Report on Accident [XXX] 4TH DISTRICT wHEATON MARYLAND [XXX] [XXX] I was driving west on [XXX] at approximately [XXX]. As I approached the intersection at [XXX] the TRAFIC light cars were stopped . I was in the left hand lane because I was going to turn on to [XXX] . As approached the intersection I applied the brakes the pedal went close to the floor but the car did not slow down. I tried pumping the brake pedal with no effect. No collosion avoidance alert To try and slow the vehicle ran the wheel against the curb of the median which slowed the car down a little . I pressed the parking brake button with no effect. The collision avoidance system did not engage. I clipped the rear of the Toyota Corolla as I jumped the curb to avoid cars using the friction against the curb and move further left when oncoming cars were clear and was able to stop the car. (The car may have stopped because the front control arm bent which made the car drag. ) I am unsure whether the master cylinder failure or ABS overrode braking, but the car would not stop. I do not know if the black box captured the data as the there was no airbag deployment. The collision avoidance system did not engage to slow the car any time. I did not see any lights light on the dashboard did not check specifically as I was busy avoiding a collision other cars . Basically, pedal went down not all the way to the floor but car was not slowing down. I checked the master cylinder reservoir, and it still had fluid. Driving slowly in a parking lot, I applied the brakes as I approached a parking space. Instead of slowing down, the car inexplicably accelerated quickly and did not respond to increased pressure on the brakes. The car was propelled into a metal post, resulting in my glasses flying off and a seat belt bruise, although the airbag did not deploy. I was examined by ambulance personnel at the scene. The car was towed, inspected by an insurance adjuster and eventually declared a total loss. There were no warning lamps prior to the incident and no reproduction of the incident was possible due to damage. The car has been relinquished to the insurance company. This was the first such incident occurring during 8 years of driving this vehicle.

The contact owns a 2018 Subaru Outback. The contact stated that when driving approximately 30- 40 MPH, veering off the highway, the vehicle suddenly lost power, but had immediately regained power. The failure had occurred 3 - 4 times total. The last time it occurred, the vehicle took a bit longer to regain power. After pulling into the desired parking lot, the contact checked the battery. When touching a wire connecting to the battery, it sparked and caught fire. The contact immediately grabbed some snow nearby and threw it onto the wire to put out the fire. There was no fire report filed. There were no injuries or medical attention needed. The contact had noticed that the instrument panel would flash with several warning lights; however, it would illuminate quickly and intermittently. The vehicle was then scheduled to be towed from the parking lot, but it had not been taken to the dealer after the incident. The vehicle had not been diagnosed or repaired. The manufacturer was not informed of the failure. The failure mileage was unknown.

MY 2018 Outback experienced a driver side smoking mirror a few days ago. While parked at the gas station, a large amount of smoke suddenly began emanating from the driver side mirror. As a result I no longer see my TPMS, the ability to lock/unlock the car with the remote, interior lights no longer work.

I slowed down to approximately 5 MPH to pull into a parking space in front of a building. Without warning, the car suddenly accelerated toward the building. The car jumped over the parking block and onto the sidewalk in front of the building. I attempted to stop the car but the brakes did not function at that point. The car stopped when it hit a pole. There was damage to the front of the car and the pole. No people were injured. There was no warning before this happened.

[XXX] at [XXX] We were sitting at a complete stop with our turn signal on, waiting for traffic to clear enough to turn left. It was dark/cold but otherwise good road conditions, no rain/snow/wind. We were suddenly slammed from behind in the rear/R passenger rear by the Black Dodge Journey. This impact then pushed us into the oncoming lane causing another vehicle to hit us in the front driver side of the vehicle. 911 was called at 1811 and the call lasted 3 minutes. SBPD was dispatched along with traffic control. The impact caused the visor on the passenger side to fall which caused broken facial bones. The headrest on the passenger side was completely dislodged and out of the seat upon inspection after the scene was secured. Drivers headrest was nearly out at the end of impact. Driver hit his head on the steering wheel. Entire back end of Subaru was smashed and a trail of what appeared to be gas from the tank made a visible line (~15 ft) of how far the car had been pushed on the road and across traffic. The man driving the journey reported that his “power steering went out while trying to pass on the shoulder.” Passenger had to forcibly push open the front passenger door to get out; the back passenger door was not usable and the gas cover popped out. The entire back end of the vehicle was smashed including the glass. The airbags on the Dodge Journey deployed, as well as the airbags in the vehicle that hit the Subaru in the front. NO airbags in the Subaru deployed. Medics were dispatched to the scene. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

I was visiting my daughter's and after I pulled in the driveway, and I hit my brakes to stop the car, but they failed, and I ended up crashing into their brick garage. I was physically fine, but my car and their garage was damaged. There were no warning lights of failure. I had Autozone check the car computer today, and they came up with an ABS failure code of C1411. I've discussed the situation with a Subaru mechanic on [XXX] , and he says that ABS module has failed and should not happen on 2018 Subaru, that only has @44,000 miles on it. Two incidents of unprovoked acceleration 1st incident: approximately March 2021 Slowly backing up at a gas station and experienced sudden acceleration causing the car to hit a metal guard at the fuel pump. This resulted in damage to the vehicle requiring repair. 2nd incident: 12/2023 Slowly entering a parking space when there was sudden acceleration causing the vehicle to to go over a concrete curb across a grassy median to hit an unoccupied car parked directly across. This resulted in damage to the bumper and damage to the car it hit. Police were called but they were busy and this happened in a parking lot so an incident number was obtained and insurance information was exchanged between driver and other vehicle owner.

While pulling into a parking spot with my foot on the brake and preparing to stop, the car continued to move up over the curb and then began to accelerate even after I applied both of my feet pushing down on the brakes until I hit our condo building. The car and building received extensive damage, and although the car was not totaled it will cost over 7,000 dollars to repair. Personally I was not injured. The air bags did not imploy at impact. There were no warning messages or prior failures with the vehicle before this day. The car was taken to a body shop and not the dealer so we have not had the dealer look at the systems that broke down, but our insurance adjuster will be.

The contact owns a 2018 Subaru Outback. The contact stated that while their wife was pulling into a parking space, uphill, when the vehicle suddenly accelerated. The vehicle jumped the curb and crashed into a sign. The vehicle continued accelerating approximately a car length of distance causing it to jump another curb onto grass and eventually come to a stop. The air bags did not deploy. The vehicle had damage to the front end and bumper of the vehicle. The contact had no injuries and medical attention was not needed. A police report was not filed. The vehicle was able to be driven away from the scene. The vehicle was taken to the dealer who had not diagnosed the vehicle and it had not been repaired. The contact stated they found an article regarding the year/make/model experiencing the same failure due to a failure of the low-pressure fuel pump. The manufacturer was not made aware of the failure. The approximate failure mileage was 41,000.

Car jerked back and suddenly accelerated without warning as driver had foot on brake and was slowly pulling into parking spot. Car hit wall of parking garage causing front end damage. Parking collision warning did not sound at any point. Car was going 1-2 MPH and was 5 feet from end of parking spot when incident occurred. Sudden acceleration when driver has foot on brake can cause injury to driver, passengers or others in the path of the car. No hazards in parking garage were present that could have caused accident, which was witnessed by driver's work colleague. No police report filed. Incident was reported to Subaru, which ran diagnostic and could not replicate issue. No warning messages were present prior to the incident. It appears there are 2 current class action lawsuits filed against Subaru reporting similar issues with 2018 Outback. I am unsure what component failed to cause this issue; car is available for inspection on request.

I was backing out of my driveway very slowly as usual. I checked the road both ways several times. A US mail truck evidently traveling at a high rate of speed came up behind me while I was looking in the opposite direction. The automatic braking did not kick in, the warning system didn't work. After the accident the green, yellow, red lines on the camera and the warning beeping stuff-all were not working for several weeks. All of the sudden, they started working. HISTORY-4 days after purchasing the vehicle we have had lane departure, driver assist problems. The passenger window does what it wants. The electric controls for the heat/ac go blank, or can't change settings. There seems to be a huge electrical issue. Every time we've taken the car in for service, the dealership just insults me that I can't drive and they can't find any problems. They seem to have an answer for everything yet the problems persist. Efforts to go thru Subaru are stalled waiting on their response. They were to look at the BLACK BOX during time of accident. We are still waiting. We are frustrated. I wish the dealership had taken the car back after we first bought it like we demanded. I'm terrified this car is going to cause an accident.

The vehicle, a 2018 Subaru Outback Limited 3.6R, was being pulled into the garage at the owner’s home. With brakes applied, and while putting vehicle into park, the vehicle suddenly accelerated forward making contact with a suspended shelf attached to the back wall of the garage. The force of the vehicle was such that the shelf and back wall of the garage were completely separated from the home in one piece and pushed approximately 10 yards away from the home before the shelf broke away from the wall upon impact with the ground, perpendicular to the back of the house. The vehicle then traveled down two hills in the back yard, across a water saturated easement, over a concrete drainage culvert and came to rest at the base of an incline in an adjacent yard approximately 150 ft from the garage. Air bags did not deploy and the owner suffered minor contusions causing pain and stiffness, but no significant injuries are noted as of this report. The vehicle was totaled and significant damage was caused to the residence. No collision warnings were displayed prior to collision and emergency braking did not apply upon acceleration. This was the first incidence of sudden acceleration experienced by the owner so this issue has never been checked out by a service technician and will not be repaired. The vehicle had less than 8000 miles on the odometer. The vehicle is currently located in a repair lot and the owner’s insurance company has been requested to hold the vehicle for inspection prior to disposal. The cause of the sudden acceleration is unknown, but the owner is confident that the brakes were being applied at the time of occurrence, and that they remained cognizant throughout the incident as they were unable to stop the forward progression of the vehicle until it came to rest on the muddy incline.

I was rear-ended on August 30 in this vehicle while at a traffic stop by a truck traveling 65+mph. Upon impact, I was pushed into the vehicle in front of me at a high rate of speed. Despite being hit from behind and pushed into the vehicle ahead of me no airbags deployed in the vehicle. As a result, my head struck the steering wheel knocking me unconscious, and has resulted in a traumatic brain injury, labyrinthine concussion, whiplash, and damage to my vision and hearing. The vehicle was totaled out by my insurance company and the vehicle was sold to CoPart and I don't believe anyone has investigated it even though all parties have been made aware of it. Prior to the accident there were no warning lights and all vehicle safety systems appeared to be in working order.

Sunday,August 1, 2021, at 9:05am, I backed out of my garage and when I attempted to turn my car, the steering locked, the car was drifting backwards and I had no brakes. As a result the car kept drifting backwards , off the driveway into the woods, hitting a tree. Once stopped, I was able to start the car and drive it out of the woods. The entire driver’s side was sideswiped and mirror torn off. I was not injured, but could have been. I called insurance that day. On Monday, we contacted the Subaru dealership. The local police were notified and documented the report. The insurance adjuster determined the car unsafe to drive and it was towed to the dealership August 4, 2021. There was no warning or indicated problem prior to the incident.

Jan 10, 20201 injury

WE PURCHASED A NEW 2018 SUBARU OUTBACK IN JULY 2018. MY WIFE WAS THE PRIMARY DRIVER OF THIS VEHICLE. BY APRIL 2019, MY WIFE WAS SUFFERING FROM SEVERE KNEE AND HIP PAIN ON THE RIGHT SIDE OF HER BODY. DURING HER VISITS TO HER ORTHOPEDIC SPECIALIST (KNEE), SHE WAS ASKED MULTIPLE TIMES IF SHE WAS IN AN ACCIDENT. IN JULY 2018, I GOT TO DRIVE HER CAR ON AN 8 HOUR TRIP. BY 5TH HOUR, MY RIGHT KNEE WAS IN MAJOR PAIN, LIKE BEING ON FIRE FROM WITHIN. IN CLOSE INSPECTION OF DRIVING POSITION, I FOUND THE FOLLOWING: 1. TRANSMISSION TUNNEL INTRUDES IN DRIVING POSITION TO OPERATE GAS PEDDLE 2. GAS PEDAL IS TUCKED AWAY TOO FAR TO THE RIGHT (LINE OF SIGHT COVERED BY TRANSMISSION TUNNEL) 3. THE DISTANCE BETWEEN THE GAS AND BRAKE PEDAL ARE LONGER THAN ANY OTHER CAR THAT WE HAVE OWNED. OPERATING THE TWO PEDALS REQUIRE WIDE TURN OF RIGHT ANKLE). THE COMBINATION OF THE ABOVE 3 DESIGN ISSUES FORCES DRIVER TO OPEN THE RIGHT LEG AT THE HIP. NEXT TO TURN TO RIGHT FOOT BELOW THE KNEE FURTHER TO THE RIGHT. LASTLY REQUIRE WIDER TURN OF RIGHT ANKLE TO OPERATE BRAKE PEDAL IF NOT LIFTING RIGHT FOOT TO OPERATE THE BRAKE PEDAL. LATER THAT DAY, WE DID AN INTERNET SEARCH ON "SUBARU" + "LEG PAIN". WE FOUND OUT REPORTED ISSUES SINCE 2013. ALSO, OUR SISTER IN-LAW WHO HAS A SUBARU FORESTER SHARED THAT AT TIME SHE FEELS DISCOMFORT IN HER RIGHT LEG WHEN TRAVELING LONGER THAN 2 HOURS. SINCE THEN WE HAVE HEARD FROM OTHER SUBARU OWNERS HAVING PAIN IN RIGHT SIDE OF THEIR BODY. INFORMATION SURFACES ONLY WHEN WE SHARE OUR EXPERIENCE. SINCE JULY 4TH 2018, I HAVE BEEN DRIVING THE OUTBACK. WHILE MY DAILY COMMUTE IS NOT AS LONG AS WIFE'S, I AM EXPERIENCING DISCOMFORT IN MY RIGHT KNEE AND ANKLE. I HAVE REACH OUT TO LOCAL DEALER AND SUBARU AMERICA TOP EXECUTIVES. SO FAR THEY REFUSE TO ACKNOWLEDGE THAT THEY HAVE A PROBLEM OR WILLING TO WORK WITH ME.
