While parked and shut off at a gas station in Lakeside, CA, my 2015 Ford F‑150 XLT spontaneously caught fire under the hood. The fire appeared to originate in the fuse box area, specifically where the fuel pump relay is integrated. The incident posed significant risk, occurring near a fuel pump while my child was in the vehicle. A gas station employee assisted with extinguishing the fire. The cause is believed to be an electrical short. There were no modifications or recent repairs.

On or around May 28, 2024. My husband and, I were going to a recital. He was already in the truck.I told him. Pick me up at the mailbox.( I had mail to go in box).I started walking down the driveway(paved). The next thing, I was on the cement screaming. I had been hit, with the truck. My husband ran to help. He said, he didn’t see me. And, the beepers ( sensors)didn’t go off.Rescue was called, checked me out. Did not transport. But, I went to hospital later. Bruises and a broken right arm. Several weeks went by. I really didn’t believe the sensors,didn’t work.We were in a parking spot. But, the truck in reverse. Waited for people to go by. Sometimes it worked, sometimes it didn’t. We did see people on the camera. Recently, I was behind the truck again. The sensors did not go off! We are very cautious now. Aware of the problem. Why hasn’t this truck been in the recall lists?. The accident was visible on our ring. I have video. But could not figure out to download. I can sent by email.

6/25/24 I PUT MY 2015 F150 IN REVERSE AND GAVE IT JUST A LITTLE ACCELERATION AND IT IMMEDIATELY ACCELERATED FAST CROSSING THE STREET AND HITTING A UTILITY POLE TOTALING MY TRUCK. I DID SOME RESEARCH ON A FORD FORUM AN SAW WHERE OTHE PEOPLE HAD HAD RAPID UNEXPLAINED ACCERATION.ONE PERSON SAID IT WAS BECAUSE OF A FAULTY ACCELERATO PEDAL POSITON ALSO KNOWN AS APP. MY SAFETY AND ANYONE WALKING OR DRIVING BY MY HOUSE WERE GREATLY PUT AT RISK. THE PROBLEM HAS NOT BEEN REPRODUCED OR CONFIRMED BY ANYONE. THERE WERE NO WARNING LAMPS OR MESSAGES. THIS NIGHTMARE CAME OUT OF NOWHERE.

On September 13, 2020, my father was driving his 2015 Ford F150, with me in the passenger seat. We were hit in a high speed head-on collision on I-55/I-240 in Memphis, TN by an oncoming driver, which killed both my father (the driver), and the oncoming vehicle driver, and I was taken to the hospital with injuries in critical condition. My father was extracted from the vehicle and taken to the morgue, but when I was able to return to the truck at the impound lot, I found no drivers side airbag in the truck. While EMTs informed me that the airbag was cut, it was odd that the airbag was not still in the truck. His cause of death was blunt force trauma, and if the airbag did deploy, it would have had to have been a part of the immediate fatality. If it did not deploy, it would also have been a factor. On the passenger side, where I had been, all airbags were still attached, but cut in order to extract me. In the days immediately following the accident, my lip was swollen from hitting the airbag, but there was also a piece of shrapnel lodged in my lip, confirmed by XRay, which later came out. I am concerned about both my father's fatality, and how the airbag deployed and what role it played in the fatality, as well as the injury of shrapnel getting embedded in my lip. I believe that the airbag could be a contributing factor in both issues.

The contact stated that while driving approximately 45 MPH the braking system had malfunctioned and while pressing the brake pedal the vehicle did not stop causing the driver to lose control of the vehicle and crash into a ditch. The front end of the vehicle was damaged, but the air bags did not deploy. The driver sustained a right leg injury which did not require medical attention. The front passenger sustained a right knee injury and the rear driver's side passenger sustained and left shoulder injury, both required medical treatment. A police report was taken at the scene and the vehicle was able to be driven home. The cause of the failure was not determined. The manufacturer was notified of the failures. The local dealer was not contacted. The failure mileage was 130,000.

We are not sure what component or system failed or malfunctioned. The truck is available at this time for inspection, there wasn't much left of the truck after it caught on fire. The passengers in the truck were in danger, because they didn't realize the smoke they were smelling was coming from the truck they were riding in. It was not until one of the passengers said his arm was hot, it was then they realized the truck was on fire. The fire seemed to have started under the seat. I am not sure what caused it. There were no warning lamps or messages prior to this incident. The vehicle has been inspected by the insurance company, in which the truck is a total loss. The sheriff office and Fire Department were on the scene.

The contact depressed the brake pedal but it became hard and failed to stop the vehicle. The contact was swerving in traffic and came to a sudden halt in a rock pit. The ABS warning indicator light was illuminated. While looking under the hood, he noticed that the supply booster spring clamp was not in the correct location. The contact stated that he sustained injuries to his neck but no medical attention was provided. The vehicle was towed out of the rock pit by a relative. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not notified. The failure mileage was approximately 2,280.

I was driving 60 mph and a car pulled out in front of me causing me to Tbone the vehicle. Drivers Seat belt and airbag did not deploy causing me to hit the steering wheel then cracking my head on the wind shield. I bent the steering wheel gettin the force of the impacted and smashed the wind shield with my head causing my neck to bend. The accident has caused pain in my head, neck, left (hand, wrist, elbow, shoulder) right (hand, wrist) and chest that happen on 5/4/2022, in result of the safety mechanisms failing to properly work.

2/11/22.While at a complete stop, foot on brake, my truck started creeping forward and I kept pushing on the brake and I couldn't stop until my truck ran into the back of a semi trailer. After shutting truck off, and starting again, it appeared to work again. My 7 year old grand daughter was with me and she heard me yelling, "I can't stop, I can't stop". It has not been confirmed by dealership because it has to go collision shop on 2/14/22. No Warnings came on my dash. Unknown: On 1/2/2021 at approximately 11:30pm, we were awakened by neighbors to warn us that our home was on fire. Neighbors witnesses an explosion from our truck, that blew up most of the Ford F-150 and cause major damage to my van next to it. The fire department has investigated the explosion and listed cause of fire to Be electrical issues from the Ford F-150. Thankfully no lives were lost, but would like to see that This be investigated to avoid loss of life as a preventative measure.

The contact owns a 2015 Ford F-150. The contact stated that he parked the vehicle in the garage and as he exited the vehicle, the vehicle inadvertently started reversing on its own. The contact stated he attempted to jump back in the vehicle to depress the brake pedal but he was unable to do so as the vehicle was moving fast. The contact stated he jumped out of the way. The vehicle reversed down the driveway and crashed into a transformer across the street. The contact stated he got into the vehicle and switched gears to neutral, however, the indicator light would still indicate that the vehicle was in park. There were no warning lights illuminated. The contact sustained bruised ribs, lacerations on the arms and legs. The contact did not seek medical assistance. The dealer was notified of the failure but did not provide any assistance. The vehicle was towed to the dealer but was not diagnosed. The manufacturer was notified of the failure. The contact was awaiting to hear back from the manufacturer. The vehicle was not diagnosed nor repaired. The approximate failure mileage was 51,000. The contact stated that the vehicle was diagnosed and needed a software update. The software was updated by the dealer but the contact stated that the issues have occurred again.

THE CONTACT MENTION THAT THE RAIL TRACKS WAS HIGH AND IT STOP THE VEHICLE. THE AIR BAGS FAILED TO DEPLOY. THE CONTACT SUSTAINED A BAD WHIPLASH BUT WAS NOT SERIOUS INJURED, THE CONTACT STATED THAT THE FRONT PASSENGER AND THE REAR DRIVER PASSENGER ALSO SUSTAINED BAD WHIPLASH. NEITHER DRIVER NOR PASSENGERS SEEK MEDICAL ATTENTION. A POLICE REPORT WAS FILED. THE CONTACT ALSO STATED THAT THE FIRE DEPARTMENT WAS THE FIRST TO ARRIVED AT THE SCENE. THE VEHICLE WAS TOWED TO A TOW YARD AND LATER WAS TOWED TO AN INSURANCE TOW LOT. THE VEHICLE WAS TOTALED. THE VIN WAS INCLUDED IN NHTSA CAMPAIGN NUMBER: 20V332000 (SERVICE BRAKES, HYDRAULIC). THE MANUFACTURER NOR THE DEALER WERE CONTACTED. THE FAILURE MILEAGE WAS APPROXIMATELY 203,000.

MY FATHER WAS INVOLVED IN A SINGLE VEHICLE ACCIDENT IN SEPTEMBER 2020, WHERE HE STRUCK A UTILITY POLE WITH THE FRONT DRIVER SIDE BUMPER ON HIS TRUCK, CAUSING THE POLE TO BREAK. HIS TRUCK WAS TRAVELING AROUND 55 MPH ON A STATE HIGHWAY, WHEN HE SUDDENLY HAD TO AVOID HITTING A TRUCK THAT BRAKED IN FRONT OF HIM, CAUSING MY FATHER TO SWERVE TO THE RIGHT. HIS TRUCK WENT THROUGH A DITCH BEFORE IT CAME TO A STOP WHEN IT STRUCK THE UTILITY POLE. THE AIR BAG IN THE STEERING WHEEL CAME OUT BUT FAILED TO INFLATE AND MY FATHER HIT HIS HEAD ON THE STEERING WHEEL. HE SUFFERED WITH INJURIES WHICH INCLUDED BLEEDING ON THE BRAIN AND HAD TO BE AIR LIFTED TO A TRAUMA HOSPITAL AND HAD TO BE PUT IN ICU!! HE'S STILL UNDER THE CARE OF HIS DOCTORS

THE MANUFACTURER WAS NOTIFIED OF THE FAILURE WHERE THEY GAVE HER A CASE NUMBER, THEY OFFERED NO FURTHER ASSISTANCE. THE VEHICLE HAD YET TO BE REPAIRED. THE FAILURE MILEAGE WAS APPROXIMATELY 31,000.

BRAKE FAILURE. BRAKE PEDAL WENT COMPLETELY TO THE FLOOR AND VEHICLE DID NOT STOP UNTIL IT HIT A TREE. I PRESSED THE BRAKE, STARTED THE CAR. THE BRAKE PEDAL DID NOT FEEL ABNORMAL. WITH MY FOOT STILL ON THE PEDAL, I PUT THE VEHICLE IN REVERSE. THE PEDAL IMMEDIATELY WENT SOFT AND COMPLETELY TO THE FLOOR. SINCE I WAS IN MY DRIVEWAY ONLY PLANNING TO MOVE THE VEHICLE BACK ABOUT 3 FEET, I DID NOT CLOSE THE DOOR. WHEN THE VEHICLE DID NOT STOP, ITS DOOR CAUGHT A TREE AND BENT BACKWARDS INTO THE FRONT FENDER. THE VEHICLE STOPPED BECAUSE THE DOOR CAUGHT THE TREE. THE ONLY WARNING DISPLAYED ON DASHBOARD WAS THAT THE DOOR WAS OPEN AND THE CAR WAS NOT IN PARK. AFTER INCIDENT I TURNED ON POWER TO VEHICLE WITHOUT STARTING TO RECORD THE MILEAGE ON THE VEHICLE FOR MY INSURANCE COMPANY AND THAT IS WHEN THE LOW BRAKE FLUID WARNING CAME ON. *TR
