesn't exist in the US, and was order directly to the manufacturer (outside of US). I have knowledge about other persons that are experimenting the same problem.

The warning light starts flashing to alert me it is not active, this happened periodically but the dealer was unable to see it happen. Saw it happening so I went to take it to the dealer but didn’t notice the car in front of me due to the flashing light taking my attention and hit the car in front of me.

The defect is reported by the air bag on the driver's side. This defect caused emotional and physical damage in which a doctor certified trauma to the head and legs due to the malfunction of the air bag. The vehicle is currently in the workshop to be fixed, so it has not been evaluated by a dealer. on the other hand, if it was inspected by shop personnel and the insurance representative. Before the accident, the vehicle did not turn on warning lights or any other signal that would notify the defect.

Front Collision Mitigation System, FCM, failed on frontal impact and airbags did not deploy. Vehicle did not activate audible or display emergency warning signals prior to frontal impact. We filed a complaint with Mitsubishi Motors national CR, and Director of Service with many calls, voicemails and emails just fell on deaf ears. Their response documented in the case report on the last call we made to Mitsubishi Motors on 6/17/22 the CR rep said the decision is final and that they would not do anything, and we should contact our insurance. Mitsubishi's not only "Did not inspect the vehicle" but also never inquired with any intelligent questions as to why the airbags and FCM system failed to operate. Vehicle has not been inspected by anyone and is currently in my garage with less then 400 miles on it and is available for inspection. No prior warning lights were displayed. The dashboard currently displays crash signal and forward warning lights.
