What 302 owners told NHTSA about the 2015 Toyota RAV4 Plug-in Hybrid

On March 14, 2022 ( about 5:05pm)got in my car to drive to the post office to mail a package. ( less than one mile from my home) I noticed when I started the car that the computer appeared to be rebooting. It was grey and had a line at the bottom appearing to progress from left to right. I drove to post office, parked, turned off the car, locked it and went into post office. Waiting in line, a man came in and said whomever owns the silver RAV4 out front, your motor is smoking. I said that’s mine, went outside in shock, called 911 and the motor was in flames within minutes. I first noticed flames coming through crack in hood by the windsheild. I have no idea why this happened. Owned the car since 2018, never had a single issue. Car is totaled. Progressive Car insurance is investigating and will hopefully approve a fair settlement, minus my deductible.This is the most bizarre thing I”ve ever seen. Please advise if you know what I can do to at least get my deductible refunded from Toyota. Thank you By the way, I almost went to the gas station first, my tank was on E, it could have been a deadly situation had i done that first. Ps. I have a video of the fire that would not upload. Road conditions were perfect - sunny, dry, nice weather. I was pulling into a parking spot and when I applied the brakes, they FROZE. I.e. the brake pedal DID NOT GO DOWN. (The mats were not an issue). Then the car suddenly accelerated and went over an embankment. I was able to stop the vehicle using the emergency brake.

The electrical system failed and the car has been towed by State Farm Insurance to Cop-art Savage Vendor in Elgin Il today. My safety was at risk by fast rapidly engulfing flames coming from under the car hood.The problem has not been reproduced or confirmed by a dealer or independent service center that we know of. Vehicle or components have been inspected by police, and might have been inspected by our Insurance after the vehicle was towed. There was a warning battery light just before smoke came out from under the hood, then the car stopped and then flames started to come out from under the hood engulfing the front part of the car.

My wife was driving her 2015 Toyota Rav4 on September 13, 2021 when the car stalled while on the interstate. She coasted to the side. It started back up so she continued to work which was off the next exit. As soon as she entered the parking lot, the Rav4 stalled again and she was able to coast in to a parking lot. As soon as she walked inside the building the Rav4 caught on fire. There were no warning lights are any indication of anything wrong with the vehicle up until this point. According to the dealer and the insurance company, the battery caught on fire. What exactly caused this I do not know. The Insurance company deemed the vehicle a total loss per the Toyota Dealers recommendation.

Slow down to make a right turn, the car completely shut off. Put on emergency lights pulled into nearest safest parking lot, braking systems failed, finally put car in park. Pulled keys out of ignition. Took a look around nothing was physically wrong, got back into vehicle started it, with no problem. Took it through the same parking lot, through the drive-thru. Started smoking at the second window, pulled back into the parking lot, shut off vehicle. Got out of vehicle to access smoke, fire started, got back int car then an explosion. Grabbed my daughter and my things the best I could. There was about two or three more explosions afterwards, the fire department came to take out the flames; this was after other employees from the same area tried to put out the fire with fire extinguishers but it wasn’t much help. It halfway burned into the ground, the Fireman had to use the jaws of life to get the hood off of the car to extinguish the fire.

While driving through the drive-thru at McDonalds his wife was notified by an employee of an abnormal burning odor. While pulling out of the drive-thru, the vehicle stalled. The driver restarted the vehicle and pulled into a parking space and the vehicle caught on fire underneath the hood. The contact stated that after his wife removed the child from the vehicle there were a series of explosions. The Fire Department was called out and the fire was extinguished. A fire report was filed. The vehicle was towed from the scene and deemed a total loss. There was no injuries and no medical attention was needed. The dealer and the manufacturer were not contacted. The approximate failure mileage was 90,000.

They noticed smoke coming from underneath the hood of the vehicle. The contact opened the hood of the vehicle to cool the vehicle down however in about 1-2 minutes they noticed flames near the engine of the vehicle. The fire department was called and put out the fire. A report was not filed. There were no injuries and no medical assistance needed. The vehicle was towed. The contact was waiting on the status of the vehicle from their insurance report. The dealer was not made aware of the issue. The manufacturer was not made aware of the issue. The approximate failure mileage was unknown.

Was driving along when some warning lights came on, specifically the check engine light, traction control off light and the AWD light. 2 seconds later I heard a loud pop, saw smoke coming from the hood. I pulled over and looked in the engine compartment and saw flames. I suspect the coolant line popped and caught fire. My safety could have been at risk if the battery exploded or oil/gas components were to catch fire. The car is currently at a dealer to be inspected. A NHP accident report was filled out and can be available if needed.

THE BATTERY CABLE CLOSEST TO THE ENGINE WAS ON FIRE, AND HAD MELTED ONE CORNER OF THE BATTERY. A PASSERBY HELPED EXTINGUISH THE FIRE, AND THE FIRE DEPARTMENT WAS CALLED TO INSPECT AND MAKE SURE THE FIRE WAS OUT. EARLIER THAT DAY, THE VEHICLE STALLED IN THE SAME WAY WHILE DRIVING APPROXIMATELY 5 MPH IN A PARKING LOT, BUT THE POWER CAME BACK IN A FEW SECONDS. IMMEDIATELY PRECEDING THAT WAS A 1-HOUR DRIVE AT INTERSTATE HIGHWAY SPEEDS WITH NO TROUBLES.

I PULLED OVER AND TURNED OFF THE CAR, WHEN WE GOT OUT OF THE CAR WE NOTICED THERE WERE FLAMES STARTING TO COME OUT FROM UNDER THE HOOD. WE IMMEDIATELY GRABBED OUR ITEMS, PURSE, PHONE, COATS AND GOT AWAY SAFELY FROM THE CAR. WE CALLED 911 AND WITHIN A FEW MINUTES THE FIRE DEPARTMENT ARRIVED AND PUT OUT THE CAR. THE WHOLE FRONT OF OUR CAR AND DASH WERE BURNED AS WELL AS PART OF THE DRIVERS SEAT. OUR INSURANCE STATED THE CAR WAS TOTALED. OUR CAR WAS INSPECTED IN DECEMBER AND HAD LESS THAN 5,000 MILES. WE PURCHASED THE CAR NEW FROM TOYOTA IN 2015 AND HAD IT SERVICED BY THE DEALER EVERY 6 MONTHS. THE FIRE DEPARTMENT SAID LEAVES HAD CAUGHT FIRE, BUT IT HAD RAINED THAT DAY AND THE ENGINE SHOULD NOT GET HOT ENOUGH TO LIGHT LEAVES ON FIRE. AN ENGINE FIRE ON A 2015 CAR WITH LESS THAN 5000 MILES SHOULD NOT HAPPEN.

THERE WERE ALSO NO SIGNS OF INTRUSION BY RODENTS OR OTHER ANIMALS. THIS HAPPENED WITH NO WARNING. I'VE ALSO SINCE LEARNED THIS HAD HAPPENED ONLY ONE MONTH BEFORE TO THE PARENTS OF ONE OF OUR CHILDRENS' FRIENDS. THEY ALSO HAD A RAV4, ALTHOUGH NOT THE SAME MODEL YEAR (I BELIEVE IT WAS A 2013). I HAVE ALSO SEEN MANY OTHER REPORTS WHILE RESEARCHING THIS ISSUE. IF THERE IS A DEFECT OR DESIGN FLAW CONTRIBUTING TO THESE FIRES, IT SHOULD BE ADDRESSED QUICKLY!

THE POWER STICK SHIFT MOVES TO EASILY OUT OF GEAR. IT REQUIRES ONLY A LIGHT DEPRESSING OF THE BREAK TO DISENGAGE IT. ON MAY 17 2019, I HAD BEEN PARKED WITH THE CAR ON. I HAD DROPPED SOMETHING ON THE FLOOR AND WENT TO PICK IT UP. MY FOOT WAS LIGHTLY TOUCHING THE BREAK AS I BENT DOWN. THE BREAK WAS DEPRESSED JUST ENOUGH TO ALLOW MY SHOULDER OR BACK TO HIT THE STICK SHIFT AND PUSH IT INTO REVERSE. UNBENOUNCED TO ME, AS I WAS STILL BENDING OVER TO PICK SOMETHING UP, THE CAR WAS PUSHED INTO REVERSE AND MOVED BACKWARDS INTO TWO CARS BEHIND ME. THIS COULD HAVE BEEN A REALLY TRAGIC SITUATION HAD SOMEONE BEEN BEHIND ME IN THE PARKING LOT. OR WORSE, IF I HAD A CHILD IN THE CAR AND I HAD GOTTEN OUT TO GET SOMETHING FROM THE TRUNK AND THE CHILD JUMPED INTO THE DRIVERS SEAT. I AM NOT VERY TALL, AND SO A CHILD COULD EASILY DEPRESS THE BRAKE JUST ENOUGH TO RELEASE THE STICK SHIFT AND COULD HAVE RUN ME OVER. THERE SHOULD BE A SECONDARY RELEASE BUTTON ON ALL STICK SHIFTS OR THE BRAKE PEDDLE SHOULD NEED TO BE DEPRESSED ALL THE WAY. THIS INCIDENT DAMAGED THREE CARS. AND WHILE NOT MY FAULT AS IT WAS A DEFECT OR SAFTEY ISSUE WITH THE CAR, MY INSURANCE HAS NOW BEEN DINGED FOR IT. I HAVE REPORTED IT TO TOYOTA BUT THEY DO NOT SEEM TO CARE.
