The contact owns a 2015 Toyota Yaris. The contact stated while driving at an undisclosed speed, the seat belt light flashed on the instrument panel, indicating that the seat belt was not being worn. The air bag warning light was illuminated. The vehicle was taken to the dealer, who diagnosed a failure with the wiring harness. The vehicle was repaired, but the failure reoccurred. After investigating the failure, the contact related the failure to NHTSA Campaign Number: 19V319000 (Air Bags, Electrical System) but the VIN was not included. The manufacturer was not notified of the failure. The failure mileage was 56,000.

TL* THE CONTACT OWNED A 2015 TOYOTA YARIS. WHILE THE CONTACT WAS MAKING A LEFT TURN, ANOTHER VEHICLE CRASHED INTO THE FRONT END OF HER VEHICLE. THE FRONT END WAS SEVERELY DAMAGED, BUT THE AIR BAGS DID NOT DEPLOY. THE VEHICLE WAS DESTROYED AND TOWED FROM THE SCENE. A POLICE REPORT WAS FILED. THE DRIVER SUSTAINED FULL BODY AND FACIAL CONTUSIONS, WHICH REQUIRED MEDICAL ATTENTION. THE CAUSE OF THE NON-DEPLOYMENT OF AIRBAGS DRIVER OF FAILURE WAS NOT DETERMINED. DRIVER OF CAR THAT HIT ME ADMITTED FAULT. THE DEALER AND MANUFACTURER WERE NOT NOTIFIED OF THE FAILURE. THE FAILURE MILEAGE WAS 35,000. THE VIN WAS UNKNOWN. *TT*JB

INSTALLED FACTORY NEW CONTROL MODULE FROM TOYOTA, HAD NALLEY TOYOTA, ROSWELL, GA RESET THE AIRBAG. HALLEY DETERMINED THAT THE NEW CONTROL UNIT WAS DEFECTIVE AND REPLACED THE BRAND NEW UNIT WITH ANOTHER NEW UNIT. AT THAT TIME THEY DETERMINED THAT AN AIRBAG THAT DID NOT INFLATE ALSO HAD TO BE REPLACED. TWO DEFECTIVE UNITS IN AN AIR BAGS SYSTEM IS A POTENTIALLY FATAL SITUATION. REPAIR WAS DONE AT A BODY SHOP. I HAVE THE AIRBAG THEY DID NOT RETURN THE DEFECTIVE CONTROL MODULE. PART NUMBER 805736514P3K
