Sudden and without any warning, complete catastrophic brake failure while stopped attempting to pay at the pay station to exit a parking garage. My truck was fully stopped, in drive with my foot on the footbrake pedal, and started moving forward and suddenly lurched thru the parking exit gate and damaging it and the front end of my truck. Fortunately, no one was crossing on the sidewalk in front my truck or they would have been injured or worse, or we would have T-boned by oncoming traffic on the street. There was brake fluid in several areas where I parked the vehicle on the street after moving from the parking garage. There was a "low brake fluid warning light" after having the truck towed to Haley Ford in Chester, VA. There were warnings prior to the accident. The truck had had a "The Works" service done at Haley Ford the week prior. Haley Ford claims the front brake lines were leaking and need to be replaced after their "diagnostic" inspection.

Unknown, I was pulling into a parking spot, at Sam's Club, and lightly tap the gas petal for the turn and the truck lunched forward in the one carlink, and hit a concrete parking pole (Sam's club in Southgate Michigan has security cameras) it warped around the pole and bent the frame as if though it was 90 mph and a telephone pole.. I had taken it in twice under warranty for the gas pedal not working correctly, the warranty company would not allow a 3rd repair nor did Ford dealership take that warranty company. The airbags did not deploy, and I now have a T4 fractured vertebrae.

The driver continuously depressed the brake pedal, but it failed to respond. The driver crashed into a mailbox, brick decorations, a stop sign, a golf cart, a gas line, and a tree. The driver pulled the emergency brake, and the vehicle stopped.  There were no warning lights illuminated. The vehicle was towed to a collision center. The contact sustained injuries to her hip, but medical attention was not provided. A police report was filed. There was no reported fire or air bag deployment. The local dealer was not contacted. The contact owns a 2016 Ford F-150. The contact stated that as he was stopping the vehicle in front of his garage and slowing to approximately 3 MPH, the vehicle was not stopping. The contact stated that the brake pedal went to the floorboard when depressed. The vehicle crashed into the garage door causing the garage door to buckle. There was minor damage to the vehicle's front end. There were no injuries. The contact had taken the vehicle to a local dealer, where it was diagnosed and determined that the brake master cylinder needed to be replaced. The vehicle was not repaired. The manufacturer was not informed of the failure. The contact researched online and related the failure to NHTSA Campaign Number: 22V150000 (Service Brakes, Hydraulic). The failure mileage was approximately 83,000.

However, the brake pedal was inoperable and the vehicle did not stop. Due to the brake pedal failure, the contact rear-ended the vehicle in front of him. No damage was done to either vehicle. The vehicle failed to stop immediately rear-ending another vehicle. No air bags deployed. The contact sustained injuries to his neck and back but did not receive medical attention. A police report was filed. Additionally, the contact stated the vehicle consumed an excessive amount of oil and the key fob failed to function as needed. The vehicle was taken to the dealer; however, the vehicle was not diagnosed nor repaired by the dealer. The manufacturer was not made aware of the failure. The failure mileage was approximately 85,000.

Downshifting while I was driving happened without warning. Besides the gear switch, I noticed problems with the speedometer, erratic gear shifting, and the transmission stuck in one gear. I could have caused a major accident by the vehicle as I was at moderately high speed in motion when the vehicle began to fail. I did also bang my head on the steering wheel when the truck began to decelerate on its own! Mechanics told me it was a problem with the OOS (Output Speed Shaft Sensor). I could have got in a major accident had I not had a quick reaction to the truck automatically downshifting while at a high rate of speed. There is currently NO recalls for the 2016 F-150 model however, I did research and found that there are issues recalls for this VERY same problem for older F-150 models. This is an incredibly dangerous problem. My truck at this time only has 66,000 miles. Not very many miles on it for a 2016 to have this type of issue. Major research needed to get this resolved asap! People’s lives are at stake with this truck!

On 12/20/21, while driving home on State Route 7 from work, the low brake fluid warning appeared. I tried the brakes and as I tried to slow down, the pedal went all the way down. I almost went off the road but after pumping it was able to slow a bit. I was merging onto Interstate 8 West and slowly drove to figure out where to get off. The best off ramp was Imperial since it was uphill and managed to pull over after getting off and stopping the truck. Replaced the lost liquid and my brother in law followed me to his home. Same issue with brakes and slowly took it to his home. Took the vehicle to Ford dealer assuming that a recall on F150 due to master cylinder failure and leaking fluid onto the booster, which occurred to my vehicle that it would be covered in good faith by Ford. My VIN was not listed although the issue was similar to the recall. After calling the NHTSA and Ford, there was nothing else that I could do at that time but pay the service. My complaint is that I almost had an accident due to an issue that Ford had recalled various trucks and did not include my truck. The least that Ford can do is in good faith and that it was not an accident due to this failure, that repayment is made on the cost to repair master cylinder and booster.

Over $4,000 damage to F150 and trailer, and $1,066 to replace Master Brake Cylinder and Brake Booster. Gaylord Ford (Bill Marsh) 1928 Otsego Ave, Gaylord, MI 49735 (989) 732-4835 SO#927036 diagnosed: Master Cylinder leaking into Brake Booster Safety Recall 20S31. Owner advised F150 VIN was manufactured Oct 2016, 3 months after 20S31 cut-off August 1, 2016. June 8, 2020 Ford Motor Company Recall 20S31 Chronology page 2: “Ford proposed conducting a Customer Satisfaction campaign to repair master cylinders if and when they fail and to continue monitoring the population of non-recalled vehicles. Due to the severity of this case at high-speed and potential for fatal and life changing injuries, full brake failure and none of the warning scenarios described by Ford in 4 items below; reasonableness on any public safety business standard would ‘red flag’ this wurst case scenario for investigation and review. Chronology page 1, ¶4 - Loss of Brake Fluid: None of the following were observed. In this case “Brake” illuminated on dash and then not illuminated to normal. Total brake failure followed 20 minutes later without any of these 4 Ford described scenarios a driver is to expect. 1) “the driver will receive an audible chime, a full screen message center alert, and a red brake warning indicator in the instrument cluster; all indicating that brake system service is required.” 2) “Braking ability would still be unchanged at that time.” 3) “If a vehicle continues to be operated and additional fluid is depleted, the driver may begin to experience a change in brake pedal travel and feel.” 4) “if a loss of brake fluid is substantial enough to reduce brake function to the front wheels, full braking function would remain in the rear wheel circuit. Indisputable evidence the failure is grossly worsened beyond 20S31 cut-off now into Oct 2020.

THE CAR WAS PARKED IN THE PARKING LOT. I STARTED TO BACK UP AND FOUND I HAD NO BRAKES. THE BRAKE PEDAL WENT TO THE FLOOR AND I WAS UNABLE TO STOP THE TRUCK. THE TRUCK THEN RAN INTO THE VEHICLE IN A PARKING BEHIND THE TRUCK. THE BRAKES WERE CHECKED BY LIEUT. EDWARDS OF THE POLICE DEPARTMENT AND HE FOUND IT THERE WERE NO BRAKES AND THERE WAS NO FLUID IN THE MASTER CYLINDER. I OBSERVED THAT THE BRAKE FLUID RESERVOIR FOR THE FRONT BRAKES WAS COMPLETELY EMPTY AND THE RESERVOIR FOR THE REAR BRAKES HAD SLIGHTLY VISIBLE FLUID IN THE RESERVOIR. PRIOR TO THE EVENT THERE WAS NO WARNING INDICATOR ON THE DASHBOARD OR ANY OTHER WARNING THAT THE BRAKES WERE GOING TO FAIL. THE BRAKE PEDAL WOULD GO COMPLETELY TO THE FLOORBOARD WHEN PUSHED. THE FORD F150 BUMPER WAS SIGNIFICANTLY DAMAGED AND THE PARKED CAR WAS SLIGHTLY DAMAGED. WE ARE NOW IN THE MOUNTAINS OF NORTH GEORGIA AND IF HAD WE BEEN GOING DOWN THE ROAD TO THE VALLEY WE POSSIBLY BEEN SERIOUSLY INJURED OR WORSE. I REPORTED A PREVIOUS EVENT WITH THIS TRUCK TO YOU FOLKS LAST YEAR RELATING TO THE COMPUTER SYSTEM THAT CONTROLS JUST ABOUT EVERYTHING ON THE TRUCK CAUSING A COMPLETE SHUTDOWN OF THE VEHICLE. AT THAT TIME I WAS DRIVING DOWN THE MOUNTAIN ROAD AND FORTUNATELY I WAS ABLE TO STOP THE VEHICLE. THE VEHICLE AT THAT TIME WAS TOWED AWAY FOR REPAIRS AS WE DID THIS TIME AFTER THE BRAKE FAILURE. IT IS MY UNDERSTANDING THAT THE BREAK FLUID RESERVOIRS HAVE LEVEL INDICATORS THAT SHOULD GIVE SOME WARNING. THEY APPARENTLY ARE INOPERABLE.

BEING THE DRIVER OF A 2016 F-150 CREW CAB 4X4 WITH 3.5 ECO BOOST MOTOR. I HAVE BEEN NOW BEEN INVOLVED IN TWO BRAKE FAILURE RELATED COLLISIONS. ONE ON JUNE 20, 2017 AND THE OTHER THIS YEAR ON JULY 3RD. THE FIRST BRAKE RELATED COLLISION WAS ON JUNE 20TH, 2017. I WAS STOPPED AT A TRAFFIC LIGHT BEHIND SOME OTHER CARS WAITING ON LIGHT TO CHANGE. NEXT THING I KNOW I FEEL A SLIGHT BUMP HITTING THE CAR IN FRONT OF ME SO I GO TO HIT THE BRAKE AGAIN AND INADVERTENTLY HIT THE ACCELERATOR ENDING UP PUSHING THE CAR IN FRONT OF ME INTO THE CAR IN FRONT OF IT. WHICH CAUSED A 3 VEHICLE ACCIDENT THAT DAY. NOW THIS JULY 3RD I WAS INVOLVED IN A MUCH SERIOUS ACCIDENT DUE TO BRAKE FAILURE. I WAS DRIVING IN FAR LEFT LANE APPROACHING A TRAFFIC LIGHT THAT WAS RED AND CARS IN FRONT OF ME STOPPED AHEAD. I WENT TO HIT MY BRAKES TO STOP AND THE TRUCK KEPT ON GET GOING IN A FORWARD MOTION WITHOUT ANY INDICATION THE BRAKES WERE WORKING. I TRIED TO DIRECT TRUCK TO THE LEFT SIDE MEDIAN TO AVOID A COLLISION AND WHILE IN FORWARD MOTION I MADE IMPACT WITH THE CAR IN MIDDLE PUSHING IT INTO THE FIRST CAR UP FRONT THAT WAS INVOLVED IN MY SECOND ACCIDENT. MY TRUCK'S MIDDLE AND RIGHT FRONT MADE CONTACT WITH CAR IN FRONT OF ME HARD ENOUGH TO PUNCTURE GAS TANK ON THE LEFT SIDE OF THAT CAR WITH MY FRONT MOUNTED TRAILER HITCH AND PUSH IT INTO THE 1ST CAR'S REAR END. MY DRIVERS SIDE STEERING WHEEL AIR BAG DEPLOYED HITTING ME IN THE STOMACH AREA AND SEAT BELT FASTER ON DRIVERS SIDE BRUISED MY RIGHT HIP, AND NECK ENDED UP WITH SOME MUSCLE STRAIN AS WELL. THE DRIVER OF THE MOST FORWARD CAR WENT TO THE HOSPITAL WITH UNKNOWN INJURIES WHILE I WAS TENDING TO THE LOCAL AUTHORITIES INVESTIGATION AND MY TRUCK. THE DRIVER IN MIDDLE CAR, THE CAR I STRUCK I KNOW THAT THEIR CAR WAS TOTALED WITH SEVERE DAMAGE TO BOTH FRONT AND REAR DAMAGE. WEATHER CLEAR AND ROADS DRY. THERE WAS NO TRACE OF SKID MARKS ON THE ROAD.

PUSHED BRAKES ALL THE WAY TO THE FLOOR AGAIN AND AGAIN TO NO AVAIL. TRUCK JUMPED THE CURB AND SMASHED INTO THE WALL. CRASH WARNING LIGHTS/ ALARM DID NOT GO ON AND THE AIRBAG DID NOT DEPLOY, THOUGH THE AIRBAG LIGHT CAME ON. TRUCK FILLED WITH SMOKE...BUT CLEARED OUT RELATIVELY QUICKLY. BRAKES SEEMED FINE AFTER THAT....IN THE SHOP NOW...SEE WHAT FORD HAS TO SAY.

IN NOVEMBER 2016 JUST 8 MONTHS AFTER OUR TRUCK WAS PURCHASED MY HUSBAND WAS TRYING TO STOP AT A RED LIGHTIN OUR NEIGHBORHOOD AND THE BRAKES WENT ALL THE WAY TO THE FLOOR AND DI'D NOT STOP, THE BRAKES DID NOT WORK. LUCKILY THERE WAS NOT AN ACCIDENT OR INJURY THIS TIME. WE TOOK THE VEHICLE TO FORD IN GRAPEVINE AND THEY TOLD US THE MASTER CYLINDER IN THE. TAKES NEEDED TO BE REPLACED SO THEY REPLACED IT. 3 MONTHS LATER IN MARCH 2017 MY HUSBAND WAS IN TRAFFIC TURNING AT A LIGHT TO MERGE ONTO THE HIGHWAY, TRAFFIC CAME TO A STOP AND HE TRIED TO STOP AND AGAIN THE BRAKES WENT ALL THE WAY TO THE GROUND AND THEY DIDN'T WORK NOR DID THEY STOP THE VEHICLE. THIS TIME HE REAR ENDED A LADY IN AN SUV. HE SAID HE HAD PLENTY OF TIME TO STOP AND WAS LUCKILY GOING AT A REDUCED SPEED BUT STILL WAS UNABLE TO STOP OR SLOW BEFORE COLLIDING WITH ANOTHER VEHICLE DUE TO BRAKE MALFUNCTION. THE TRUCK IS NOW AT GRAPEVINE FORD AGAIN AND WE HAVE YET BEEN TOLD WHAT THE ISSUE WAS. BOTH TIMES THESE INCIDENTS OCCURRED THERE WAS NO WARNING, NO LIGHTS, NOTHING THAT WE NOTICED THAT COULD INDICATE AN ISSUE. WE HAVE HAD TONS OF OTHER ISSUES WITH THIS TRUCK, DRIVERS SIDE SEATBELT BROKE, AIR CONDITIONING UNIT BROKE, SKYLINK SYSTEM SHORTED AND BRAKE FAILURE. WE HAVE BEEN REFUSED A REPLACEMENT VEHICLE BY FORD AND ARE CURRENTLY DEALING WITH LAWYERS BUT THAT COULD TAKE 3 MORE MONTHS. I AM FURIOUS THAT NO CONSIDERATION HAS BEEN GIVEN TO ME AND MY FAMILY OVER THESE ISSUES. HAVE THEY FIXED OUR VEHICLE? SURE, BUT I AM TERRIFIED TO PUT MY BABIES IN THIS TRUCK EVER AGAIN NO MATTER FIXED OR NOT. THE NEXT TIME WE MAY NOT BE SO LUCKY AND I CAN NOT TAKE THAT CHANCE. THIS IS A WORK AND FAMILY VEHICLE AND OUR WORK/FINANCES HAVE BEEN SEVERELY COMPROMISED AS WE PAY A LOT OF MONEY FOR THIS TRUCK. I RECENTLY PURCHASED A NEW 2016 FORD F150 XLT 2.7L ECOBOOST TRUCK. ON DECEMBER 6, 2016 I WAS INVOLVED IN AN ACCIDENT WITH THIS VEHICLE. I FEEL THAT THIS TRUCK DID NOT ADEQUATELY STOP IN AN EMERGENCY SITUATION WITH AMPLE STOPPING DISTANCE. I HAVE DRIVEN TRUCKS OF THIS SAME SIZE TOWING A BOAT AND UNDER AN EMERGENCY SITUATIONS GOING DOWNHILL AND HAVE BEEN ABLE TO STOP WITH LESS DISTANCE THAN I HAD WITH THIS TRUCK TOWING ABSOLUTELY NOTHING. I WAS NOT SPEEDING AND ONLY TRAVELING BETWEEN 30 TO 35 MILES AN HOUR ON A FOUR LANE HIGHWAY WHICH IS CURRENTLY UNDER CONSTRUCTION, THE SPEED LIMIT FOR THIS AREA IS 45 RIGHT NOW BECAUSE OF THIS CONSTRUCTION. I WAS TRAVELING IN THE LEFT HAND LANE PLANNING TO MAKE A LEFT HAND TURN ONTO A ROAD THAT WAS A FEW MILES AWAY AT A MAIN INTERSECTION. SEVERAL VEHICLES AHEAD OF ME SOME INDIVIDUAL DECIDED THEY WERE GOING TO JUST MAKE A LEFT HAND TURN TO GET TO A STREET CLEAR ON THE OTHER SIDE OF THE HIGHWAY CAUSING EVERYONE BEHIND THEM TO SLAM ON THEIR BRAKES, HAVING TWO TO THREE CAR LENGTHS IN FRONT OF ME I HAD SLAMMED ON THE BRAKES AND THE TRUCK WAS SLOWING DOWN BUT NOT FAST ENOUGH SO THEN I PUT MY OTHER FOOT ON THE BRAKES AND STOOD ON THE BRAKES HOLDING THE STEERING WHEEL AND STILL THE TRUCK WAS NOT STOPPING ABSOLUTELY NO CHANGE IN THE RESPONSE OF IT BREAKING, KEEP IN MIND I WAY AROUND 235 LBS. I JUST DON'T UNDERSTAND WHY MY BRAND NEW TRUCK WITHOUT EVEN HAVING ITS FIRST OIL CHANGE HAS FAILED TO STOP AND EVERY OTHER VEHICLE ON THE ROAD WITH LESS DISTANCE THAN ME WAS ABLE TO STOP . I WAS ALSO PONDERING IF THE ECOBOOST SYSTEM MAY HAVE CONTRIBUTED TO IT SOMEHOW WITH THE START AND STOPPING OF THE MOTOR CAUSING SOME SORT OF PRESSURE LOSS IN THE SYSTEM. I NO LONGER TRUST THIS VEHICLE, I HAVE DRIVEN FORD TRUCKS FOR OVER 20 YEARS. *TR

TL* THE CONTACT RENTED A 2016 FORD F-150. WHILE DRIVING APPROXIMATELY 60 MPH, THE BRAKE PEDAL TRAVELED TO THE FLOOR. THE VEHICLE FAILED TO COME TO A COMPLETE STOP AND CRASHED INTO A GUARDRAIL. NONE OF THE AIR BAGS DEPLOYED. THE DRIVER SUSTAINED BACK, NECK, AND SHOULDER PAINS. THE FRONT PASSENGER SUSTAINED BACK AND SHOULDER PAINS. THE REAR PASSENGER ON THE DRIVER SIDE SUSTAINED ANKLE BACK AND NECK PAINS. THE REAR PASSENGER ON THE PASSENGER SIDE SUSTAINED BACK AND SHOULDER PAINS. A POLICE REPORT WAS FILED. MEDICAL ATTENTION WAS REQUIRED FOR ALL THE OCCUPANTS IN THE VEHICLE. THE INSURANCE COMPANY TOWED THE VEHICLE TO A COLLISION CENTER TO REPAIR THE DAMAGES, BUT THE VEHICLE HAD NOT YET BEEN DIAGNOSED BY THE DEALER. THE MANUFACTURER WAS NOT NOTIFIED. THERE WERE TWO INJURIES THAT REQUIRED MEDICAL ATTENTION. THE OCCUPANTS OF THE OTHER VEHICLE EXPERIENCED SOME NECK DISCOMFORT. A POLICE REPORT WAS NOT FILED. BACKING UP INTO A PARKING SPOT. HIT THE BRAKES AND THE BRAKES FAILED. DID NOT STOP THE TRUCK. BACKED INTO A POLE WHICH STOPPED THE TRUCK. WAS IN A PARKING LOT. BACKING INTO A MARKED SPOT ON THE LOT. FORTUNATELY THERE WAS A POLE RATHER THAN A CAR OR PERSON THERE. THE POLE STOPPED THE VEHICLE. TOLD MY WIFE ABOUT IT. SHE DIDN'T BELIEVE ME OF COURSE - I GOOGLED BRAKE FAILURE AND THEN 4 DAYS LATER I SAW THE REPORT ON THE NEWS.

WHILE IN PARK WITH THE DRIVER DOOR OPEN, TRUCK OFF, RADIO ON, CONFIRMED SHIFTER WAS IN PARK POSITION. I ACCIDENTALLY ENGAGED MANUAL TRANSMISSION CONTROL IN FRONT OF AND GEAR SELECTOR AT THUMB POSITION OF SHIFTER AS BOTH ARE UNAVOIDABLE. AS I WAS GETTING OUT TO REACH SOMETHING IN THE BACK SEAT, TRUCK RELEASED PARK AND ROLLED AWAY. IN ADDITION, WHILE DRIVING IN D (DRIVE) THUMB ACCIDENTALLY MADE CONTACT WITH (-) CONTROL AND QUICKLY DECELERATED TO A QUICK STALLED STOP, BEGINNING AT 50 MPH DOWN TO A STOP IN THE ROAD IN LESS THAN ONE QUARTER OF A MILE. M (MANUAL) SELECTOR IS AN OPTION ON THE PANEL, HOWEVER, BOTH THESE EVENTS HAPPENED WHILE IN P AND D, RESPECTIVELY. IN PARK THE TRUCK WAS IN A STALL AND STOPPED. IN D THE TRUCK WAS TRAVELING AT 50 MPH. THIS IS A SOFTWARE MALFUNCTION THAT IS ALLOWING AN OVERRIDE OF BRAKING WHILE IN PARK AND RAPID SHIFTING AT THE TOUCH OF ONE CONTROL THAT SITS UNDER THE DRIVERS THUMB.
