The involved individual was found with altered mental status on a highway on 8/13/2023. The vehicle he had been driving was found in the highway median a few hundred feet away. He was admitted to a local hospital, diagnosed with blunt force injuries of the head, and pronounced brain dead on 8/14/2023. Autopsy examination revealed fractures of the skull, subdural and subarachnoid hemorrhage, and cerebral cortical contusions. Mechanical vehicle inspection revealed that the driveshaft flexible joint failed while the vehicle was in motion, with resulting driveshaft uncoupling and severe damage to surrounding vehicle components including the brake and fuel lines. Investigation was consistent with the decedent exiting the moving motor vehicle.

The contact stated while driving 70 MPH, the brake line fractured, and there was a humming noise, and the driveshaft disintegrated from the front end of the vehicle. The vehicle was towed to the residence; however, a month later, the vehicle was towed to the dealer for a scheduled appointment. The vehicle was diagnosed as having a disassembled accelerator assembly. The contact referenced NHTSA Campaign Number: 17V408000 (Power Train) however, the VIN was not included. The manufacturer was made aware of the failure, and a case was opened. The failure mileage was approximately 55,000.

The driveshaft carriage bearing that connects the two driveshafts together. The center of the carriage bearing is deteriorating and causing a bad vibrations. This has progressively gotten worse and now can feel the vibrations at any speed.

My van was called for a change of part called a coupler or quibo. The fact is that after this repair under warranty, the vehicle vibrates at 40m/h. I went back to the dealer and they told me that the toque converter was damaged and that it would cost me $3,546 to fix it. The vehicle went into recall perfectly and now it is failing due to the same repair

The vehicle also had a vibration sensation whenever the vehicle was being driven. There were no warning lights on the vehicle. The contact contacted the dealer who stated that the flex disc had failed. The contact stated that this was a known issue for the vehicle. There was a recall and lawsuit involving the vehicle's rubber cupper between the transmission and driveshaft was made of plastic was causing severe failures. The manufacturer was made aware of the matter and had since been looking into the matter and informed the contact that their research was being furthered along regarding parts before something was to be done. The contact stated that the manufacturer had failed to provide a permanent fix or the guaranteed maintenance at replacing the part at every 30,000 40,000 miles. The approximate failure mileage was 71,000.

I received a safety recall via mail for a driveshaft coupling malfunction. I had no noticeable problem with my vehicle safety nor did I have a warning light or message on the instrument panel. I turned in the vehicle to the local "Gosch" Ford dealership to perform the inspection. My vehicle was turned in on 07/21/2021 and the dealership has yet to receive the part. However, it has not been returned to me with the dealer citing the fact that it is unsafe to drive without the repair. I continue to receive the same statement that I need to wait and check back each week.

THE DRIVELINE BROKER FROM IT'S HOLDING MOUNT WHILE DRIVING AND TANGLED UP THE FUEL LINE AND BRAKE LINES GOING TO THE REAR OF THE VAN. MAKING THE VAN LOSE POWER AND NOT BE ABLE TO BE DRIVEN ANYMORE. THIS HAPPENED WHILE DRIVING ON THE FREEWAY. THIS IS EXTREMELY DANGEROUS. ALSO THEY HAVE A RECALL ON THE REAR FLEX JOINT WHICH THE FORD DEALERSHIP HAS RECOGNIZED BUT ARE REFUSING TO PAY FOR THE TOTAL REPAIR. HOW IS THAT FAIR? IT IS FORD'S DESIGN FLAW THAT MADE THIS HAPPEN.

IT ALSO SHAKES WHEN GOING FROM 0 TO 40 MPH THEN IT GOES AWAY. LUCKY IT DID NOT FALL APART. THIS RECALL MUST BE TAKEN SERIOUSLY BY FORD AND ENFORCE IT TO BE FIXED ASAP TO ALL VEHICLE WITH SUCH ISSUE.

HAVE LEARNED THERE IS A SAFETY RECALL PER OEM AND NHTSA REGARDING DRIVETRAIN COUPLERS ( UNOINTS ). MY PARTICULAR VEHICLE HAS INCURRED DAMAGE AS RESULT OF COUPLER(S) FAILING. DAMAGE INCLUDING; NOT LIMITED TO DRIVESHAFT BECOMING DETACHED, SEVERANCE OF FUEL & BRAKE LINES, BREACH OF TRANSMISSION BELL HOUSING, OTHER. CATASTROPHIC FAILURE LEFT ME STRANDED IN DESERT FOR 2 DAYS, LOSS OF BUSINESS, LOSS OF INCOME, HEALTH RELATED ISSUES. THE RUBBER / POLYMER LIKE MATERIAL THESE COUPLERS ARE MADE OF ARE A SEVERE SAFETY HAZARD; ESPECIALLY IF VEHICLE IS OPERATED UNDER LOAD. FURTHERMORE, CUSTOMERS OPERATING THESE HAZARDS ARE NOT BEING NOTIFIED UNTIL NOVEMBER 11TH, 2019. SOMEONE NEEDS TO TAKE INITIATIVE BEFORE SOMEONE IS SERIOUSLY HURT. I WAS AWARE OF THE RECALL 17S15 IN REFERENCE TO THE DRIVING SHAFT COUPLING, EVERY 30,000 MILES I CHECK THIS COUPLING AND EACH TIME THEY WOULD TELL ME THAT IT WAS CRACKED AND NEEDS TO BE CHANGE. AS SOON THE CALLED FIXED CAME OUT, IT WAS INSTALLED, BUT JUST LAST MONTH IT WAS CHECKED AGAIN AND THEY NOTICED THAT THE FAMOUS FIX ALSO WAS CRACKED AND THIS TIME THEY (LEVITTOWN FORD LONG ISLAND)TOLD ME THAT IT WAS NOT COVERED UNDER RECALL AND I HAD TO PAY OVER $800.00 DOLLAR'S FOR A NEW FIX. THAT IS THE REASON I AM PUTTING THESE COMPLAINT. THE IS REALLY DANGEROUS

DEALER PERFORMED RECALL 17S15 DRIVESHAFT ASSY THAT REPLACED THE REAR DRIVESHAFT COUPLING. AT THE TIME, THE FRONT DRIVESHAFT COUPLING WAS OBSERVED TO BE LOOSE. THE FRONT COUPLING WAS INSPECTED AND REASSEMBLED. LESS THAN 6000 MILES LATER THE FRONT DRIVESHAFT COUPLING (THE FIBROUS PART) DISINTEGRATED, THE SHAFT SEPARATED, CAUSING DAMAGE TO THE BRAKE LINES. AT THE TIME THE VEHICLE WAS TRAVELING AT HIGHWAY SPEED AND DOWN HILL. THE DRIVER BROUGHT THE VEHICLE TO A STOP USING THE EMERGENCY BRAKE. PRIOR TO THE EVENT, THE DRIVER HAD NOTICED A LIGHT VIBRATION AND SOUND FOR ABOUT 50 MILES. THE VIBRATION SUDDENLY GOT WORSE JUST BEFORE THE EVENT.

THE DEALER STATED THAT THE VEHICLE SHOULD HAVE BEEN REPAIRED PER NHTSA CAMPAIGN NUMBER: 17V408000 (POWER TRAIN). THE CONTACT STATED THAT THE RECALL NOTICE WAS NOT RECEIVED. THE VEHICLE WAS NOT REPAIRED. THE FAILURE MILEAGE WAS APPROXIMATELY 119,000.
