I was involved a car accident with two other co-workers (USDA-NRCS) on the job in our government issued F-150 by Ford. As were sitting still waiting to make a left turn, we were rear-ended by a 1990 Dodge van on 5/31/2023. During the impact my headrest deployed which led to me having three surgeries( ACDF procedure on C3-C5, right shoulder surgery, deviated septum) surgery and having to put in my disability retirement application in this summer since I haven't worked in three years. I want a recall on the headrest deploying in the 2022 F-150.

I bought a used Ford f-150 from Toyota used care dealership on [XXX]. On [XXX] while vehicle was being driven on [XXX], north of Santa Clarita, on the way down to San Diego. I was in the backseat, my friend who was the driver notified me. That we had to pull over and check because he was noticing smoke, then we pulled over and the truck was leaking a fluid that was on fire for about 20 minutes. We had exited the vehicle, called 911, the fire then stopped after about 20 minutes. The fire department came and then put out a small fire within the truck. We then had to wait on the side of the rode for 5 hours waiting for a tow truck to pull the truck. I got the truck towed to a ford service center because the truck still had new vehicle manufacturer warranty with Ford. The truck was given full warranty and Ford covered the damages and fixed the vehicle giving it the green light in there service report. I then waited until March 4th 2025 to receive my truck back. On [XXX], the truck was being driven down southband the [XXX], on the [XXX], again due to smoke and also hearing a loud banging, my friends had to pull over and the truck is going to be towed to the service center which it was first fixed. The structural integrity of the hood and front bumper failed due to unsafe modifications performed by Rocky Ridge and endorsed by Ford Motor Company. The hood's structural reinforcements and crush zones were cut to fit a hood scoop, and sections of the front bumper were removed, compromising crashworthiness. The vehicle is available for inspection upon request and I have documented with images. The modifications to the hood and front bumper compromised the vehicle’s crashworthiness, potentially increasing the risk of severe injury or death in a collision. The removal of structural reinforcements and crush zones on both components prevents them from functioning as designed to absorb and distribute impact forces during a crash, endangering all occupants of the vehicle. The problem has been confirmed by an independent collision repair center, a Ford Certified Collision Center and my insurance company USAA. Both parties have stated that the modifications to the hood and bumper are unsafe, and the insurance company has refused to authorize repairs that replicate the original modifications due to safety concerns. The vehicle has been inspected by my insurance company and a collision repair center. Both confirmed the safety concerns resulting from the modifications and highlighted that the affected components cannot be repaired or replaced without compromising safety. Images were provided to Ford Motor Company and inspected by Ford Collision Center Manager. There were no warning lamps, messages, or other visible symptoms of the problem prior to the discovery of the modifications during a repair process. Ford Positon Statements and On Target Tech Bulitens prohibit these types of modifications and were violated, then conceled so that the consumer would not be able to see them unless in an accident. Ford is citing that the time frame exceeds 24 months so there is nothing that they are willing to do about it. This is a failure to warn and product defect that impacts safety.

This morning, as soon as I started the vehicle and started backing down the driveway, an unbelievably loud popping sound emitted from the speakers. I immediately slammed on the brakes having no idea what had just happened. This has happened previously but have not heard in a few months so I thought the problem was corrected. It is the loudest noise I have ever experienced and is extremely dangerous as it happens at different times and is just ear splitting. My ears are still ringing and it happened at 8:45am this morning.

While driving down the highway, my 2022 Ford F-150 Tremor randomly caught on fire. I was driving down the highway and was flagged down by other drivers to let me know the bottom of the truck was on fire. I pulled over and luckily someone had a fire extinguisher and we were able to get the fire under control. The fire department was also called and they also helped put out the fire after their arrival. The truck has always been serviced at a Ford dealer and had gone in for an oil change less than 350 miles prior to the truck catching fire.

I was driving this morning and out of nowhere, my radio starting making the loudest noise I've ever heard (x3). Louder then a darn gun shot. I almost had an accident because I thought the truck was exploding. Had to slam on the breaks and jump out. Dealership was no real help, so had to walk all the way home to get noise canceling headphones so I could reenter the vehicle. Re-entered to a burning smell. Called Ford and was told to try and remove and reinstall the battery. I did this and it fixed it, but now driving with noise canceling headphones and can't have anybody in the car out of fear for hearing loss. Sitting here at my desk with ringer ears and bad headache.

Upon entering the Ford f150 vehicle and turning it on there was loud sonic boom noise through the speakers and then ear piercing static. It could not be turned down by the radio controls. Radio was turned off but static still wouldn’t stop. Vehicle was turned off and static still was still going. I waited 15 minutes with the vehicle off and it finally quit. Truck started no problem and static gone. I have read of numerous reports about this same thing with the B&O Unleashed radio system which I have. I have suffered some hearing loss and ringing in my ears even after 2 days. If this had happened on the highway or in traffic it would probably have caused a wreck.
