The contact stated that the air bags deployed in his vehicle. The contact stated that he sustained head and back injuries. Medical attention was required. The involved individual was found with altered mental status on a highway on 8/13/2023. The vehicle he had been driving was found in the highway median a few hundred feet away. He was admitted to a local hospital, diagnosed with blunt force injuries of the head, and pronounced brain dead on 8/14/2023. Autopsy examination revealed fractures of the skull, subdural and subarachnoid hemorrhage, and cerebral cortical contusions. Mechanical vehicle inspection revealed that the driveshaft flexible joint failed while the vehicle was in motion, with resulting driveshaft uncoupling and severe damage to surrounding vehicle components including the brake and fuel lines. Investigation was consistent with the decedent exiting the moving motor vehicle.

The contact owns a 2016 Ford Transit. The contact stated that there was a rubbing burning order coming from inside the front end of the vehicle. The vehicle also had a vibration sensation whenever the vehicle was being driven. There were no warning lights on the vehicle. The contact contacted the dealer who stated that the flex disc had failed. The contact stated that this was a known issue for the vehicle. There was a recall and lawsuit involving the vehicle's rubber cupper between the transmission and driveshaft was made of plastic was causing severe failures. The manufacturer was made aware of the matter and had since been looking into the matter and informed the contact that their research was being furthered along regarding parts before something was to be done. The contact stated that the manufacturer had failed to provide a permanent fix or the guaranteed maintenance at replacing the part at every 30,000 40,000 miles. The approximate failure mileage was 71,000.

I OWN AND OPERATE A FORD F 250 TRANSIT VAN. TASK IS THE EXPEDITE OF AUTO & AEROSPACE PARTS THROUGH OUT THE COUNTRY. HAVE LEARNED THERE IS A SAFETY RECALL PER OEM AND NHTSA REGARDING DRIVETRAIN COUPLERS ( UNOINTS ). MY PARTICULAR VEHICLE HAS INCURRED DAMAGE AS RESULT OF COUPLER(S) FAILING. DAMAGE INCLUDING; NOT LIMITED TO DRIVESHAFT BECOMING DETACHED, SEVERANCE OF FUEL & BRAKE LINES, BREACH OF TRANSMISSION BELL HOUSING, OTHER. CATASTROPHIC FAILURE LEFT ME STRANDED IN DESERT FOR 2 DAYS, LOSS OF BUSINESS, LOSS OF INCOME, HEALTH RELATED ISSUES. THE RUBBER / POLYMER LIKE MATERIAL THESE COUPLERS ARE MADE OF ARE A SEVERE SAFETY HAZARD; ESPECIALLY IF VEHICLE IS OPERATED UNDER LOAD. FURTHERMORE, CUSTOMERS OPERATING THESE HAZARDS ARE NOT BEING NOTIFIED UNTIL NOVEMBER 11TH, 2019. SOMEONE NEEDS TO TAKE INITIATIVE BEFORE SOMEONE IS SERIOUSLY HURT. I WILL BE HAPPY TO FORWARD ANY ADDITIONAL INFORMATION/DOCUMENTS PERTAINING TO THIS MATTER. THANK YOU.
